02 · Common mix-up
Is 15mg the same as 15 ml?
No. Milligrams measure the weight of tirzepatide in your weekly dose. Millilitres measure the volume of liquid the pen delivers. A 15mg dose is not 15 ml, and 2.5mg is not 2.5 ml.
This matters because the pen is dialled by dose, not by volume. You never need to convert mg to ml yourself. The pen does it for you, which is why the injection is set by the number showing in the window.1

04 · Reading the pen dial
How do you read the pen: clicks or the window?
You set the dose by turning the knob until the correct dose shows in the dose window. You confirm it by the number in the window, never by counting clicks.13 That is why a question like "how many clicks is 2.5mg on a 15mg pen" has no safe answer: the pen is not designed to be used that way.
The pen will not let you dial past the medicine left inside. 05 · Doses in a 15mg pen
How many doses are in a 15mg pen, and can you take smaller ones?
A 15mg pen contains four weekly doses. Each is the 15mg dose, so the pen is meant for people already prescribed 15mg weekly.1
Taking smaller or more spread-out doses than the pen is designed to deliver is called microdosing. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator, and it means unknown dosing accuracy and no safety data.13 If you want a different dose, your prescriber can step you up or down within the licence. 06 · Who 15mg is for
What is Mounjaro 15mg used for?
Mounjaro 15mg is the highest strength of tirzepatide, licensed for weight management in adults and for type 2 diabetes.1 Everyone starts on 2.5mg weekly. After at least 4 weeks the dose rises to 5mg, then can step up by 2.5mg at minimum 4-week intervals to a maximum of 15mg weekly, depending on response and tolerance.1
Higher is not automatically better. Many people settle at 5mg, 10mg or 15mg. In SURMOUNT-1 (2,539 adults), average weight loss over 72 weeks was 15% on 5mg, 19.5% on 10mg and 20.9% on 15mg, against 3.1% with lifestyle changes alone. These are trial averages; individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity.2
07 · Storage and in-use life
How long does an opened pen last?
Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13 Keep the cap on to protect the pen from light.
Unopened pens keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it.
