Gut Health Weight Loss Injections: How They Work (October 2026)
Gut health weight loss injections such as Mounjaro work by activating two natural gut-hormone receptors, GIP and GLP-1, which slows stomach emptying and makes you feel full sooner.1 In SURMOUNT-1, adults lost on average 15% of body weight on 5mg and 20.9% on 15mg over 72 weeks; individual results vary.2 The Mounjaro pen starts at £169.99 per month for 2.5mg. A quick overview
What to know
before you start.
-
Mounjaro activates GIP and GLP-1, two natural gut-hormone receptors, to reduce appetite and slow how quickly the stomach empties.
-
Slower stomach emptying is the main reason these injections change how your digestion feels, especially in the first days after each dose.
-
In SURMOUNT-1, average loss was 15% on 5mg to 20.9% on 15mg over 72 weeks; trial averages do not predict your own result.
-
Mounjaro is a prescription-only medicine, supplied after a free online consultation reviewed by a GPhC-registered prescriber.
-
Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99).

01 · The core answer
What gut-hormone weight loss injections actually do
They copy the signals your gut already sends after a meal. Tirzepatide, the active ingredient in Mounjaro, is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve how the body responds to insulin.
In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That dual action is what separates Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1
The phrase "gut health" can mislead. These injections are not probiotics or digestive treatments, and they are not licensed to improve gut health. They are licensed for weight management in adults, and for type 2 diabetes.1 Their effect on your gut is a consequence of how they work. For a closer look at the digestive side, see our guide to Mounjaro and gut health.
| Gut signal | What the injection does | What you may notice |
|---|---|---|
| GLP-1 receptor activation | Helps regulate appetite and slows stomach emptying | Feeling full sooner, less hunger between meals |
| GIP receptor activation | Works alongside GLP-1 as part of the dual action | Combined effect on appetite and insulin response |
| Slower stomach emptying | Food leaves the stomach more slowly | Mild nausea or altered appetite in the first days after each dose, which usually settles |
| Insulin response | Improves how the body responds to insulin | Relevant if you also have type 2 diabetes or prediabetes |
02 · Cost and access
What it costs and how to get treatment
Mounjaro is supplied as a monthly pen, and the price depends on your dose. The 2.5mg starting Mounjaro pen has a standard price of £169.99. A new customer's first order is £40 off at any dose with code FIRST40, which brings your first 2.5mg pen to £129.99. Every later order is £25 off the standard price, applied automatically (code DOSE25). Prices were checked 11 October 2026.
If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year of 13 pens costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. Those totals are before discounts. If you switch to us at a higher dose, your first order is still discounted; the 15mg first-order price is £289.99.
Across the UK pharmacies we track, the cheapest and dearest Mounjaro pens at any dose are £143.96 and £339.00 per month (checked 11 October 2026). The gap between providers for the same dose ranges from £27 to £41, so it is worth comparing like for like.
You can start with a free online consultation. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Delivery is free, next day and tracked, in temperature-controlled packaging. Same-day dispatch applies when you order by 3pm. Needles, a sharps bin and injection guidance come with every order. You can browse the full range on our weight loss treatments page.
03 · Digestion and stomach emptying
How these injections affect digestion
The main digestive change is slower stomach emptying. Tirzepatide slows how quickly the stomach empties, and that is part of why you feel full for longer.1 It is a designed effect, not a malfunction.
Many people feel the effect most in the first days after each injection. Mild nausea, altered appetite and tiredness are the commonest early effects and usually settle.1 The patient information leaflet lists the full set of possible side effects, so read it before your first dose.13
If a symptom worries you, or you notice something new, speak to your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme.19 Our prescribing team offers side-effect support and monthly reviews alongside your treatment.
One interaction is worth knowing about. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar; tell the prescriber about every medicine you take.1
04 · What results to expect
What results to expect, and how fast
Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks. The lifestyle-only group lost 3.1%.2 These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and more physical activity, not instead of them.
As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of those on 10mg and 57% on 15mg lost at least 20%.2
Think in months, not days. Early loss includes water weight, so judge progress by the trend rather than a single weigh-in.1 If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether to continue, adjust or stop.1 That is why "lose weight in two weeks" remedies do not fit this medicine.
05 · Dose ladder and injecting
Dose ladder and how to inject
Everyone starts on 2.5mg once a week. This starting dose lets your body adjust; it is not about weight loss itself.1 After at least four weeks the dose rises to 5mg. It can then step up by 2.5mg at intervals of at least four weeks (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on response and tolerance.1
Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else injects. Rotate the site each dose. Each KwikPen holds four weekly doses.1
If you miss a dose, take it within 4 days (96 hours) of the scheduled time. After more than 4 days, skip it and take the next on your usual day. Never take two doses within 3 days of each other.13 You can change your injection day if at least 3 days (72 hours) have passed since the last dose.13
06 · Supplements and gut boosters
Are gut supplements and "natural Ozempic" products the same thing?
No. Products sold as "natural Ozempic" or the "poor man's Ozempic", such as berberine, are low-cost supplements with no clinical oversight. They are not licensed weight loss medicines, and dosing is not standardised.
Mounjaro acts on GIP and GLP-1 receptors.1 If you are weighing up add-ons, our guide to the GLP-1 gut health booster idea explains what to look for and what to ignore.
Be especially wary of anyone offering to supply a prescription injection without a consultation or prescription. Genuine pens come from a registered pharmacy, and you can check ours on the GPhC register (registration 9012878).12 Our Mounjaro comes through official LillyDirect supply, direct from Eli Lilly's UK distribution. Counterfeit medicine alerts are published by the MHRA.7
07 · Eligibility and NHS access
Who can have these injections, privately or on the NHS
Privately, Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1 A prescriber assesses your BMI, medical history and current medicines before deciding if it suits you.
On the NHS, Mounjaro is available, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. Ask your GP where you sit in the rollout.
08 · Storage and travel
Storing and travelling with your pen
Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can stay out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the cap on to protect it from light.13
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label. For hot climates use an insulated pouch. Unopened pens keep in the fridge until the printed expiry date; if one has been out of the fridge, check with the pharmacy before using it.13
09 · Wider health questions
Beyond the gut: other health questions
Our related guides cover weight loss injections and heart health, brain health and weight loss injections, cellular health and weight loss injections and weight loss drugs and mental health.
Your prescriber can discuss what is licensed and suitable for you today.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. Nothing needs doing while it clears, and there is no way to speed it up.1
10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
