Is there a herbal alternative to GLP-1 medicines?

There is no herbal product that does what a GLP-1 medicine does. Supplements such as berberine are sold with labels like "natural GLP-1", but they are not medicines, are not regulated for this use and have no comparable trial evidence.20 A nickname is not a mechanism.

If you want something that changes appetite in a measurable, studied way, the realistic choices are licensed treatments, reviewed by a prescriber. If you prefer to avoid medicines altogether, that is a legitimate choice. It simply means relying on diet, activity and your GP's advice rather than a bottle with a clever name.

Trial results come from different studies and should not be read as a head-to-head. In the first large head-to-head trial, SURMOUNT-5, tirzepatide users lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg.15 Both are effective, licensed treatments, and individual results vary.

03 · Choosing

How to decide between natural and licensed routes

Start with what you need the product to do. If you want a clinically meaningful change in weight supported by trial data, a supplement cannot offer that. If you only want to tidy up eating habits, then food and activity changes cost nothing and carry no medicine risk.

04 · What "natural GLP-1" means

What "natural GLP-1" products really are

"Natural GLP-1" is a marketing phrase, not a category of medicine. Products using it, such as berberine, are not medicines, are not regulated for this use and have no comparable trial evidence.20

Licensed GLP-1 medicines are different. They are prescription-only, have their own dosing schedules, go through eligibility checks and are backed by published trials. Wegovy tablets and Foundayo, for example, each have a product licence and an SmPC.2021

That gap matters because a bottle can say "supports GLP-1" without any of that evidence behind it. When you read a label, ask three questions. Is it licensed? Is there a published trial for this exact product? Who reviews whether it is safe for you? For most supplements the answer to all three is no.

05 · Berberine and fibre

Berberine, fibre and the nicknames

Berberine is the supplement most often called "nature's Ozempic", and fibre such as psyllium husk is often called the "poor man's Ozempic". Neither nickname means the product acts like a GLP-1 medicine. Berberine is not a medicine and has no comparable trial evidence for weight loss.20

Is berberine just turmeric? No. They are separate products that sometimes get lumped together in wellness marketing. Neither is a substitute for a prescribed treatment.

Has anyone actually lost weight with berberine? Online stories are not evidence. Dosing is not standardised across supplements, which is another reason to be cautious. Dramatic weight-loss claims on a label are a warning sign, not a selling point.

Fibre-rich foods are a normal part of a balanced diet and there is no harm in eating more of them. The point is only that they are food, not a replacement for a GLP-1 medicine.

06 · Licensed alternatives

Licensed alternatives to a GLP-1 injection

If you want something that works differently from Mounjaro, there are licensed choices. Two GLP-1 weight-loss tablets are now licensed in the UK: Wegovy tablets (oral semaglutide, approved by the MHRA in June 2026) and Foundayo (orforglipron, approved in August 2026).2021 They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only.

Orlistat is a tablet that blocks some dietary fat absorption and produces smaller average losses.11 Wegovy and Saxenda (liraglutide) are other licensed injectables. Saxenda is a daily injection, unlike once-weekly Mounjaro and Wegovy.

Wegovy's pens run 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least 4 weeks to the 2.4mg maintenance dose from week 17.18 In STEP 1, 86.4% of adults on semaglutide 2.4mg lost at least 5% of body weight over 68 weeks versus 31.5% on placebo.9 Our Wegovy pen page shows current pricing, and our guide to an alternative GLP-1 medication explains how the options differ.

07 · Mounjaro evidence

What the evidence says about Mounjaro

Mounjaro is tirzepatide, a once-weekly injection licensed in the UK for weight management in adults and for type 2 diabetes.1 It activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

In SURMOUNT-1, 2,539 adults on tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, versus 3.1% with lifestyle changes alone.2 At 72 weeks, 85% on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5%. For someone starting at 100 kg, 20.9% is about 21 kg.

These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

08 · Safety and stacking

Safety: supplements, stacking and unlicensed products

Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence) or compounded copies. These combinations have no safety data and no regulated supply chain.7 If something promises faster results outside a licensed pathway, that is the warning sign.

On the same principle, tell your prescriber about any supplement you take. Your prescriber can advise on your own circumstances.

Beware of sellers offering to supply prescription weight-loss medicine without a consultation or prescription. Check any pharmacy on the GPhC register; ours is registration 9012878. If you notice a new symptom on any product, speak to your prescriber and report it through the Yellow Card scheme.19 Serious side effects of licensed treatment are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

NHS access and your next step

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, so many people who meet the licensed criteria choose regulated private treatment rather than wait.
