If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day.1

After at least four weeks the dose increases to 5mg, and can then step up in 2.5mg stages at a minimum of four-week intervals, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

That means a full climb through the ladder takes months before anyone reaches the top dose. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.1

A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure — the dose you respond to may still be ahead of you.2

The long-term figures show where a steady course can lead, but they describe the end point, not the early weeks. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result.

The trial also reports how many participants reached certain thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

As a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg. That is the trial average at the highest dose after 72 weeks, not a prediction for any single person.

08 · Signs it is working

How to tell if GLP-1 treatment is working

The earliest sign is usually a change in how hungry and full you feel. Most people feel fuller sooner and less hungry between meals.1 You may find portions naturally shrink or that you think about food less often between meals.

The scales are a later and noisier signal. Because early loss includes water weight, a single weigh-in tells you little. A better approach is to weigh at the same time of day under the same conditions and look at the direction over several weeks.

Other measures also matter. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

Plateaus are a normal part of the journey. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1

Within the licence, the responses are practical. Review food intake honestly, because portions drift back up. Protect protein and activity. Check the basics, such as sleep and alcohol. Then discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1

A plateau is not a reason to double doses or add unlicensed products.1 Low-cost supplements sold as natural alternatives have no clinical oversight and dosing is not standardised, so they are not a substitute for a licensed medicine reviewed by a prescriber.

If the scales rise slightly for a short period, look at the trend over weeks before drawing any conclusion. Fluid, hormones, bowel habits and meal timing can all shift a single reading.

Your response is formally reviewed rather than left to chance. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

This check is about the highest dose you can tolerate, not the starting dose. That is another reason not to judge the treatment too early on 2.5mg.
