01 · The core answer
How long before you lose weight on Mounjaro?
The honest answer is that it varies from person to person, and the first weeks are not the main event. Some people notice their appetite change and the scales move in the first week. Others feel very little until they reach a higher dose, and both patterns are normal.1 Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2
02 · At a glance
A realistic timeline at a glance
| Stage | Dose usually in use | What is realistic |
|---|---|---|
| First weeks | 2.5mg | Adjustment phase. Some people notice appetite change and early loss; others feel little. Early loss includes water weight. |
| Following months | Stepping up through 5mg, 7.5mg and 10mg | Most measurable progress builds from 5mg onward, month by month, with plateaus of a few weeks along the way. |
| Around the quarter mark | Often still on the lower rungs of the ladder | Modest compared with headline trial figures, with wide individual variation. |
| after 6 months on the highest dose you can tolerate | Your personal maintenance dose | Your prescriber reviews response and discusses whether to continue, adjust or stop. |
| 72 weeks | Maintenance dose of 5mg, 10mg or 15mg for many people | The point at which trial averages were measured. |
Each row is a general pattern rather than a prediction for you. Your own response depends on your dose, your eating pattern, your activity and how you tolerate treatment.
03 · Your next step
What this means for you
If you are deciding whether to start, the useful takeaway is that patience is built into the treatment. Our page on struggling to lose weight on Mounjaro covers the practical checks in more depth.
If you are ready to begin, the Mounjaro pen is supplied after a free online consultation reviewed by a GPhC-registered prescriber. Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99), and every later order is £25 off the standard price. Stock of all six doses is in stock. Prices checked 12 October 2026. For the wider picture of how treatment fits around food, activity and support, see our weight loss clinic overview.
Mounjaro (tirzepatide) is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary. This page is information, not a substitute for medical advice.
04 · The first weeks on 2.5mg
What happens in the first weeks on 2.5mg
The first weeks on Mounjaro are an adjustment period, and the most useful thing to know is that experiences differ widely. The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.2 If you feel nothing at first, that does not mean the medicine is failing. If you feel a lot, that does not mean you will lose faster than anyone else in the long run.
Why is the starting dose so low? Because its purpose is tolerability. Everyone starts Mounjaro on 2.5mg once a week, and the dose then rises only if you are responding and coping with the medicine.1 Your body needs time to get used to a medicine that slows how quickly the stomach empties and changes how hungry you feel.1 Starting low gives the digestive system a chance to settle before the dose goes up.
What about the scales? Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.2 Water weight can shift quickly in either direction with salt, hydration, bowel movements and the time of day you step on the scales. A dramatic drop in the first days is therefore not a reliable sign of fat loss, and a flat first fortnight is not a reliable sign of failure.
Side effects are the other feature of the early weeks. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.2 Many people choose a weekend morning for their injection so that early side effects fall on days off. There is no best time of day, because Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1
Practical habits that help in this phase are simple. Eat smaller portions when you feel full rather than finishing the plate out of habit. Keep protein in your meals, drink water regularly and avoid forcing large meals when your appetite is low. If any symptom worries you or does not settle, speak to your prescriber, and report suspected side effects through the Yellow Card scheme.19 If you have severe, persistent stomach pain, swelling of the face or throat or difficulty breathing, get medical help straight away. Our guide to when you notice weight loss on Mounjaro looks at the same early period from the viewpoint of what you can actually see and feel.
One more reassurance: the first weeks are not wasted if the scales barely move. You are learning the routine of a weekly injection, finding out how your appetite behaves, and giving your prescriber information about how you tolerate treatment. That groundwork shapes every later decision about when to step up.
05 · Month-by-month pace
A month-by-month view of progress
Progress on Mounjaro is best understood as a staircase that follows the dose ladder, not a straight line from the first injection. The ladder works like this: after at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Because each step takes time, the earliest weeks inevitably sit on the lowest doses.
That is why most measurable progress builds from 5mg onward. Losses in the 2.5mg phase are usually modest, which is not a sign that something has gone wrong.2
Around the quarter mark the picture is still early. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2 A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure — the dose you respond to may still be ahead of you.2
Notice what that means for planning. If you step on the scales after a few weeks expecting the dramatic figures quoted in headlines, you will almost certainly be disappointed. Those figures describe the end of a long programme, not its beginning. Our overview of how long it takes to lose weight on Mounjaro frames the whole journey in the same way.
A helpful habit is to pick a single weighing routine, such as the same morning each week before breakfast, and record the result somewhere you can see the trend. The free GLPTrackr companion app is one option for logging your journey. Looking at several weeks together smooths out the noise from water, bowel habit and daily fluctuation.
It also helps to measure things other than weight. Clothes fit, energy, how often you think about food, and how quickly you feel full all move over months. Progress that does not yet show on the scales may still be real, and it usually becomes visible on the scales as the dose rises and the routine settles.
Finally, remember that higher is not automatically better. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1 The right dose is the one you respond to and tolerate, not simply the biggest one on the ladder.
06 · What the trials measured
What the SURMOUNT-1 trial actually measured
The best-known evidence for tirzepatide in weight management is the SURMOUNT-1 trial, and the way it is reported explains why early expectations should stay modest. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
The key phrase is over 72 weeks. Those averages describe where groups of people ended up after well over a year of treatment, not where they were after the first month. They are also averages, which means some people lost more and some lost less. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
The trial also reported how many participants reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Those figures describe the end of the trial, which is why it is unhelpful to compare them with where you stand after a few weeks.
A worked example makes the scale clearer. For someone starting at 100 kg, 20.9% is about 21 kg. That is a result measured at the end of a long treatment period at the top dose, and it is an average for a group, not a target date or a promise for any single person. It does not tell you what to expect in the first month.
It is also worth being careful about what you compare. Figures from different trials of different medicines should not be set side by side as if they came from a head-to-head test. If you are curious how another injection's timeline looks, our page on how long before Wegovy works covers that medicine on its own terms.
The practical lesson is this. Trials are designed to measure outcomes at a fixed late point, so they tell you where treatment can lead over time, not how fast the early weeks feel. Use them to set a long-range picture, and use your own trend over weeks and months to judge whether you are moving in the right direction. Anyone who tells you that you will lose a specific amount in a specific number of days is going beyond what the evidence supports.
07 · Why dose and tolerance matter
Why your dose shapes how quickly you lose weight
Mounjaro comes as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Someone on the starting strength is taking a deliberately small amount.
To understand why dose matters, it helps to know how the medicine works. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Our explainer on how Mounjaro works to lose weight covers the mechanism in more detail.
Note the phrase supports a reduced-calorie diet. Mounjaro does not burn fat by itself; it makes it easier to eat less. That is why the effect on the scales follows how much your appetite changes, and appetite change tends to grow as the dose rises.
Tolerance is the second factor. The dose can step up at a minimum of four-week intervals, but your prescriber decides the pace based on how you respond and tolerate it.
So when you ask how long before you lose weight, part of the real question is how long before you reach a dose that works for you. Some people respond well at 5mg and stay there. Others need to climb further. Neither route is better; the aim is the lowest dose that gives you steady progress with side effects you can live with.
08 · Early signs it is working
The first signs Mounjaro is working
The first thing most people notice is not the scales but a change in appetite. In practice, most people feel fuller sooner and less hungry between meals.1 You may find you leave food on the plate, stop thinking about snacks, or feel satisfied by a portion that would once have left you hungry. For many people that shift is the earliest sign that the medicine is doing what it was prescribed to do.
It does not happen on the same schedule for everyone. Some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.2 So a quiet start is not a verdict. Equally, a strong start does not tell you how the next months will go.
Other signals that tend to appear before dramatic scale changes include looser clothing around the waist, a change in how quickly you feel full, and less preoccupation with food. These are everyday observations rather than clinical measures, so treat them as encouragement rather than proof. The only reliable way to judge fat loss over time is the trend in your weight and measurements across several weeks.
There is no single symptom that proves it. Be wary of any list of signs promising to confirm fat loss; tiredness, thirst or a changed appetite can have many causes.
What about being noticed by other people? Visible change to friends and colleagues usually lags behind your own sense of it, because it takes a sustained trend rather than a few early weeks. Larger losses over months are much easier for others to see than a small early drop.
Equally important is what to avoid reading as a sign. Nausea, tiredness and altered appetite in the first days after each injection are common early effects and usually settle, but they are side effects rather than evidence of fat loss. If they are severe or do not settle, speak to your prescriber instead of assuming they mean the medicine is working harder. If you have severe, persistent stomach pain, get medical help straight away.
09 · Plateaus and slow progress
Plateaus and slow progress: what to do
A pause in weight loss on Mounjaro is normal, and it is rarely a sign that the medicine has stopped working. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.2 Many people find these pauses frustrating precisely because the early weeks felt easier, but they are part of the pattern.
If progress has stalled, the responses within the licence are practical and unglamorous. Review food intake honestly, because portions drift back up over time. Protect protein and activity. Check the basics such as sleep and alcohol. And discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.2 A plateau is not a reason to double doses or add unlicensed products.2
This is also the honest answer to the question of how to speed up weight loss on Mounjaro. There is no licensed trick to make the medicine work faster than its dose ladder allows. The levers you control are the ones that work alongside it: eating a reduced-calorie diet, increasing physical activity, sleeping well, limiting alcohol and keeping to your weekly injection day.
Be especially cautious about shortcuts you may see online. Doses stretched, extra injections and taking smaller amounts to spread a pen further are outside the licence and carry unknown dosing accuracy and no safety data.1 Supplements marketed with dramatic weight-loss claims are not a substitute for treatment and have no clinical oversight.
There is also a formal checkpoint. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2 That review exists so that nobody continues indefinitely without benefit.
Finally, keep perspective on the timescale. If you are only a few weeks in and the scales are flat, you have not yet reached the point where the review applies, and the dose that works for you may still be ahead. Keep logging your trend, keep your appointments and raise any concerns early. Ongoing monthly clinical reviews with our prescribing team are part of the service for exactly this reason.
10 · Consistency and injection routine
Getting the routine right so results are not delayed
A reliable routine will not make Mounjaro work faster than its dose ladder allows, but a chaotic one can disrupt progress, so the basics deserve attention. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Pick a day and time you will remember and stick to it.
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Each KwikPen contains four weekly doses, so a pen lasts a month of weekly injections. Needles, a sharps bin and injection guidance are included with every order.
Missed doses are common and easy to handle if you know the rule. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 Do not try to catch up by injecting extra.
If your schedule changes, you can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1
Storage matters too, because a pen that has been mishandled may not be usable. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13
Travel is rarely a problem. You can fly with Mounjaro, carrying pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.13 Check destination rules before you fly. Keeping the routine intact through holidays and busy weeks protects your steady progress.
If you stop treatment, the medicine does not leave immediately. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and there is no way to speed this up.1 Our page on how long Mounjaro takes to leave your body explains this further.
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · NHS, private and reviews
NHS, private treatment and how progress is reviewed
Where you get Mounjaro does not change how quickly it works, but it affects how soon you can start and how your progress is reviewed. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
If you choose a private route, make sure it is a regulated one. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. EverydayMeds is a GPhC-registered UK online pharmacy (registration 9012878), and the consultation is free, with no obligation. Be wary of any seller offering to supply without a consultation or prescription, because counterfeit products are a real risk and the MHRA publishes alerts about them.7
Our service is built around the idea that results come from sustained treatment, not a single order. Aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team, and side-effect support. Repeat orders re-check suitability, and there is no auto-shipping, so every dose decision is deliberate. Supply is through official LillyDirect channels, and delivery is free, next-day and tracked in temperature-controlled packaging.
The review structure ties directly to the timeline question. Your prescriber is not looking at your first few weigh-ins. They look at how you are tolerating the dose, whether the step-up is appropriate and, over the longer term, whether you are benefiting. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2
That review is the safety net behind the patience treatment asks for. You are not expected to wait indefinitely without a plan; you are expected to give each dose a fair trial while a clinician watches the trend. The cost side of that plan matters too, and the cost of titration and a first year are set out on our pricing pages using live figures.
For a broader look at the timeline from the same angle, you can also read our guide to how long before weight loss on Mounjaro. Whichever route you take, aim to keep your expectations anchored to months and to the trend, not to the first week.
