Mounjaro at 2.5mg stays in your system for roughly four to five weeks after your final injection. It does not depend on which strength you took.1 The reason is the half-life. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
A half-life is the time it takes for the amount in your body to fall by half. The medicine does not vanish on a set day; it tapers away gradually, and the amount left becomes smaller and smaller. Because you start low, there is less in your body after a 2.5mg pen than after a higher dose, but the clearance window stays the same.

What this means for you in practice
The practical meaning depends on which situation you are in. If you are mid-treatment, the long half-life is the reason a once-weekly injection works at all. Tirzepatide is still active between doses, so you are not meant to feel it switch on and off each week. Any small shift in appetite towards the end of a dosing week is normal, and it is not a sign that the medicine has left your body or that you should inject earlier.
If you have decided to stop, expect the effects to fade over several weeks, not overnight. Appetite may return gradually. That is the moment to agree a plan with your prescriber about eating habits, activity and whether to restart at some point.
If you are worried about a side effect, the clearance window matters because a reaction to a dose will not disappear instantly either. Half-life explained in plain English
Half-life is the single most useful idea for understanding how long Mounjaro stays in your system. It describes how quickly the amount of medicine in your body falls. After one half-life, half remains. After another, half of that remains, and so on. The amount never drops to nothing in a single step; it simply becomes too small to matter, which is why the practical clearance window is quoted in weeks.1
It also explains why a medicine taken once weekly can work steadily: each injection tops up a level that has not yet fallen away.
Why the starting dose is not special
It is tempting to think a small dose like 2.5mg must be gone in days. The half-life does not change with strength, so a smaller dose falls away at the same proportional rate as a larger one. The difference is how much medicine you begin with.
Clearance is not the same as effect
Two questions get mixed together online. The first is how long the molecule remains detectable in your body. The second is how long you notice appetite and fullness changes. These overlap, but you may feel effects fade before the medicine is fully gone, because very small amounts stop making a noticeable difference.
Why a long half-life is helpful
A long half-life is the reason the weekly schedule exists. It means steadier levels, fewer peaks and troughs, and an injection you can put in your calendar. The medicine is still active when your next injection is due, which is how the weekly schedule is designed to work.1 Even so, many people describe a rhythm across the week, and it is worth understanding why, so you do not misread it.
Why some days feel different
Appetite is influenced by sleep, stress, hormones, social events and routine, not just medication. A day when hunger feels louder does not necessarily mean the dose has run out. Early in treatment, when you are on 2.5mg, your body is still adjusting, and the effects can feel inconsistent from week to week. That is normal. The starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal.1
When it is worth speaking to your prescriber
If you consistently feel that appetite returns strongly before your next injection and it is disrupting your plan, tell your prescriber. They can review how you are doing on the dose ladder and decide whether a step up is appropriate, and that is the correct route. Injecting early, splitting a dose or adding extra injections is not licensed and is not something we can advise.
Choose a routine that fits you
There is no best time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember, and many people choose a weekend morning so early side effects fall on days off.1 A consistent day helps you notice real patterns, instead of reading noise into your week.
What about the peak?
People often ask when Mounjaro reaches its peak. What matters in practice is that levels build over successive weekly doses and stay broadly steady, not that you can feel a spike on a particular afternoon.
06 · Side effects timing
How long do side effects last on 2.5mg?
Side effects on 2.5mg are usually mild and tend to settle as your body adjusts. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1
The link with clearance
Because tirzepatide takes roughly four to five weeks to clear after a final dose, an effect linked to a dose does not vanish the moment you decide to stop. If you have a reaction that bothers you, it may ease gradually, not instantly. That is another reason to speak to your prescriber early instead of waiting for it to pass on its own.
Why early effects often peak after injection day

Picking a weekend morning, for example, means any early tiredness or queasiness falls on days off.

If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up — speak to your clinical team rather than pushing through severe symptoms. That is the safe route.

07 · Stopping and missed doses
Stopping, missed doses and changing your day
Missing or moving a dose is common, and the licence gives clear rules, so you do not need to guess. The long half-life is why a short delay does not undo your treatment.1
The missed dose rule
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
Changing your injection day
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Holidays, shift patterns and new routines are all good reasons. Move the day once, then keep to the new day each week.
What if you stop completely?
If you stop, the medicine clears over roughly four to five weeks, and nothing needs doing while it does.1 Expect appetite to come back gradually, not suddenly. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber. That is the time to lean on routines you built while treated: regular meals, protein, activity and sleep. If you stop and later want to restart, speak to your prescriber, because the right starting point depends on how long you have been off treatment and how you feel.
Will the pace of the ladder change?
Everyone starts on 2.5mg once weekly. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Your prescriber will guide the pace. 08 · Flushing and myths
Can you flush Mounjaro out? Myths and dosing shortcuts
You cannot flush Mounjaro out of your system faster. There is no way to speed clearance up, and nothing needs doing while it clears.1 Detox drinks, supplements and extreme water intake do not change this, and some carry their own risks. They are low-cost products with no clinical oversight, and dramatic weight-loss or detox claims should be treated with suspicion.
Injecting every few days or every other week
The licence is built around a once-weekly injection on the same day each week, and each KwikPen contains four weekly doses.1 Changing your day is allowed as long as at least 3 days (72 hours) have passed since your last dose, but changing the rhythm is not. Because the medicine is long-acting, dosing at shorter gaps risks stacking levels, and longer gaps are outside what the licence supports.
Why this matters for clearance
All of these shortcuts change how much tirzepatide is in your body at a given moment, and the safety data for the licensed schedule does not cover them. If the dose feels wrong for you, whether too strong or too weak, the right move is a conversation with your prescriber about staying put, stepping down or stepping up within the ladder. 09 · Storage and pen life
Storage, travel and why a pen is discarded after 30 days (not above 30°C)
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1
Why can you not use a pen after 30 days?
The 30 day figure, not above 30°C, is the in-use limit the UK licence sets for the pen. After that, the product has been outside the tested window, and sterility and potency are no longer assured. It is a safety limit and not a sales device. Each KwikPen contains four weekly doses, so with weekly use you will normally finish a pen before the limit matters, but it is a useful reminder if you pause treatment mid-pen.
How this relates to clearance
Do not confuse pen life with how long the drug stays in your body. The first is about the pen's contents staying reliable; the second is about your body's clearance of an injected dose, which takes roughly four to five weeks after a final dose.
Travelling
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1
Unopened pens
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. 10 · Results on 2.5mg
Can you lose weight on 2.5mg of Mounjaro?
Some people lose weight on 2.5mg, but the starting dose is about letting your body adjust, not weight loss itself.1 Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.
What the first weeks look like
The 2.5mg starting weeks are about adjustment, and experiences differ widely. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Because the medicine builds up over successive weekly doses and clears slowly, you should judge progress over weeks and not days.
What the trials measured
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. Note that these figures describe the higher strengths over a long period, not the starting dose.
A realistic three month picture
Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.1
The prescriber review
Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Plateaus
Plateaus of a few weeks are normal and expected as the body adapts to a lower weight. Within the licence, review food intake honestly, protect protein and activity, check sleep and alcohol, and discuss a dose step-up with your prescriber if you are not yet at maximum. 11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · NHS and private access
Getting Mounjaro: NHS and private routes
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
Why this matters for the clearance question
Whether you are treated through the NHS or privately, the pharmacology is identical. The same medicine, the same dose ladder and the same four to five week clearance after a final dose apply. What differs is how quickly you can start and how your follow-up is arranged.
