The headline number is not the whole story. 02 · The reasons

Why are GLP-1 drugs so expensive?

GLP-1 drugs are expensive mainly because they are new, branded prescription medicines with very high demand.

First, these are branded medicines supplied by their manufacturers. Mounjaro is made by Eli Lilly, and the UK list price is set by the company. Eli Lilly announced an increase in the UK list price of Mounjaro that took effect in September 2025, which is the main reason many people ask why Mounjaro is so expensive now.10 Pharmacies buy at confidential discounted terms, which is why sell prices differ between providers.

Second, treatment is ongoing. Unlike a short antibiotic course, weight management is used for months and often longer, so the cost accumulates. A drug that looks affordable for one month can feel costly across a year.

03 · Spending sensibly

What is the cheapest way to get GLP-1 medication?

The least expensive route is the NHS, if you qualify, because the only charge in England is the £9.90 prescription fee, and many people are exempt. Access is limited, though, and is being phased in over several years. If you do not qualify yet, the next question is how to spend less on private treatment without cutting corners.

NHS prescription costs versus private GLP-1 treatment

On the NHS in England, a prescribed item costs £9.90 unless you are exempt. That is far below any private price, but it only applies if you are offered the medicine. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.4 NHS access to Wegovy runs through specialist weight-management services; ask your GP.

05 · Is it worth the money?

Is GLP-1 worth the money?

Whether GLP-1 is worth the money depends on what you expect from it and what you can sustain. The SURMOUNT-1 trial gives one picture for Mounjaro.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

For someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result, so treat that as an illustration, not a promise.

Value also depends on pace. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 If you are not seeing a response, your prescriber reviews it: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

06 · Cost per dose step

How the dose ladder shapes your monthly cost

Your monthly cost depends on where you end up on the dose ladder, and that is decided by response and tolerability, not by budget. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 That matters for cost: someone who does well at 5mg pays £218.99 per pen at standard prices, while someone who needs 15mg pays £329.99.

07 · Dangers and trade-offs

What are the potential dangers of taking GLP-1 medication?

Like any prescription medicine, GLP-1 treatment has side effects, and cost should never push you towards an unsupervised source. The commonest early effects are mild. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13

This is why a prescriber reviews you before supply and why Mounjaro is a prescription-only medicine. Cheap products sold without a consultation carry unknown contents and no oversight. Counterfeit medicine alerts exist for a reason, and you can report any suspected side effect through the MHRA Yellow Card scheme.19

If you notice a new or lasting symptom, tell your prescriber. 08 · Staying on treatment

Do you have to stay on GLP-1 forever?

Stopping should always be planned with your prescriber. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. What matters for cost is that treatment is not a one-off purchase, so it helps to plan for it as a longer-term spend.

The review process is built into treatment. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1 Your prescriber reviews how you are responding and whether to continue, adjust or stop.

If you do stop, nothing needs doing while the medicine clears. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and there is no way to speed this up.1

09 · Mounjaro and Wegovy

Mounjaro, Wegovy and liraglutide: how cost compares

The three most-discussed GLP-1 options differ in how often you inject and how the dose ladder works, and that shapes what you pay. Mounjaro and Wegovy are once-weekly pens. Liraglutide (Saxenda, and products such as Erly) is supplied at a concentration of 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy.

Wegovy (semaglutide) pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. Titration steps up after at least four weeks, reaching the 2.4mg maintenance dose from week 17.18 Your first Wegovy pen is £99.99 on a first order, and later pens rise with the dose.

For Mounjaro, if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices, while your first 2.5mg pen is £129.99 on a first order with code FIRST40. Both are effective, licensed treatments; which suits you depends on your health profile, tolerance and prescriber advice, and price alone is a poor way to choose. Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
