The way to come off a GLP-1 is to decide with your prescriber, take your final weekly dose, and then rely on a plan rather than the medicine. The decisions you actually face are when to stop, whether to step down a dose first, and what replaces the medicine's appetite support.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.1 That means the change is usually gradual rather than overnight. You may not notice much in the first week or two, then appetite can feel different as the level falls.

The Mounjaro pen at 5mg is £218.99 per month at the standard price, and the 15mg pen is £329.99. 04 · What happens when it clears

What happens when the medicine clears

As Mounjaro leaves your system, you lose the medicine's action on the gut-hormone receptors it targets. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals while taking it.

Because that effect comes from the medicine, it is sensible to plan for it to fade as the medicine clears. Expect to feel hungry sooner and to feel less full after meals than you did on treatment.

The clearance itself takes roughly 4–5 weeks after the final dose. There is no way to speed it up, and nothing needs doing while it clears.1

05 · Withdrawal and side effects

Withdrawal symptoms and how you will feel

The UK patient leaflet does not list withdrawal symptoms as a side effect. Tell your prescriber about any new or lasting symptom.13

What you can reasonably expect is a change in appetite as the medicine clears, which is its expected action fading rather than a new symptom. Early side effects such as mild nausea, altered appetite and tiredness in the first days after injections are the commonest effects while taking it and usually settle.13 Those should ease as doses stop.

06 · Keeping weight off afterwards

How to keep weight off after a GLP-1

The most reliable plan is to build the habits that carry results before the medicine leaves, not after. Mounjaro is licensed for use alongside a reduced-calorie diet and increased physical activity, and the trial averages came with those, not without them.2 In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. Individual results vary.

The lifestyle-only group shows what diet and activity changes did on their own over the same period, which is a reminder that the habits are part of the treatment, not an optional extra.2

  • Start before your last dose. Use your final weeks on treatment to practise the meal pattern and activity routine you will rely on.
  • Keep the same weigh-in rhythm. Judge your trend over weeks, not single weigh-ins, as you did on treatment.
  • Plan for hunger. Expect to feel hungry sooner after meals and decide in advance what you will eat.
  • Book a review. Tell your prescriber you are stopping so they can support the transition.

07 · Should you stop at all?

Do you have to stay on a GLP-1 forever?

The licence does not say you must take Mounjaro for life, and it does not say you must stop at a set point. The decision is made with your prescriber, who reviews your response over time. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

Good reasons to talk about stopping include reaching a weight you are happy with, side effects that do not settle, cost, a change in your health or medicines, or planning a pregnancy. If you are thinking about pregnancy, read our guide on getting pregnant on a GLP-1 and speak to your prescriber before you stop or start trying. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Realistic pace matters too. Weight loss on Mounjaro is gradual by design, and most measurable progress builds from 5mg onward, month by month.1 Stopping before you have reached a stable maintenance dose is a different decision from stopping after months at one, so tell your prescriber where you are.

08 · Missed doses vs stopping

Missed doses, gaps and restarting

A short gap is not the same as stopping. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose and take the next one on your normal day. Never take two doses within 3 days of each other.13

If a gap is longer than a missed dose, do not restart at your old strength on your own. Contact your prescriber, who will decide where to restart. Repeat orders with us re-check suitability, and we never auto-ship, so a restart always goes through a prescriber review.

Your pen's storage matters if you are pausing too. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 Do not keep a part-used pen "for later" beyond that window.

Stopping on the NHS or privately

The process is the same in principle: you talk to whoever prescribes. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.46

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
