05 · What the trials show

What the trial evidence does and does not say

The headline trial results were gathered with diet and exercise in place, so they cannot be read as a result for someone who skips activity. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 The comparison group shows that lifestyle changes alone produced a much smaller average, and the medicine produced a much larger one on top of the same lifestyle advice.

06 · Muscle and protein

The real cost of skipping exercise: muscle

The main trade-off is lean tissue. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. Without exercise, you lose one of the two tools for guarding it.

The two levers are adequate protein — aim to include a protein source at every meal, which is harder when appetite is low — and resistance exercise two or more times a week. This matters more, not less, as the dose rises and appetite falls.

In practical terms, if you truly cannot do resistance work, protein becomes the lever you cannot skimp on. Plan meals around a protein source first, then add vegetables and fibre. 10 · Plateaus and pace

Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Without exercise you have one fewer lever to pull when that happens, so the other levers matter more.
