03 · Why it is not instant
Why Mounjaro does not work immediately
Mounjaro does not work immediately because it is a once-weekly medicine that acts on appetite and digestion, not a quick-acting fat burner. Each injection starts to act on the gut-hormone receptors it targets, but the benefit you can measure comes from weeks of lower appetite and a sustained reduced-calorie diet.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 That dual action helps regulate appetite, slow how quickly the stomach empties, and improve how the body responds to insulin.1

04 · The first week
What to expect in the first week of Mounjaro
In the first week you are most likely to notice a change in appetite, or very little at all. The starting pen is the lowest strength, 2.5mg, and it is designed as a first step rather than a long-term dose, so a quiet first week is normal.
Things worth doing in that first week:
- Follow the injection instructions that come with the pen and with your order, including the sharps guidance.
- Pay attention to how full you feel after meals and how long that fullness lasts.
- Eat regular, balanced meals and keep the reduced-calorie approach your prescriber recommended.
- Note any symptoms that worry you so you can discuss them at your review.

Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If something new or concerning appears, contact your prescriber and consider reporting it through the Yellow Card scheme.19

05 · Weight loss over time
How long until you see weight loss on Mounjaro?
You can lose weight on the starting dose, but the larger results in trials came over a long period at higher doses. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages, so individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 They tell you the scale of what is possible over the long term.
Two practical points follow from this:
- Early weight change is usually smaller than people expect. Judging the medicine after a few days is rarely fair.
- Your result is shaped by the dose you reach, how long you stay on treatment, and how consistently you follow the diet and activity plan.

06 · Dose steps and timing
How dose increases affect how fast it works
Your dose is increased in steps, and each step is decided by your prescriber, not by how impatient you feel. Mounjaro comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each pen provides four weeks of weekly injections, which is why monthly reviews matter.
At standard prices, if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96; a new customer's first order is £40 less, and every later order is £25 less.
Knowing the cost of stepping up does not mean you should rush. Do not move up faster than prescribed; any dose changes should be agreed with your prescriber.

What to eat on Mounjaro, and what if you eat too little
Mounjaro is used alongside a reduced-calorie diet, so what you eat still matters. Because most people feel fuller sooner and less hungry between meals, many find smaller, regular, balanced meals easier to manage.1
Some sensible starting principles:
- Choose meals with protein and fibre so smaller portions still feel satisfying.
- Eat at regular times rather than skipping meals entirely.
- Drink fluids through the day.
- Avoid treating reduced appetite as a reason to eat very little.

08 · When it wears off
How long Mounjaro stays in your system
Mounjaro stays in your system for weeks after the last dose, which is why stopping does not end its effects overnight. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
The same slow rhythm explains why the effect builds gradually after you start. Neither the start nor the end is a switch.
