Not as a direct effect. Some people feel they look healthier after losing excess weight, and some notice a leaner face. Others notice hollowing. Neither outcome is a promised effect, and individual results vary.

04 · Face and skin changes

Face and skin changes during weight loss

Facial fat is part of what gives the face its shape. When you lose weight, some of it goes from the face as well as the body. Cheeks can look flatter, and the skin may look looser until it catches up.

Skin commonly adapts gradually after weight stabilises.1 If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up.1

05 · Muscle and strength

Protecting muscle as you age

Losing weight on any method can reduce lean mass as well as fat, and this matters more as we get older. The measures that protect lean mass are adequate protein, resistance exercise and a sensible pace of loss.1

Fatigue is a recognised common side effect, most noticeable in the first weeks and after dose increases. It usually reflects a mix of the body adjusting and simply eating less, so keep meals balanced, stay hydrated and keep gentle activity going.13 Tell your clinical team if tiredness is severe or not settling.

06 · Hair and side effects

Hair shedding and other visible effects

Temporary hair shedding is a recognised side effect, reported as common in trials and linked to rapid weight loss itself rather than a direct drug effect.1 For most people it settles as weight stabilises. Protein intake and overall nutrition matter.

Injection-site reactions are another visible effect. Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days. Rotating sites each week reduces problems.13

Most other side effects are digestive: nausea, diarrhoea, vomiting, constipation and abdominal discomfort are the most common. They are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks.1 If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.13

07 · Longevity claims

Longevity and anti-aging claims: what is established?

Mounjaro is licensed in the UK for weight management in adults and for type 2 diabetes.1 Slowing biological aging, clearing arteries or extending lifespan are not licensed uses of Mounjaro.

Headlines about epigenetic clocks and inflammation come from early or observational research. They may be interesting, but they are not a reason to take a prescription medicine you do not otherwise need. A treatment decision should rest on weight and health need, assessed by a prescriber.

08 · Older adults

Older adults and GLP-1 medicines

Older adults often take several medicines, so a full review matters. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion), because your prescriber may need to adjust those medicines.13

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen with fever or yellowing of skin or eyes, or swelling of the face or throat with difficulty breathing.13

Report any suspected side effect through the Yellow Card scheme.19

09 · Mood and wellbeing

Mood, wellbeing and how you feel

Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

Any big change in weight, appetite and routine can affect how you feel, including how you feel about your appearance. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Speak to your own prescriber or pharmacist about your full medicines list.
Why are older adults stopping GLP-1?
In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo. Speak to your clinical team rather than pushing through severe symptoms.
Does GLP-1 remove plaque from arteries?
That is not a licensed use of Mounjaro. Mounjaro is licensed for weight management and type 2 diabetes. Questions about heart or artery health should go to your GP or specialist, who can advise on treatments that are actually approved for that purpose.
Can you smell when someone is on Ozempic?
A noticeable smell is not a listed side effect in the Mounjaro patient leaflet. Reflux and burping are listed as common. If you notice any new symptom, discuss it with your prescriber and report it through the Yellow Card scheme.
What is known as the "poor man's Ozempic"?
The phrase is marketing for low-cost supplements such as berberine, sold with dramatic weight-loss claims. They are available cheaply without assessment or clinical oversight, and dosing is not standardised. They are not equivalent to licensed prescription medicines, which are reviewed by a GPhC-registered prescriber.
