01 · The core answer
Is the smell from Mounjaro or something else?
The smell is most likely indirect rather than a direct drug effect. Mounjaro slows how quickly the stomach empties and reduces appetite1, so many people eat and drink less than usual. Drinking less can mean darker, stronger-smelling urine.
Ask yourself three things: am I drinking less, has my diet changed, and do I have any urinary symptoms? If the answer to the first two is yes and you feel well, the smell usually settles once you drink more regularly. If you have urinary symptoms, speak to a clinician rather than waiting.

Diet matters too. Treatment works alongside a reduced-calorie diet and increased physical activity2, and a changed diet, new supplements or strongly flavoured foods can all affect odour.
03 · When to get it checked
When to get it checked
See a clinician if the smell persists after you have improved your fluids, or if it comes with burning, pain, cloudy or bloody urine, fever, back or side pain, or marked thirst. These can have causes unrelated to Mounjaro, so a clinician should assess them.
Do not assume it is harmless because you are on treatment. Tell your prescriber about any new symptom, and you can report suspected side effects through the Yellow Card scheme19.

Dehydration and when to seek help
Yellowing of the skin or eyes needs you to get medical help straight away. Sweet or fruity urine, very dark urine, swelling or persistent tiredness need medical assessment whether or not you take Mounjaro.
Severe vomiting or diarrhoea can also affect the kidneys through fluid loss, so do not ignore it. Get medical help straight away if you cannot keep fluids down.

06 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
