Mounjaro contains tirzepatide, which activates the GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.2 The slower stomach emptying is the part that matters for contraception.

A contraceptive pill has to be swallowed, pass through the stomach, and be absorbed in the gut. If the stomach empties more slowly, the medicine can reduce absorption of oral contraceptives. That is the reason the warning exists, and it is the mechanism described in the regulator guidance on contraception with these medicines.7

An implant takes a different route. Nexplanon is placed under the skin and does not depend on your stomach emptying at a particular speed. That is why the pill warning is not simply transferred to the implant.

What Mounjaro actually changes in the body

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.2 The effect on appetite and on stomach emptying is the part of the medicine that touches anything you swallow. Anything that works through the skin, such as an implant, is not part of that picture.

The reason for the caution is specific to oral contraceptive pills.

05 · Weight loss and your contraception

Treatment with Mounjaro is meant to run alongside a reduced-calorie diet and increased physical activity. In the SURMOUNT-1 trial of adults taking tirzepatide, average weight loss over 72 weeks was substantial at higher doses, though individual results vary and trial averages do not predict your own result.2

Contraception planning matters for another reason: some people start weight management while thinking about a future pregnancy. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. If that is you, say so in your consultation. The prescriber can advise how treatment fits with your plans, and you do not have to work that out alone.

If you use a method that depends on swallowing a tablet, ask specifically whether you should add or change something while on treatment. If you rely on an implant, mention the date it was fitted and when it is due for replacement, so your records are accurate. On the choice of arm, the person fitting the implant decides and explains where it goes; it is not something you need to plan around Mounjaro. On side effects people dislike, such as changes to bleeding patterns, that is a conversation for your fitter or sexual health service, and the Nexplanon leaflet lists what to expect. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
