On the first day of treatment, you may feel nothing unusual. Some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.1 Not feeling anything is not a sign that the medicine is failing, and feeling a lot is not a sign that it is working better.

Early effects can also be tangled up with the practicalities of injecting. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Keeping that routine steady gives you a cleaner picture of your own pattern, because you can see how you feel on the same day of each cycle rather than guessing.

Timing varies because the medicine works gradually and bodies differ. Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

05 · Dose steps and symptoms

Do symptoms come back when the dose goes up?

The dose ladder is designed to be gradual, and the pace is flexible. Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Note the words "at least" and "minimum". Four weeks is the earliest a step can happen, not a deadline. If you are still feeling the early effects strongly, your prescriber can hold you at the current dose for longer.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 If a step-up makes you feel worse for longer than you expected, that is information to share, not something to push through silently.

Nausea, diarrhoea, vomiting, constipation and stomach pain are listed in the patient leaflet as very common side effects, and most digestive effects ease within days to a few weeks as the body adjusts, most often when starting treatment or moving up a dose.

Digestive symptoms are not something to quietly tolerate indefinitely. If diarrhoea or stomach discomfort is persistent or keeps waking you, contact your prescriber, who can review your dose and your circumstances. Severe, persistent stomach pain that may spread to your back, with or without vomiting, needs medical help straight away. The commonest early effects usually settle, but that statement covers mild nausea, altered appetite and tiredness specifically.1

Stay well hydrated, and if you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.

07 · Pancreatitis and warning signs

Pancreatitis: what to watch for

There is no proven way to prevent pancreatitis or gallbladder problems while taking Mounjaro. Before you start, tell your prescriber if you have ever had pancreatitis, gallstones or gallbladder disease, or severe problems with your digestion.13

The sensible principle is to treat severe or unusual symptoms seriously. Severe, persistent stomach pain that may spread to the back, with or without being sick, could be pancreatitis — stop using Mounjaro and get medical help straight away. Pain in the upper right of the abdomen, fever or yellowing of the skin or eyes could be a gallbladder problem — get medical help straight away.

08 · What to avoid and what helps

What should you avoid while taking Mounjaro?

Several practical habits are worth getting right, and most come straight from how the pen is meant to be used.

  • Do not change doses yourself.
  • Do not double up after a missed dose. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
  • Do not use a damaged or frozen pen. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Never use a pen that has been frozen.1
  • Do not add unlicensed products when progress slows. A plateau is not a reason to double doses or add unlicensed products.1

Medicines you already take also matter. Insulin and sulfonylureas may need dose adjustments because of the risk of low blood sugar, and Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Tell your prescriber about everything you take.7

09 · Injection day and routine

Using your routine to make symptoms easier to live with

A steady routine will not remove symptoms, but it can make them more predictable. Getting the technique right each week removes one possible source of confusion.

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

If your injection day does not suit you, you can change it. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Moving to a weekend can help if the days after the injection are the hardest. 10 · Expectations and results

Symptoms versus progress: setting realistic expectations

Feeling rough early on does not mean you are getting results, and feeling fine does not mean you are not. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1

Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Plateaus are normal too: weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1

For context on outcomes, in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Your prescriber reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Tolerability is part of that conversation, which is another reason to speak up if symptoms are hard going.

11 · If you stop treatment

How long do effects last after stopping?

Because tirzepatide leaves the body slowly, anything you feel does not end the moment you stop injecting. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.

That is why stopping is a decision to make with your prescriber rather than in response to a bad few days. If symptoms are the reason you are thinking of stopping, tell them first.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
