GLP-1 Cardiovascular Benefit: What the Evidence and Licences Say (October 2026)

A quick overview

What to know
before you start.

  • GLP-1 medicines are not interchangeable: any cardiovascular claim belongs to a specific medicine and a specific trial, not to the class as a whole.
  • Mounjaro is licensed in the UK for weight management in adults and for type 2 diabetes, and we do not market it as a heart medicine.
  • Headline percentages quoted online often come from high-risk groups and cannot be applied to your own situation.
  • Never stop or swap a statin or other heart medicine without your prescriber; tell us about every medicine you take.
  • The SmPC and patient leaflet are the authoritative record of what each GLP-1 is licensed to do.
In this guide

01 · The core answer

Which GLP-1 has cardiovascular benefit?

The medicine with a cardiovascular claim is the one whose official product information says so. That information lives in each product's Summary of Product Characteristics, and it can differ from one GLP-1 to the next. Headlines that say GLP-1 drugs protect the heart treat a varied group of medicines as if they were a single product.

If cardiovascular protection is the main reason you are looking at these medicines, the right route is a conversation with your GP or cardiologist, who can look at your history, blood pressure, cholesterol and current medicines. Our own role is narrower: weight management, supplied after a consultation.

02 · Your decision

Is this relevant to you, and what next?

It is relevant if you live with excess weight and are also thinking about heart health. It is not a reason to start or stop any medicine on your own. Weight management with Mounjaro is a licensed use, and our consultation is the place to talk through your wider health, including any heart or circulation conditions.1

Before you apply, think about three things. First, which medicines you already take, particularly for blood pressure, cholesterol, diabetes or blood thinning. Second, whether you have a history of heart disease, stroke or other cardiovascular conditions. Third, what you actually want from treatment: weight loss, a licensed diabetes treatment or heart protection. Those goals can overlap but are not the same.

If weight is your goal, you can start with a free online consultation for the Mounjaro pen. Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). Our wider weight loss clinic page explains the full process.

03 · What GLP-1 means

What GLP-1 medicines are, and why the class is not uniform

GLP-1 medicines mimic a natural gut hormone that helps regulate appetite and the way the body handles insulin. Different products target this system in different ways, and they differ in how they are dosed, what they are licensed for and how they have been studied.

Mounjaro illustrates the point. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 That dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

Because the medicines differ, a result seen with one does not automatically transfer to another. Anyone who writes that GLP-1 drugs reduce heart attacks in general terms is compressing several separate products and several separate studies into one sentence. When you see a cardiovascular claim, ask which medicine, in which patients, and for what outcome.

04 · What Mounjaro is licensed for

What Mounjaro is licensed for in the UK

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Mounjaro's licence does not say the medicine is approved to prevent heart attacks or strokes. The SmPC and the patient information leaflet are the documents that set out the approved uses, the warnings and the side effects.13

If you want to explore the topic from the Mounjaro side, our related guides cover whether Mounjaro has cardiovascular benefits, the cardiovascular benefits of Mounjaro and the wider Mounjaro cardiovascular picture.

05 · Weight loss evidence

The weight-loss evidence

The headline trial evidence for Mounjaro is weight-loss evidence. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 SURMOUNT-1 measured weight, not heart attacks or strokes, so it does not tell you what the medicine does for cardiovascular events.

Many readers assume that losing weight and lowering heart risk are the same result. They are related questions, but a trial has to measure each one.

06 · Reading headline claims

How to read headline cardiovascular claims

They are worth reading carefully, because a single figure hides a lot of context.

Look at who was studied. Outcome trials often recruit people who already have established heart disease or a high risk of it, and their results describe that group. They may not describe a healthy adult who simply wants to lose weight.

Look at which medicine was used. A figure for one GLP-1 is not a figure for the class, and it is certainly not a figure for tirzepatide.

Look at what was measured. Heart attack, stroke and cardiovascular death are different outcomes from blood pressure, weight or cholesterol readings. Be wary of any page that blurs them.

Look at how the claim is framed. Explanations that mention inflammation, blood vessel function or direct effects on heart muscle describe proposed mechanisms. A mechanism is a hypothesis about why something might happen, not proof that it does for you.

Finally, look at who is selling. A page that links a heart-benefit claim to a product you can buy is marketing.

07 · Plaque, cholesterol and statins

Plaque, cholesterol and statins

Readers often ask whether GLP-1 medicines remove plaque from arteries, lower cholesterol or can replace a statin. Treat any source that does as something to verify with your prescriber.

Do not stop, reduce or swap a statin or any other cholesterol medicine because of something you read about GLP-1 drugs. Those decisions belong to the clinician who knows your history and your test results. A weight-management injection is not a substitute for a medicine prescribed to protect your heart.

If you already take cholesterol medication and want to add Mounjaro for weight management, tell us during the consultation. A GPhC-registered prescriber reviews your answers, including your current medicines, before anything is approved. We also re-check suitability at every repeat order.

Stories about new drugs that melt plaque or replace statins are best met with caution. If a treatment is genuinely approved in the UK, its SmPC will say so. If you cannot find it there, ask your GP or cardiologist rather than a headline.

08 · Why cardiologists differ

What cardiologists think, and why opinions vary

We do not speak for cardiologists, and no single view represents the profession. What is fair to say is that clinicians generally weigh a patient's overall risk, not a single drug. Blood pressure, cholesterol, smoking, diabetes, family history and weight all feed into that picture.

That is why a cardiologist may be enthusiastic about a medicine for one patient and unconvinced for another. It also explains why statins remain a standard conversation in cardiology: they are one tool aimed at a specific risk factor, and a GLP-1 medicine is a different tool aimed at others.

If you want a professional opinion on whether a GLP-1 belongs in your own plan, ask the specialist who looks after your heart. Bring a list of everything you take, including over-the-counter products and supplements. Our guides on GLP-1 and cardiovascular health and the cardiovascular benefit of GLP-1 agonists give further background to take to that conversation.

09 · Weight and heart health

Why people link weight management to heart health

The link people have in mind is intuitive: carrying less excess weight is widely discussed alongside better overall health. The Mounjaro trial evidence describes weight outcomes in a defined trial, and it is not a statement about heart attack or stroke risk.

What the weight data do show is substantial average loss when treatment is combined with lifestyle change. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Trial averages do not predict your own result, and the trial did not set out to prove a cardiovascular effect.

The sensible reading is this: weight management is a legitimate health goal, Mounjaro is licensed for it, and a conversation about your heart should be run alongside it, not replaced by it. Our overview of the benefits of GLP-1 receptor agonists and the benefits of GLP-1 medications covers the wider picture.

10 · NHS and private access

NHS and private access to Mounjaro

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6

If you have a heart condition, your GP is the right first call for NHS options. If you go private, a GPhC-registered prescriber reviews your consultation, and our team provides ongoing monthly reviews, dose titration and side-effect support. Mounjaro is a prescription-only medicine, used alongside a reduced-calorie diet and increased physical activity, and this page is information, not a substitute for medical advice.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Which GLP-1 has proven cardiovascular benefit?
It depends on the individual medicine and what its UK licence and product information say, not on the class as a whole. Check each product's SmPC, and ask your GP, cardiologist or prescriber which option suits your own history. Mounjaro is licensed for weight management and type 2 diabetes.
Does Mounjaro protect your heart like Ozempic?
Mounjaro and Ozempic are different medicines, and a result seen with one does not automatically apply to the other. Mounjaro is licensed for weight management in adults and for type 2 diabetes. If heart protection is your priority, discuss it with your GP or cardiologist and read each SmPC.
Does GLP-1 remove plaque from arteries?
Mounjaro's licence does not describe it as removing arterial plaque, and mechanisms discussed online are not proof of an effect in you. If you have been told you have narrowed arteries, your cardiologist is the right person to ask about treatment options.
Does Mounjaro reduce cholesterol?
Cholesterol reduction is not part of Mounjaro's licence, which covers weight management and type 2 diabetes. Keep taking any cholesterol medicine you are prescribed, and tell the prescriber about it in your consultation. Your GP can review your cholesterol results and advise what is right for you.
Can GLP-1 be used instead of statins to manage cholesterol?
Do not replace a statin with a weight-management injection. Mounjaro is licensed for weight management and type 2 diabetes, not as a cholesterol treatment. Decisions about statins belong with the clinician who knows your history and test results, so speak to your GP before changing anything you have been prescribed.
Can I take GLP-1 with cholesterol medication?
Tell us about every medicine you take during the consultation, including cholesterol tablets. A GPhC-registered prescriber reviews your answers before anything is approved, and suitability is re-checked on repeat orders. Do not stop your existing medicine unless your own prescriber tells you to.
Why is GLP-1 good for the heart?
We would not say GLP-1 medicines are good for the heart as a blanket statement. Explanations online describe possible mechanisms, which are not the same as proven outcomes for a particular medicine. Ask your cardiologist about heart-specific questions.
What is the safest way to find out if a GLP-1 suits my heart condition?
Speak to the clinician who manages your heart condition, bring a list of your medicines, and read the SmPC and patient leaflet for the product. If you apply through us, declare your cardiovascular history in the free consultation so a GPhC-registered prescriber can review it. Report any new symptoms through the Yellow Card scheme, and get medical help straight away for severe, persistent stomach pain, pain in the upper right abdomen, fever, yellowing of the skin or eyes, or signs of a severe allergic reaction.
