This is why the often-quoted clearance time is a range rather than a single day. The drug is not switched off at a set moment; the amount in your body keeps dropping until it is negligible. The licensed summary puts that at roughly four to five weeks after a final dose.1

Why this matters for a once-weekly pen

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 A weekly schedule only makes sense because the medicine lingers. If it cleared within a day, you would need far more frequent injections.

Over the weeks that follow, the effects that depend on the medicine being present are expected to ease. Those include feeling full sooner and less hungry between meals.1 Many people therefore notice appetite coming back gradually. How strongly differs from person to person, and your prescriber can talk it through before you decide. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Can you quit Mounjaro cold turkey?

You can stop at any time, because the medicine tapers off on its own as it clears. Nothing needs doing while it clears.1 Even so, stopping is a decision worth making with your prescriber, not alone.   • Protect protein and activity, and watch portion sizes, which tend to drift back up.
  • Check the basics such as sleep and alcohol, which affect appetite.
  • Judge progress by trend, not by individual weigh-ins.

Those four points echo how plateaus are handled during treatment, and they apply just as well afterwards. If your weight starts moving in a direction you do not want, speak to your prescriber early rather than waiting.

06 · Missed doses and breaks

Missed doses, breaks and restarting

The long half-life means a late dose is rarely a crisis, and the licence gives a clear rule. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

Is it safe to restart after a break?

A longer break means much of the drug has cleared, and which dose is appropriate when you restart is a prescriber decision. Contact the prescribing team before your next injection rather than resuming at the dose you left off on.

Can you take Mounjaro every 10 days?

No. The licensed schedule is once a week; taking it less often is unlicensed and not supported.

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

07 · Side effects linger

Do side effects last after your last dose?

They can, because the medicine is still present while it clears. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If you stop, any effect tied to the drug being present should fade gradually as levels fall over the following weeks.

A side effect that persists, worsens or feels unusual deserves a conversation with your prescriber. Tell your prescriber about any new or lasting symptom. You can also report suspected side effects through the MHRA Yellow Card scheme.19

Do not assume a new symptom is caused by the medicine just because you recently took it. Equally, do not dismiss it because you have stopped. Tell a clinician what you take and when your last dose was.

08 · Pen storage vs body

Do not confuse time in your body with time in the pen

How long Mounjaro stays in your body is a separate question from how long a pen stays usable. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.1

09 · Weight and plateaus

Is it normal to gain weight while using Mounjaro?

Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Water weight, food timing and routine all move the scales, so judge progress by trend rather than a single weigh-in.

If you are not yet at the maximum dose, your prescriber may discuss a step-up. A plateau is not a reason to double doses or add unlicensed products.1 Trials measured results at 72 weeks, so think in months, not days.2

If you are gaining steadily rather than plateauing, tell your prescriber. They can review your dose, food intake and any other medicines.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed: switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.
