How long until you notice Mounjaro working?
Most people notice something within the first few weeks, but what they notice differs. Some feel fuller sooner and less hungry between meals in the first week. Others feel almost nothing until the dose rises. Both patterns are normal.1
Mounjaro works by activating two gut-hormone receptors, GIP and GLP-1. Together they help regulate appetite and slow how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals.1 That change in appetite comes before any change on the scales, because weight only falls once you have eaten less for a while.

Is your pace normal, and what should you do about it?
If you are in the first weeks and feel little, you are very probably within the normal range. The starting dose is deliberately low, and your prescriber raises it only after at least four weeks, depending on how you respond and tolerate it.1 Waiting is not failing.

03 · The first week
What to expect in the first week on Mounjaro
The first week is mostly about how your body reacts to the medicine, not about the scales. Some people notice appetite change and early loss in the first week, while others feel little until a higher dose. Both are normal.

Early scale movement deserves caution. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. A big drop in the first days says little about the long run. A flat first week says just as little.
Practical points help you read this stage fairly. The pen is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day, because Mounjaro works the same morning or evening. What matters is consistency. Many people choose a weekend morning so early side effects fall on days off.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
04 · Can 2.5mg work?
Can you lose weight on the 2.5mg starting dose?
Some people do lose weight on 2.5mg, but the dose is not designed for that. Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1
After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Each pen contains four weekly doses, so the starter pen covers the adjustment phase.
That is why a slow first month is not a warning sign. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.
Your prescriber guides the pace. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Do not try to speed things up yourself.
05 · The first few months
How much weight can you lose in the early months?
In the early months the average is modest, and that is expected. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In the pivotal trial, average weight loss built month by month, with the full averages measured at 72 weeks, not at the three-month mark.2
A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 That is a planning guide, not a target you must hit. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
It helps to see why early figures look small beside the headline numbers. The dose ladder is deliberately gradual. Each step needs at least four weeks, and a dose often needs time to show its full effect before the next step. So the early months are spent climbing, and the larger results come later, on higher doses, for people who tolerate and need them.

06 · The 72-week picture
What the trial results say about the long run
The headline trial figures describe where people ended up after a long period, not where they were in the first weeks. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
The time frame is 72 weeks. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.11
Trial averages also do not predict your own result. Some people lose more, some less, and your own pace depends on the dose you reach, how you tolerate it, and your eating and activity. A worked example shows the scale of an average: for someone starting at 100 kg, 20.9% is about 21 kg, but that is the average at the highest dose after a long period, not a promise for any individual.

07 · Plateaus and slow spells
What to do if progress stalls
A stall of a few weeks is normal and does not mean the medicine has stopped working. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.
Within the licence, the sensible responses are practical:
- Review food intake honestly, because portions drift back up.
- Protect protein and activity.
- Check the basics, such as sleep and alcohol.
- Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.
A plateau is not a reason to double doses or add unlicensed products.
There is also a formal checkpoint. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4 Because the first weeks are spent on lower doses, the review point comes later than many people assume.
Missing a dose can also affect how things feel. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
08 · Beyond the scales
Results you may notice before the scales move
Changes in how you feel, eat and fit your clothes can show up before the scale does. Appetite is usually the first: feeling fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is often the earliest sign treatment is doing something.
Over a longer period, trials also found wider changes. Weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises. They are why prescribers talk about weight management as health management rather than appearance.

09 · Helping your results
Getting the most from treatment
The medicine is used alongside a reduced-calorie diet and increased physical activity, so what you do around the injection matters. Consistency is the biggest lever you control.
- Inject on the same day each week. You can change your injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1
- Rotate the site with each dose. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.1
- Store pens correctly. Keep them in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
11 · NHS and private routes
NHS and private access: does it change the timeline?
The route you take does not change how the medicine works, but it can change how soon you start. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46
Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. Whichever route you use, the same dose ladder and the same realistic pace apply.
