What should you do if you have chest pain on a GLP-1?
Treat sudden, severe, crushing or spreading chest pain as an emergency: call 999 and get medical help straight away. Do not drive yourself, and do not wait to see whether your next dose settles it. Pain or pressure that spreads to the arm, back or jaw, or comes with breathlessness, sweating or feeling faint, is never something to manage at home: call 999 and get medical help straight away.
If the discomfort is mild, burning, comes after meals or when lying down, and you have had reflux or indigestion since starting, it may well be digestive. Even so, if it is new for you, get medical help straight away so a clinician can decide whether it needs checking.

Is it likely to be the medicine?
Reflux, burping and indigestion are all listed side effects, and they are the most plausible link between a GLP-1 medicine and a burning ache behind the breastbone. Common side effects (up to 1 in 10 people) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.13 Tirzepatide also slows how quickly the stomach empties, so large portions can sit heavily.1

03 · Digestive causes
Reflux and indigestion: the common explanation
Digestive effects are the most frequent reason for discomfort around the chest on treatment. The label lists reflux, indigestion and burping as common, and nausea, vomiting and abdominal discomfort as very common.13
Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13 Practical steps that help most people are eating smaller meals slowly, stopping at the first feeling of fullness, favouring plain lower-fat foods while adjusting, staying well hydrated, and avoiding lying down straight after eating.13
If discomfort is persistent, speak to your clinical team rather than pushing through severe symptoms.
04 · Red flags
Warning signs that need urgent help
Some pain near the chest is really abdominal, and some cases need immediate attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis.13 Pain in the upper right abdomen, fever or yellowing of the skin or eyes may point to gallbladder problems: get medical help straight away.13
Swelling of the face or throat with difficulty breathing can signal a severe allergic reaction: get medical help straight away.13 If you take insulin or a sulfonylurea alongside Mounjaro, shakiness, sweating or confusion can mean low blood sugar, and your prescriber may need to adjust those medicines.13

Your GP or prescriber knows your history and is the right person to weigh it up, especially if you have existing heart conditions or diabetes.
Muscle strain, anxiety and heart problems can feel alike, and nobody can reliably tell them apart by symptoms alone. If you are unsure whether chest pain is heart related, get medical help straight away. If a symptom is new since starting treatment, tell your prescriber and consider reporting it through the Yellow Card scheme.19
