Can You Stay on 2.5mg of Mounjaro? (October 2026)

You can stay on 2.5mg of Mounjaro for a while if your prescriber agrees, but 2.5mg is the licensed starting dose, not a usual long-term dose. It exists to let your body adjust; your prescriber can hold you there longer if side effects are the concern, then move you up when you are ready.1 Current as of October 2026.

A quick overview

What to know
before you start.

  • 2.5mg is the starting dose, designed for adjustment rather than for weight loss itself, so most people move up after the starting weeks.
  • Your prescriber can hold you on 2.5mg longer within the licence if side effects are the concern.
  • Most measurable progress builds from 5mg onward, so a plateau or returning hunger on 2.5mg is a reason to review your dose.
  • Tell your prescriber before you decide to stay put or step up; the right pace is a clinical decision.
In this guide

01 · The short answer

Can you stay on 2.5mg of Mounjaro?

Yes, for a limited time and with your prescriber's agreement, but not as a default plan. Everyone starts on 2.5mg weekly, and the label describes this dose as being about letting your body adjust rather than about weight loss itself.1 It is the first rung of a ladder, not the destination.

The licensed ladder allows your prescriber to move you up in steps, depending on how you respond and tolerate treatment. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1 So the real question is not whether 2.5mg is allowed, but whether staying there is serving you.

If you are weighing up a longer hold, our guide to staying on 2.5mg long term goes deeper.

02 · Your decision

Should you stay or move up?

The honest answer depends on two things: how your body is coping, and how your appetite and weight are responding. Use the table as a conversation starter with your prescriber, not as a self-prescribing tool.

What you are noticingWhat it usually points toWhat to do
Nausea, tiredness or an unsettled stomach that is still bothering youYour body may need longer to adjustAsk your prescriber about holding the dose longer within the licence
Side effects have settled and appetite is only slightly changedYou may be ready for the next stepDiscuss moving up when you have had at least four weeks on the dose
Steady, comfortable progress and manageable appetiteTreatment may be working well for youKeep going and review regularly with your prescriber
Hunger or food noise returning after the early weeksThe starting dose may no longer be enoughRaise it at your next review
Tempted to take less than the pen deliversNot a licensed approachTalk to your prescriber about a licensed alternative instead

If you are thinking of going further than a short hold, the week-by-week guide shows how the early weeks typically unfold. Ready to start or continue treatment? You can see current options on the Mounjaro pen page, where a new customer's first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99), and every later order is £25 off the standard price.

03 · Why 2.5mg exists

Why 2.5mg is a starting dose

The 2.5mg strength is there so your body can get used to tirzepatide before the dose rises. The label is explicit that this starting dose is about adjustment, not about weight loss itself.1 That framing matters, because it explains why some people feel very little at first and why that is normal.

Everyone begins here. After at least four weeks the dose increases to 5mg, and can then step up in stages up to the maximum, depending on how you respond and tolerate it.1 The pen is injected under the skin on the same day each week, and each pen holds four weekly doses.13

Put simply, 2.5mg is a runway. Staying on it for a short extra stretch can be sensible if the takeoff is bumpy.

04 · The dose ladder

How the licensed dose ladder works

The ladder moves in measured steps, and your prescriber sets the pace. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Notice the phrase "at a minimum". The intervals are a floor, not a deadline. Your prescriber can take longer between steps if you need it, and that is the legitimate way to slow things down.

The six strengths are 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, which means there is no obligation to climb all the way to the top either.1 If you want to see what each strength involves, our 2.5mg overview covers the starter pen in detail.

05 · Can you lose weight?

Can you lose weight on 2.5mg?

Some people do, but progress on the starting dose is usually modest. Experiences differ widely: some people notice appetite change and early loss in the first week, others feel little until a higher dose, and both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Think in months, not days.

The headline trial results also come from higher doses. In SURMOUNT-1 of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

So if you are asking whether 2.5mg alone will get you where you want to be, the evidence points towards needing a higher maintenance dose for most people, though your own response is what counts.

06 · Side effects on 2.5mg

Side effects on the starting dose

The commonest early effects are mild and usually settle. Mild nausea, altered appetite and tiredness in the first days after each injection are the most frequent, and they tend to ease as your body adjusts.1

This is the main legitimate reason to hold at 2.5mg for longer. If side effects are still bothering you when the time to step up arrives, your prescriber can keep you on the dose or, if needed, step down within the licence.1 You do not need to push through symptoms to keep to a timetable.

Like all medicines, Mounjaro can cause side effects, and most are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 Our side effects and safety guide adds practical context.

If you notice a new or lasting symptom, tell your prescriber, and you can report suspected side effects through the Yellow Card scheme.19

07 · Signs you may be ready

When to move from 2.5mg to 5mg

The licence sets the earliest point, and your own response sets the right one. The earliest is after at least four weeks on 2.5mg, which is the licensed minimum before the dose increases to 5mg.1

Questions worth asking yourself before a review:

  • Have the early side effects settled between injections?
  • Is your appetite change fading, with hunger or cravings creeping back?
  • Are you eating sensibly and moving more, so that any change in the scale is genuinely due to the medicine?
  • Do you feel ready, or are you hoping the next step solves everything?

Remember that judging progress by trend, not individual weigh-ins, is more reliable.1 Ask your prescriber for their view, because they know your history. Our guide to what happens if you stay put sets out what a prolonged hold tends to look like in practice.

08 · Reviews and response

How your response is reviewed

Your prescriber reviews how treatment is going, and that review is where the dose decision belongs. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

With us, ongoing monthly reviews are included, and your treatment is reviewed by a GPhC-registered prescriber. Repeat orders re-check suitability, with no auto-shipping, so you always have a point at which to raise the dose question.

09 · Stopping or switching

What if 2.5mg is not working for you?

If 2.5mg is not helping, talk to your prescriber for a review. Options within the licence include holding, stepping up or, if needed, stepping down, all agreed with your prescriber.1

If you decide to stop altogether, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system; there is no way to speed this up, and nothing needs doing while it clears.1 Our page on how long 2.5mg stays in your system explains more.

Access is also a factor. Mounjaro is available on the NHS, but access is being phased in over several years, with NICE recommending it for adults with a BMI of at least 35 plus at least one weight-related condition.4 Many people who meet the licensed criteria choose regulated private treatment rather than waiting. Our weight loss clinic page explains how that works.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Can you stay on 2.5mg of Mounjaro?
You can for a limited period if your prescriber agrees, particularly when side effects are the concern. But 2.5mg is the licensed starting dose, meant for adjustment rather than weight loss itself. Most people move up the ladder, and your prescriber decides the right pace for you based on how you respond and tolerate treatment.
How long can you stay on 2.5mg Mounjaro?
The licence sets a minimum, not a maximum: the dose increases to 5mg after at least four weeks. How much longer you stay depends on your tolerance and your prescriber's judgement. A short extra hold can be reasonable, but a prolonged stay should be discussed at your review rather than assumed.
Can you lose weight on 2.5mg of Mounjaro?
Some people notice appetite change and early loss in the first week, while others feel little until a higher dose, and both are normal. Losses on 2.5mg are usually modest, and most measurable progress builds from 5mg onward, month by month. Results vary, and treatment works alongside diet and activity.
When should I move from Mounjaro 2.5mg to 5mg?
The earliest is after at least four weeks on 2.5mg. Beyond that, the right moment is when early side effects have settled and your prescriber agrees. If hunger or cravings are returning, raise it at your review. Your own tolerance and your prescriber's advice set the timing, not the calendar alone.
What are the side effects of Mounjaro 2.5mg for weight loss?
The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle. Most side effects are digestive: the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. If you notice something new, tell your prescriber and consider reporting it through the Yellow Card scheme.
Why isn't Mounjaro suppressing my appetite?
On 2.5mg this is common. The starting dose is about adjustment, and experiences differ widely: some people notice little until a higher dose. Most measurable progress builds from 5mg onward. Speak to your prescriber about whether you are ready to step up, rather than changing the dose yourself.
Is it safe to take less than a full 2.5mg dose?
No, you should not take less than the pen is designed to deliver. Taking smaller or more spread-out doses than the pen delivers is not licensed, not supported by the pen's design and not endorsed by any UK regulator, and there is no safety data. If side effects are the worry, your prescriber can hold a dose longer or step down within the licence.
