How Many 3.75 Doses in a 5mg Mounjaro Pen? (October 2026)
A 5mg Mounjaro KwikPen contains four weekly 5mg doses, and the UK dose ladder has no 3.75 step, so the pen is not designed to deliver 3.75 doses.1 The licensed route is 2.5mg, then 5mg, then up in 2.5mg steps. What to do if 5mg feels too much
The licensed dose ladder starts at 2.5mg for tolerability. After at least 4 weeks it rises to 5mg, then in 2.5mg steps at minimum 4-week intervals.1

04 · Why splitting is unsafe
Why splitting a pen is not endorsed
Altering doses outside the licence means unknown dosing accuracy and no safety data.
The same applies to using two 2.5mg pens in place of one 5mg pen to mix doses. A 2.5mg pen is its own set of four 2.5mg doses; see how many 2.5 doses are in a 5mg pen and doses in a 2.5 pen.
05 · Timing and storage
Timing, missed doses and the 30-day limit
Mounjaro is injected once a week on the same day. You can change your injection day if at least 3 days (72 hours) have passed since your last dose.1 If you miss a dose, take it within 4 days (96 hours); after that skip it. Never take two doses within 3 days of each other.1
Once in use, a pen can stay out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 Four weekly doses fit within that window. Keep unopened pens in the fridge until the printed expiry date. For the wider picture, read how many doses are in one pen.
