How Does Weight Loss Medication Work? (October 2026)

Modern weight loss medication works by acting on gut-hormone receptors that control appetite and stomach emptying, so you feel fuller sooner and less hungry between meals. Mounjaro (tirzepatide) activates both GIP and GLP-1 receptors, which supports a reduced-calorie diet.1 It is not a substitute for diet and activity.

A quick overview

What to know
before you start.

  • Weight loss medicines such as Mounjaro act on gut-hormone receptors to reduce hunger and slow stomach emptying.
  • They work alongside a reduced-calorie diet and increased activity, not instead of them.
  • Progress is gradual: think in months, not days, and judge the trend rather than single weigh-ins.
  • Mild nausea, altered appetite and tiredness are the commonest early effects and usually settle.
  • Tell your prescriber about every medicine you take, because slower stomach emptying can change absorption.

01 · The core answer

How weight loss medication works in the body

Prescription weight loss medicine works mainly by changing appetite signals, not by speeding up your metabolism. Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the whole mechanism in plain terms: the medicine makes the diet easier to follow.

EffectWhat it means day to day
Appetite regulationLess hunger between meals and fewer cravings for many people
Slower stomach emptyingFood stays in the stomach longer, so you feel full sooner
Improved insulin responseThe body handles blood sugar more effectively
Lifestyle supportA reduced-calorie diet and more activity become easier to sustain

Mounjaro's dual action is what separates it from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. For a deeper look, see our guides on how GLP-1 medication works and the science behind Mounjaro.

02 · Your next step

Is weight loss medication relevant to you?

It may be, if you meet the licensed criteria and want medical support alongside diet and activity. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber.1

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If that sounds like you, our weight loss clinic explains the process. The 2.5mg starting pen on our Mounjaro pen page has a standard price of £169.99, and new customers save £40 on their first order with code FIRST40.

03 · What the trials show

Does weight loss medication actually work?

Yes, for suitable adults it produces clinically meaningful weight loss in trials when used alongside lifestyle changes. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 The trial average does not predict your own result, so treat it as a range of what is possible rather than a promise.

We cover this in more depth in does GLP-1 work for weight loss.

04 · Pace of results

How quickly does it work?

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

Slower early progress does not predict failure, because the dose you respond to may still be ahead of you. Our guide to how fast Mounjaro works sets out realistic expectations in more detail.

05 · First weeks and side effects

What the first weeks feel like

The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 If you notice a new symptom, discuss it with your prescriber, and you can report any suspected side effect through the Yellow Card scheme.19

06 · Plateaus and diet

Plateaus and what to do about them

Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Within the licence, the sensible responses are practical.

  • Review food intake honestly, because portions drift back up.
  • Protect protein and activity.
  • Check the basics, such as sleep and alcohol.
  • Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

A plateau is not a reason to double doses or add unlicensed products.1 Low-cost supplements marketed as natural alternatives come with no clinical oversight and with dramatic weight-loss claims that medicines licensed in the UK do not make.

07 · Health beyond the scales

Benefits beyond weight

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

08 · Safety with other medicines

Taking other medicines alongside it

Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose.1 Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas, where doses may need reducing to avoid low blood sugar. No adjustment is needed for paracetamol.

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, so ask your prescriber about contraception.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

If treatment is stopped, tirzepatide takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

09 · Injections compared

Other weight loss injections

Mounjaro is a weekly pen, as is Wegovy (semaglutide), whose pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg; the dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17.18 Liraglutide is supplied at 6mg/ml and is a daily injection, unlike weekly Mounjaro and Wegovy.

For single-medicine explanations, read how Wegovy works for weight loss, how weight loss injections work, and how Mounjaro works as a UK injection.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
How does weight loss medication work?
Weight loss medicines such as Mounjaro act on gut-hormone receptors that regulate appetite. Tirzepatide activates GIP and GLP-1 receptors, which helps you feel fuller sooner, slows how quickly the stomach empties and improves the body's response to insulin. The result is less hunger between meals, which supports a reduced-calorie diet.
Does weight loss medication actually work?
For suitable adults, yes, when used alongside a reduced-calorie diet and increased physical activity. In the SURMOUNT-1 trial, people on tirzepatide lost far more on average than those making lifestyle changes alone. Trial averages do not predict your own result, and individual results vary.
What are the potential side effects of weight loss injections?
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Most side effects are digestive. Discuss any new or worrying symptom with your prescriber, and report suspected side effects through the Yellow Card scheme. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
What are the main ways to lose weight?
A reduced-calorie diet and increased physical activity remain the foundation. For eligible adults, prescription medicine such as Mounjaro can be added to make that easier to sustain, under prescriber supervision. Medication is used alongside diet and activity, not instead of them, and your prescriber can advise what suits you.
How long does it take to see results from weight loss medication?
Progress is gradual by design. The early weeks on 2.5mg are an adjustment phase and losses are usually modest, with most measurable progress building from 5mg onward, month by month. Think in months, not days, and judge progress by trend rather than individual weigh-ins.
Do you gain weight after stopping weight loss medication?
Yes, weight can come back. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. The medicine works alongside diet and activity, so the habits you build matter. Tirzepatide takes roughly four to five weeks to clear after a final dose. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
Can weight loss medication affect my other medicines?
Yes, it can. Because Mounjaro slows stomach emptying, it can change how quickly some oral medicines are absorbed, especially when you start or increase the dose. Tell your prescriber about everything you take, particularly warfarin, insulin or sulfonylureas, where doses may need reducing. No adjustment is needed for paracetamol.
