Is GLP-1 good for the heart?

GLP-1 medicines are widely discussed for heart health, but the answer is not the same for every product. Different medicines have been studied in different groups of people, and a result for one medicine does not automatically transfer to another. That is why a blanket line such as "GLP-1s protect your heart" is less useful than it sounds.

Mounjaro is a once-weekly injection that activates two gut-hormone receptors, GIP and GLP-1, and that helps regulate appetite, slow stomach emptying and improve the body's response to insulin.1 In the UK it is licensed for weight management in adults and for type 2 diabetes.1 Anything beyond that, such as reduced heart attack or stroke risk, belongs in a conversation with your own clinician.

What should you do next?

Start with your own situation. If you have no known heart condition and want weight-management treatment, the question is mainly whether you meet the licensed criteria. If you have heart disease, high blood pressure, or take heart medicines, tell the prescriber and your GP or cardiologist before you begin.

03 · Weight loss and your heart

Weight loss versus heart protection

Losing weight and protecting the heart are related goals, but they are not the same measurement. A medicine can reduce body weight without that being proven to change heart outcomes, and the reverse can also be true. Clinicians therefore look at the evidence for each outcome separately.

For weight, the Mounjaro trial evidence is clear. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

That trial measured weight, not heart events. So the fair reading is that Mounjaro is a well-evidenced weight-management medicine. Whether that translates into a lower chance of a heart attack for you is something to discuss with a doctor who can see your blood pressure, cholesterol and family history. 04 · Which GLP-1 for the heart

Which GLP-1 is best for heart health?

No single GLP-1 medicine is best for heart health; the prescriber or specialist decides what suits the person.

The medicines you will see discussed in the UK include tirzepatide (Mounjaro), semaglutide (Wegovy and Ozempic) and daily liraglutide. They are not interchangeable. Tirzepatide acts on both GIP and GLP-1 receptors, which distinguishes it from single-action medicines such as semaglutide.1 Liraglutide is a daily injection, unlike once-weekly Mounjaro and Wegovy.

If heart protection is a main reason you are considering a GLP-1, that is a cardiology or diabetes-clinic decision rather than a pharmacy-shelf choice. Take these medicine names to your appointment and ask which has evidence for your situation. 05 · Existing heart conditions

If you already have a heart condition

Tell the prescriber about every heart diagnosis and medicine before treatment starts. This includes heart failure, previous heart attack or stroke, irregular rhythm, and blood pressure or cholesterol treatment. The prescriber can then decide whether the order is suitable, and unsuitable orders are refunded automatically.

First, Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows, so ask about contraception if it applies to you.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Second, if you take insulin or sulfonylureas, doses may need adjusting because of low blood sugar risk. It does not mean the medicines cannot be used together.

06 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

07 · NHS and other questions

NHS access, bones, coverage and headlines

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first.6 Many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If heart protection is your priority, ask your cardiologist or GP which medicine has evidence for people with your condition before choosing.
Do cardiologists prescribe GLP-1?
Prescribing usually sits with GPs, diabetes teams and specialist weight-management services, though a cardiologist may recommend or advise on one. If you see one, ask directly whether a GLP-1 medicine suits your heart condition and current treatment.
Does GLP-1 remove plaque from arteries?
Mounjaro is licensed for weight management and type 2 diabetes, not for treating artery disease. If you are worried about plaque or cholesterol, your GP or cardiologist can arrange the right tests.
Who should avoid GLP-1?
Suitability is decided case by case. Every order is reviewed by a GPhC-registered prescriber who checks your health history and current medicines, and unsuitable orders are refunded automatically. Tell the prescriber about heart conditions, other diabetes treatments and contraception before starting. Mounjaro should not be taken if you are allergic to tirzepatide or any of its ingredients, or during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Stop Mounjaro at least one month before trying to conceive.
What is the downside to taking GLP-1?
Like any medicine it can cause side effects, which are listed in the patient information leaflet. Results also vary between people, and treatment works alongside a reduced-calorie diet and more activity. Report any new symptom to your prescriber and through the Yellow Card scheme.
