01 · The core answer
Can you take 2.5mg from a 7.5mg Mounjaro pen?
No, not safely and not within the licence. The KwikPen is set by turning the dose knob until the indicator shows the correct dose in the window, and you confirm the dose by that number, never by counting clicks.13 A pen labelled 7.5mg is made for a 7.5mg injection.
Charts or forum posts sometimes claim a precise number of clicks that gives a smaller dose from a higher-strength pen. There is no licensed click number, because the pen is set by the dose window, never by counting clicks.
The pen itself also gives a warning. It will not let you dial past the medicine left inside, and if the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.13

02 · Dose steps
Can you go from 2.5mg to 7.5mg on Mounjaro?
Not in one move. Everyone starts Mounjaro on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
So 7.5mg is the third strength on the ladder, reached through 5mg. Skipping 5mg is not part of the licensed schedule. If you want the longer discussion, read whether you can jump from 2.5mg to 7.5mg.
Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.11
03 · Next step
What to do if your pen strength does not match your dose
Contact the prescriber or pharmacy that supplied it before you inject. A mismatch usually has a simple fix: a replacement pen of the right strength, or a prescriber review of the plan. Improvising with clicks has no safety net.

How the KwikPen dial works
Mounjaro is a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each strength is a different pen, filled for that dose. Each KwikPen contains four weekly doses.1
You set the dose by turning the knob until the indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.

05 · Why click charts mislead
Why click-count charts are not reliable
Splitting doses by clicks is not licensed, and the pen is designed to be used at its labelled dose.13
The safety position is plain: taking smaller or more spread-out doses than the pen is designed to deliver is not licensed, not supported by the pen's design and not endorsed by any UK regulator, and it means unknown dosing accuracy and no safety data.1
The same logic applies if someone suggests using a higher-strength pen for a lower dose. People ask the same question about other strengths, for example using a 5mg pen for 2.5mg or a 10mg pen for 7.5mg. The answer does not change with the strength.
06 · Microdosing and gentler options
Microdosing, and what you can do instead
People usually look for click counts because they want a smaller dose, either to ease side effects or because they hold a higher-strength pen. That goal is reasonable. The method is the problem.
The licensed ladder already starts low (2.5mg) for tolerability, and your prescriber can hold a dose longer or step down within the licence if side effects are the concern.1 That is the safe version of the same idea.
Practical options to raise with your prescriber:
- Stay longer on your current strength before stepping up.
- Step back to the previous strength if the latest increase was hard to tolerate.

07 · Early weeks and pace
What the 2.5mg weeks are for
The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose. Both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days.2
In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, and individual results vary.2
Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4
08 · Injecting and storing pens
Injecting and storing your pen correctly
Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13
09 · Getting the right strength
Getting the right strength from a regulated supplier
The cleanest way to avoid pen-strength problems is to receive the exact strength your prescriber chose, from a regulated pharmacy. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber.

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
