If you need 2.5mg, the correct route is a 2.5mg pen prescribed to you. Everyone starts Mounjaro on 2.5mg once weekly, and this starting dose is about letting your body adjust, not about weight loss itself.111 The Mounjaro pen is available in all six strengths, and the 2.5mg pen has a standard price of £169.99 per month. If you are on 5mg and struggling with side effects, do not try to engineer a lower dose with the dial. Your prescriber can hold a dose longer or step down within the licence if side effects are the concern.1 Tell them what you are experiencing, and they can decide what is appropriate for you.

03 · How the KwikPen works

How the KwikPen dial works

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Each pen contains four weekly doses, and the pen will not let you dial past the medicine left inside.113

This matters for anyone trying to take a smaller amount. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1

04 · Why part-dosing is not licensed

Why part-dosing is not licensed

Taking smaller or more spread-out doses than the pen is designed to deliver is sometimes called microdosing. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1

The licensed dose ladder already starts low for tolerability. Altering doses outside the licence means unknown dosing accuracy and no safety data.1 In plain terms, nobody can tell you what you actually injected, and nobody has studied it.

A conversation with your prescriber is always safer than a workaround.

05 · Licensed ways to go slower

Licensed ways to go slower

If the dose ladder feels too fast, the licensed options are straightforward. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111

That wording sets a minimum time on each step, not a fixed schedule. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1 Your prescriber will guide the right pace, including holding a dose for longer.

06 · Side effects and warning signs

Side effects and warning signs

Like all medicines, Mounjaro can cause side effects, and most are digestive. They are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13 Nausea, diarrhoea, vomiting, constipation and abdominal discomfort are among the most common.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.113

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.1

Do not alter the dial to manage side effects. Report them to your prescriber, and you can also report suspected side effects through the Yellow Card scheme.19

07 · Missed doses and timing

Missed doses and injection timing

The licensed rule is simple: if you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can also change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

There is no best time of day. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.1

Storage and injecting correctly

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.113

Mounjaro is injected under the skin once a week. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

Keep the cap on to protect the pen from light, and never use a pen that has been frozen.1 Needles, a sharps bin and injection guidance are included with every order from us.
