01 · The core answer

Can you take GLP-1 with hyperthyroidism?

Having hyperthyroidism does not automatically stop you taking a GLP-1 medicine, but it is a decision for a prescriber, not something to settle yourself. Mounjaro's own list of people who should not take it covers allergy, pregnancy or trying to conceive, and conditions needing extra care such as pancreatitis, severe gut disease, diabetic retinopathy and gallbladder problems.13 An overactive thyroid is not named there. That is not the same as being told it is irrelevant. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Your prescriber will want the whole picture. That means your diagnosis, how it is being treated, whether you are under a specialist, and what else you take. Always share your full medical history and medicine list during consultation.13 If your thyroid condition is currently unsettled or newly diagnosed, a prescriber may reasonably ask you to speak to your own GP or specialist first.

What the thyroid warning actually says

Mounjaro's thyroid precaution is specific, and it is not about hyperthyroidism. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials.1

As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2 — they will weigh this in your assessment.1 Routine thyroid monitoring is not required for most people.1

So the questions that matter are about thyroid cancer history and family history. Most online summaries blur this into "thyroid problems", which makes an overactive thyroid sound more worrying than the label suggests, and it also makes a genuine cancer history sound more routine than it is.

Hyperthyroidism is not listed as a side effect in the UK patient leaflet. The thyroid safety information refers to C-cell tumours in rodents, with no increase shown in human trials, and says routine thyroid monitoring is not required for most people.1 That is a different matter from an overactive thyroid gland.

If you notice new symptoms after starting treatment, do not guess whether they are connected. Tell your prescriber. You can also report a suspected side effect through the MHRA Yellow Card scheme, which is how regulators spot patterns that individual trials miss.19

05 · Thyroid cancer and MEN 2

Thyroid cancer, medullary carcinoma and MEN 2

The part of the thyroid label that carries real weight is medullary thyroid cancer and the inherited condition MEN 2. These are different from the more common thyroid cancers that many people have heard of, and different again from hyperthyroidism.

The precaution is simple: tell your prescriber if you or a close relative have had either one. They will weigh this in your assessment.1 If a family member has had thyroid cancer and you do not know which type, ask them or their clinician before your consultation. That detail genuinely matters.

06 · What to tell your prescriber

What to tell your prescriber

A useful consultation with a thyroid condition covers a handful of specific points. None of these is a hurdle; they simply let the prescriber decide properly.

  • Your diagnosis. Say which thyroid condition you have, and whether it is current, past or in remission.
  • Your treatment. Name any thyroid medicines or procedures, and who manages them.
  • Family history. Mention any medullary thyroid cancer or MEN 2 in you or a close relative.1
  • Other conditions. Pancreatitis, severe gut disease, diabetic retinopathy and gallbladder problems all need extra care.13
  • Everything else you take. Include prescribed, over-the-counter and supplement products.

Be accurate rather than reassuring. A prescriber working from partial information cannot protect you well, and incomplete answers are the commonest way treatment goes wrong.

07 · Hashimoto's and other conditions

Can people with Hashimoto's take GLP-1?

Hashimoto's is not named among the conditions in Mounjaro's list of reasons for caution, so it does not automatically exclude you.13 The same is true of other common thyroid conditions: the thyroid warning is about medullary thyroid cancer and MEN 2, not about autoimmune thyroid disease.1

If you are on thyroid replacement, tell your prescriber the medicine name. If your thyroid care is under a specialist, a short conversation with that clinician before you start is sensible.

The same approach applies to an overactive thyroid, an underactive thyroid, thyroid nodules and past thyroid surgery. Disclose, explain, and let the prescriber weigh it.

08 · Insulin resistance and thyroid

Insulin resistance and hyperthyroidism

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.

If you have both a thyroid condition and a blood-sugar issue, your prescriber needs both on the record. If you take insulin or a sulfonylurea, watch for low blood sugar, with shakiness, sweating or confusion, because those medicines may need adjusting.1

09 · Symptoms worth reporting

Symptoms worth reporting during treatment

Report any new or worsening symptom to your prescriber rather than waiting to see whether it settles. This matters more when you already have a thyroid condition, because symptoms can overlap and it is hard to tell the cause from the outside.

Some symptoms are urgent whatever your thyroid status. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could be gallbladder problems, and for swelling of the face or throat with difficulty breathing, which could be a severe allergic reaction.1

Severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys, so keep fluids up and get medical help straight away if you cannot keep fluids down.1 Our monthly reviews and side-effect support are there for exactly these moments.

Any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17 Regulators have reviewed the evidence on mood and concluded it does not support a causal link.17 If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. 10 · Other GLP-1 medicines

Do other GLP-1 medicines differ on the thyroid?

Wegovy, Ozempic and tirzepatide are different medicines, and each has its own product information. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss. If you are considering the Wegovy pen instead, the same advice applies: disclose your thyroid history and let the prescriber check the product information for that medicine.

Do not move between products to avoid a conversation about your thyroid. Whichever medicine you consider, your history travels with you.

11 · Weight loss without a GLP-1

If a GLP-1 medicine is not suitable

Tirzepatide works alongside a reduced-calorie diet and increased physical activity, and the same foundations support any other plan. A decision not to prescribe is not a judgement on you; it is the prescriber doing the job of weighing your history.

NHS and private routes

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
