Does Mounjaro cause loose skin or does weight loss?
Weight loss causes loose skin, not the medicine itself. Mounjaro is a once-weekly injection that helps you eat less by making you feel fuller sooner, so the visible changes come from the weight coming off.1 The same changes follow dieting, exercise programmes and surgery.
"Ozempic face" is a good example. It is not a drug effect. It is what losing a significant amount of weight quickly can do to facial volume, on any method including diet or surgery. The same logic applies to loose skin on the tummy, arms and thighs.1
So when people ask whether weight loss injections cause loose skin, the honest answer is: they make substantial weight loss possible, and substantial weight loss can leave extra skin. That is a trade-off to understand before you start.

04 · Why skin loosens
Why loose skin happens after weight loss
Skin stretches to accommodate extra body weight, and when that weight reduces, the skin has to catch up. If the loss is large and fast, there can be a gap between the body shrinking and the skin adapting. That is the whole story at a high level, and it is why the pace of loss comes up so often.
The medicine's role is indirect. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Eating less leads to weight loss, and weight loss is what changes the skin.

05 · Face and body changes
"Mounjaro face", neck and body changes
A thinner or more lined face is the most talked-about change, and it has a simple explanation. Losing a significant amount of weight quickly can change facial volume, and this happens on any method including diet or surgery.1 It is not a drug effect.
The same applies to the neck, chest, upper arms, tummy and thighs. These are areas where many people store weight, so they are where changes tend to show. Clothes often fit differently before the mirror shows much, which is normal during loss.
It is worth separating two things people sometimes blur together. Volume loss in the face can make someone look older or more tired. Loose skin on the body is extra skin that has not yet contracted. They have the same root cause but can feel different, and you may notice one without the other.
If you find the change distressing, tell your prescriber. They can talk through your concerns and the pace of your treatment. This is a personal judgement and there is no single right answer.
06 · Protecting lean mass
How to reduce the risk of loose skin on Mounjaro
Nothing can promise how your skin responds, but you can support your body while the weight comes off. The counter-measures are the ones that protect lean mass: adequate protein, resistance exercise, and a loss pace guided by your prescriber rather than maximum speed.1
Protein. Appetite is lower on treatment, so it is easy to eat too little protein without noticing. Build each meal around a protein source before filling up on anything else. Balanced meals also matter for energy, since tiredness is a recognised common effect, most noticeable in the first weeks and after dose increases.1
Resistance exercise. Strength work asks your muscles to hold on to what they have. It does not need to be a gym programme. Bodyweight exercises, bands or weights at home all count, and gentle activity is a good place to start if you are tired.
A sensible pace. Mounjaro uses a gradual dose ladder. Everyone starts on 2.5mg once a week, and the starting dose is about letting your body adjust, not about weight loss itself. The dose can then step up in 2.5mg steps at minimum four-week intervals, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better. 07 · Will it tighten later
Will saggy skin go away after weight loss?
Skin commonly adapts gradually after weight stabilises, so some improvement over time is common.1
It helps to judge your skin once your weight has settled rather than mid-loss. During active loss everything is still changing, so the picture is incomplete. Taking photos in the same light at regular intervals can give a fairer view than the mirror on a bad day.
For some people with a large amount of excess skin, it does not fully resolve. That is a realistic outcome and worth knowing in advance. If loose skin causes discomfort, rashes or affects daily life, speak to your GP about what options exist.

08 · Skin, hair and side effects
Thinner skin, hair shedding and other effects
Hair shedding is a recognised side effect, but thinner skin is not. Temporary hair shedding is reported as common in trials and is linked to rapid weight loss itself rather than a direct drug effect. For most people it settles as weight stabilises.1 In the weight-management trials hair loss was reported by 4.9% of people taking tirzepatide, compared with 1.0% on placebo.2
Protein intake and overall nutrition matter here too, which is another reason the reduced-calorie diet should stay balanced.1
If you have noticed your skin looking thinner or feeling different, note that this is not a listed side effect in the SmPC or patient leaflet.13 Any new symptom is still worth discussing with your prescriber, and you can report it through the Yellow Card scheme.19
Injection-site reactions, by contrast, are listed. Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days. Rotating sites each week helps. Contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week.1
09 · Creams, collagen and vitamins
Collagen, vitamins, drinks and "tightening" products
Be cautious about claims for collagen, vitamins, drinks or creams for tightening skin. Be cautious of any product promising dramatic results. Low-cost supplements come with no clinical oversight, and dosing is not standardised.
People ask why others take collagen on Mounjaro, and the likely answer is hope rather than proof. Marketing for these products often trades on worry about appearance. If you are thinking of taking something new alongside your treatment, ask a pharmacist or your prescriber first, so it is checked against your medicines and health.
Nothing applied to the skin or swallowed will tighten it immediately, and promises of that kind should make you sceptical. Skin adapting is a gradual process.
10 · Weighing the trade-off
Weighing up loose skin against the benefits
Loose skin is one consideration, not a reason on its own to avoid or start treatment. Mounjaro is licensed in the UK for weight management in adults and for type 2 diabetes, and it is used alongside a reduced-calorie diet and increased physical activity.1
Some people feel the change in their skin is a fair price for the health improvements that weight loss brings; others find it harder. Both reactions are reasonable. What matters is going in with realistic expectations and a plan to protect lean mass.
On access, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first. Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
