  • Pre-plan the harder days. Have set meals ready so that decisions are not made when you are hungry.
  • Protect protein. Include a protein source at every meal, and keep up regular activity.
  • Check portions. Portions drift back up over time, so an honest review of intake is worth doing.
  • Look at the basics. Poor sleep and alcohol both make appetite harder to manage.
  • Keep your injection day consistent. A steady weekly routine gives the most predictable pattern.
  • Drink water regularly. It does not replace the medicine, but thirst and hunger are easy to confuse.

Can you inject earlier to avoid the dip?

Does Mounjaro become less effective over time?

Weight loss on Mounjaro is not linear. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight.4

Treatment response is reviewed by your prescriber. Weight loss is gradual by design, so think in months, not days, and judge progress by trend, not individual weigh-ins.4 In the SURMOUNT-1 trial, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. 08 · Pen and dose timing

Could the pen or your dose timing be the issue?

Occasionally, yes, and it is worth checking before assuming the medicine has weakened. Store unopened pens in the fridge at 2–8°C until first use. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Follow the pen instructions for each injection, and contact your pharmacy if you suspect a faulty pen or an incomplete dose.

A weekly dip is a fluctuation within ongoing treatment. Stopping is different: after a final dose it takes roughly 4–5 weeks to clear the system, and in SURMOUNT-4 people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, so stopping should always be planned with your prescriber.14 If you have missed injections or taken a break, the picture is different and you should talk to your prescriber about restarting rather than guessing.

10 · Safety and reporting

When to seek advice rather than adjust things yourself

Contact your prescriber if hunger or other symptoms are affecting your wellbeing, if you are struggling to eat enough, or if you notice anything unexpected after a dose. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Do not change how you use the pen yourself.
