01 · The core answer
Do the effects of Mounjaro wear off? It depends which kind you mean
Mounjaro does not stop working after a set number of weeks, but its effect rises and falls with each weekly injection, and it ends gradually once you stop. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly.1
| What you notice | Most likely explanation | Sensible next step |
|---|---|---|
| Hunger or 'food noise' returns on the last days before your next dose | Tirzepatide has a half-life of about five days1 | Plan protein-rich meals and tell your prescriber at your review |
| Progress slows after the early months | Weight loss is not linear; plateaus of a few weeks are normal as the body adapts to a lower weight | Review food intake, protein, activity, sleep and alcohol, then discuss a dose step-up if appropriate |
| Appetite and weight drift back after you stop | Mounjaro manages weight; it does not cure the biology behind it14 | Plan any stop with your prescriber, with diet and activity habits ready |
02 · Your next step
Is this relevant to you, and what should you do next?
If you are still taking Mounjaro and feel the effect dipping, the first move is to note when it happens and tell your prescriber, not to change the dose yourself. Keep your injection on the same day each week, because consistency is what keeps the weekly cycle predictable.1 Our guide to wear-off through the week explains the pattern day by day in qualitative terms.
If you are considering starting, the Mounjaro pen starts at £169.99 for the 2.5mg starting dose, and a free consultation is reviewed by a GPhC-registered prescriber who will agree a plan with you. You can also see our wider weight loss clinic information.

03 · The weekly cycle
Why Mounjaro can feel weaker late in the week
Mounjaro can feel weaker late in the week because it is a once-weekly medicine whose level in your body gradually falls between injections. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
In practice, most people feel fuller sooner and less hungry between meals while the medicine is working.1 It does this by activating the GIP and GLP-1 receptors, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.
That pattern is not a fault. It is how any weekly medicine behaves. Mounjaro is injected once a week, on the same day each week, at any time of day, with or without food.1 Our related guides on whether Mounjaro wears off at the end of the week and after a few days cover that experience in more detail.
04 · Managing late-week hunger
What helps when hunger returns before your next dose
The most useful response to late-week hunger is to plan for it rather than fight it. Mounjaro supports a reduced-calorie diet; it does not replace one, and it works alongside increased physical activity.1 Treat the quieter-appetite days as the time to build habits, and the hungrier days as the time to lean on them.
- Prepare filling, protein-rich meals and snacks for the days you expect appetite to be stronger.
- Keep to regular meal times so that hunger does not catch you out.
- Check sleep and alcohol, which both affect appetite.
- Keep your injection day consistent. There is no best time of day, because Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1
Many people choose a weekend morning so that early side effects fall on days off. That choice is about convenience, not effectiveness.
If you need to move your injection day, you can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Do not shift doses closer together to chase a stronger effect. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
If late-week hunger is bothering you, raise it at your monthly review. Your prescriber can look at whether your dose is right, since the dose can step up depending on how you respond and tolerate it.
05 · Does it stop working?
Does Mounjaro stop working over time?
Mounjaro does not stop working after a set number of weeks, but weight loss does slow. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.
It also helps to remember the pace the medicine was designed around. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.2
That matters for the feeling that it has stopped working early on. Everyone starts on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself.1 Some people notice appetite change and early loss in the first week, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
If progress feels stalled, the dose ladder is the licensed tool. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1 Our guide to whether effectiveness wears off looks at this in more depth.
Your prescriber also reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.
06 · Plateaus
Plateaus: what is normal and what to try within the licence
A plateau is a normal pause, not a sign that the medicine has worn off. The licensed-dose responses are straightforward and can be tried in order.
- Review food intake honestly, because portions drift back up over time.
- Protect protein intake and keep up activity.
- Check the basics: sleep and alcohol.
- Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.
A plateau is not a reason to double doses or add unlicensed products. Supplements sold as natural alternatives are low-cost and come with no clinical oversight, and their dosing is not standardised, so they are not a substitute for a licensed medicine.
There is also a bigger picture worth keeping in view. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they show why a slower scale is not the only measure of whether treatment is helping.
For expectations at around three months, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
07 · After stopping
How long after stopping Mounjaro do the effects wear off?
The drug itself takes roughly four to five weeks to clear, and appetite control fades as it does. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 You will not feel an abrupt switch. Hunger tends to return gradually as levels fall, rather than all at once.
The more important question is what happens to weight. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 These are trial averages and individual results vary, but they show the direction of travel: regain is common when treatment stops without a plan.
Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14 That does not mean you must stay on treatment forever, and it does not mean regain is inevitable. It means the habits you build while on the medicine are what carry forward, so it is worth deciding in advance how you will eat, move and monitor your weight.
Practical points for a planned stop:
- Agree the plan at a review, not on an impulse after a hard week.
- Expect appetite to return gradually over the weeks as the medicine clears.
- Keep the protein, activity and sleep routines you built during treatment.
- Weigh yourself on a regular schedule and look at the trend, not single readings.
- If you decide to restart later, speak to a prescriber first, because the starting dose and pace need to be set again for you.
08 · What not to do
Stretching doses and microdosing: why they are not the answer
Mounjaro is licensed as a once-weekly medicine; its blood levels are designed around a 7-day cycle.
Altering doses outside the licence means unknown dosing accuracy and no safety data.1
Talk to your clinical team rather than self-adjusting. Our ongoing monthly clinical reviews are there for exactly this conversation.
09 · Storage and potency
Could a weak effect be a storage problem?
Occasionally a weaker effect traces back to how a pen has been stored, so it is worth checking. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1
When travelling, carry pens in hand luggage, because hold luggage can fall below freezing. A cool bag with the prescription label helps, and in-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13
If you think a pen was mishandled, contact the pharmacy before injecting.
10 · Age, belly fat and side effects
Age, belly fat and long-term side effects
Weight loss is gradual and works alongside a reduced-calorie diet and increased physical activity. A plateau at any age is normal, and your prescriber can tailor the approach to you.
On side effects, the commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.1 For the full list of side effects, see the patient information leaflet. If you notice a new symptom, discuss it with your prescriber and report it through the MHRA Yellow Card scheme.19
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.4 Many people who meet the licensed criteria choose regulated private treatment rather than waiting. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber (GPhC 9012878), and individual results vary. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
