What is clear is the reason the question arises: a GLP-1 or GIP/GLP-1 medicine slows how quickly the stomach empties, so surgical teams may give specific fasting or medication-pause instructions before a procedure under sedation or general anaesthetic.13
Some teams ask patients to pause the injection beforehand. Others keep the medicine going and change the fasting or preparation plan instead. Both approaches exist because the aim is the same: an empty stomach at the time of anaesthesia. Your team chooses the approach that fits your procedure and your risk, so a pre-op instruction sheet beats any general rule you read online.

Work out where you are in the process. If you have a date, contact the pre-assessment service now rather than the week before. If you only suspect you may need a procedure, such as an endoscopy or a dental extraction under sedation, ask whether sedation is planned. Then give them the exact name of your medicine, your current dose and the day of the week you inject.
Do not stop on your own initiative and hope for the best. Equally, do not carry on injecting on the day you have been told to pause. If the written instructions conflict with what you were told by phone, ask for clarification in writing before the day.

GLP-1 medicines matter for anaesthesia because they slow stomach emptying. Mounjaro's active ingredient, tirzepatide, activates two natural gut-hormone receptors, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1
For most of daily life, slower stomach emptying is part of why people feel full sooner and less hungry between meals. Before sedation or general anaesthesia, it is a different matter. Fasting rules exist so that the stomach is empty when protective reflexes are dampened. If the stomach empties more slowly than usual, the standard fasting window may not be enough for some people, which is why teams may adapt their instructions.13
It does not mean the medicine is unsafe for anyone who has surgery, and it does not mean everyone must stop. It means the team needs the information so they can plan.

You will find conflicting advice because professional guidance has not stayed fixed. Some sources describe pausing weekly injections ahead of elective procedures. Others describe continuing the medicine and adjusting the preparation instead.
Several things make a single rule hard to set. Your reason for taking the medicine matters, since the same drug is prescribed for type 2 diabetes and for weight management. Your dose and stage of treatment matter, since someone who has just stepped up may have more gut side effects. The procedure matters, because a minor procedure without sedation differs from major abdominal surgery under general anaesthetic. Symptoms such as nausea or vomiting on the day also matter to the anaesthetist.
The safest approach is to treat your own team's written plan as the answer. The product information tells you to inform the surgical team,13 and that conversation produces the hold time that applies to you.

05 · What to tell your team
What to tell your surgical team
Tell your surgical team the name of your medicine, your dose, the day you inject and when you last injected. If you take Mounjaro, say so clearly and early. If you have recently had a dose change, mention it. The product information specifically advises telling the team if you are having an operation or procedure under sedation or general anaesthetic.13
It helps to bring a short written note with you:
- The brand and generic name, for example Mounjaro (tirzepatide).
- Your current strength, and the date and day of your last injection.
- Why you take it: weight management, type 2 diabetes, or both.
- Any recent nausea, vomiting, bloating or reflux.
- Other medicines, especially anything for diabetes.
Mention the same facts at every stage: the pre-assessment call, the admission nurse and the anaesthetist on the day. Information sometimes does not travel between these stages, and the anaesthetist is the person who makes the final call on the day.
If you also take insulin or a sulfonylurea, tell the team as well. Doses of those medicines may need adjusting alongside a GLP-1 because of the risk of low blood sugar, and the team should plan that together rather than you doing it alone.
06 · Mounjaro clearance and pausing
Mounjaro stays in your system for weeks
Tirzepatide has a long half-life, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
This is important for how you think about a pause. A short break from injecting is not the same as the medicine being gone. Whatever your team decides about skipping a dose before the procedure, the medicine does not disappear on the day of the operation. It also means you should not try to engineer your own timing. 07 · What not to do
What not to do around a procedure
Do not change your dosing on your own to fit around surgery.
Never take two doses within 3 days of each other.1
Do not assume that a hold instruction means you can eat normally on the day. Fasting rules still apply, and the team may tailor them. Do not hide your medicine because you worry the operation will be cancelled.
Finally, do not rely on online summaries, including AI-generated ones, as your instruction.
08 · Scheduling your injection day
Scheduling your injection day around a procedure
If your procedure date sits awkwardly against your weekly injection, you do not have to keep the same day indefinitely. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Your prescriber can help you plan a sensible new day, especially if the team asks you to pause for a particular dose.
There is no best time of day to inject. Mounjaro works the same morning or evening, with or without food, so what matters is keeping to the same day each week.1 After the procedure, many people find it easiest to return to their usual day once the team says they can.
If you travel for treatment, for example to a hospital in another city or abroad, remember that you can fly with Mounjaro. Carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.1 Keep pens in the fridge between 2–8°C until first use, and once in use a pen can stay out of the fridge not above 30°C for up to 30 days, then must be thrown away.1
09 · Restarting afterwards
Restarting after surgery
Restarting is a joint decision between your surgical team and your prescriber, and it depends on how your recovery goes. Do not restart simply because the calendar says it is injection day. Ask the surgical team when they are happy for you to resume, then tell your prescriber so your plan stays current.
If your break was long or you had trouble with nausea, vomiting or eating after the operation, your prescriber may want to review your dose before you carry on. Dosing is stepped for tolerability: everyone starts on 2.5mg, and the dose can only rise in steps from there, depending on how you respond and tolerate it.1
Recovery can also affect appetite and nutrition. Be honest with the prescriber about how much you are eating. Weight loss is gradual by design, and any pause or stop should be planned with your prescriber. Judge progress by trend over months, not individual weigh-ins.
10 · Previous weight-loss surgery
If you have already had weight-loss surgery
Some people regain weight years after a gastric sleeve or bypass and ask about Mounjaro. It can be clinically appropriate, but it sits with an individual prescriber decision. Surgery changes gut anatomy and nutrition needs, and trial data in post-surgical patients is limited. Declare any bariatric history in your consultation.
The same openness applies to any future operation. Whether your earlier surgery was bariatric or not, a complete surgical history helps both your prescriber and any new surgical team plan around the medicine.
11 · Planning ahead and the NHS
Planning ahead if surgery is on the horizon
If you know a procedure is coming, raise it with your prescriber before you begin or step up treatment. Early weeks bring the commonest effects of treatment: mild nausea, altered appetite and tiredness in the days after each injection, which usually settle.1 Serious side effects are uncommon but need urgent attention: get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Starting or increasing a dose shortly before an operation can leave you feeling less settled on the day, so timing is worth discussing.
