Everyone starts on 2.5mg once a week, and that starting dose is about letting your body adjust rather than about weight loss itself. After at least four weeks the dose can increase to 5mg, and then step up by 2.5mg at a minimum of four-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

The titration timeline in detail

Titration is the one part of the timeline that is clearly defined. Everyone starts on 2.5mg once a week, and the dose can then rise through 5mg, 7.5mg, 10mg and 12.5mg to a maximum of 15mg, with at least four weeks between steps.1 Each KwikPen contains four weekly doses, so a pen usually covers one step of the ladder.13

Four weeks is a minimum, not a target. Your prescriber can hold you on a dose for longer if side effects are troublesome or if the dose is already working well. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1 Reaching the top of the ladder is not the aim in itself; reaching the dose that works for you is.

This is why a plan to take Mounjaro for only a very short time rarely makes sense. By the time you have stepped through the ladder, a short course may be over before you reach the doses where most measurable progress builds.

Weight loss on Mounjaro is gradual by design, so the timeline you need depends on how far you want to go. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1

A sensible expectation in the early months on a standard titration is modest progress as a share of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

05 · Reviews and stopping rules

When you take your final dose, the medicine does not vanish at once. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Mounjaro works by activating two natural gut-hormone receptors, which helps regulate appetite and slows how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals.1

  • Protect protein and activity. They matter for how you feel and how you hold your progress.
  • Check the basics. Sleep and alcohol both influence appetite and routine.
  • Keep weighing, but read the trend. Weight loss is not linear, and plateaus of a few weeks are normal as the body adapts to a lower weight.1
  • Stay in touch with your prescriber. Ask whether a lower maintenance dose or a planned stop suits you.

08 · Staying on track while on treatment

Staying on track while you are on treatment

Treatment length is shaped as much by consistency as by the calendar. Inject on the same day each week, at any time of day, with or without food. There is no best time of day; what matters is a day and time you will remember.1 Many people choose a weekend morning so early side effects fall on days off.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.

Store pens in a fridge at 2–8°C and never freeze them.
