This does not mean your experience is imaginary. Libido is shaped by sleep, stress, mood, relationships, other medicines, health conditions and life stage. Starting a weight-loss treatment is often a period of big change in eating, routine and body image, and any of those can shift desire.

The sensible next step is to note when the change began, what else changed around then, and whether it is steady or comes and goes. That gives your prescriber something concrete to work with.

How to tell whether the medicine is the likely cause

Timing is the first clue, but it is a weak one on its own. A change that begins in the same week as a dose increase is worth noting. A change that crept in over months alongside poor sleep or a stressful period points elsewhere. Neither pattern is proof.

What the product information says about libido

The UK product information for Mounjaro does not list reduced libido as a side effect. The Summary of Product Characteristics is the document prescribers rely on, and the patient information leaflet is the plain-English version that comes in the box.113 Neither names loss of sexual desire as an effect of tirzepatide.

That matters because these documents are built from clinical trials and from safety monitoring after a medicine is on the market. If a pattern were established, it would be added. The absence of an entry is not the same as proof that no individual could ever be affected. It means that the effect is not established.

The honest position is in between: not a recognised effect, and still worth discussing if you notice it.

Why individual reports do not settle it

Online forums are full of personal stories, some saying desire fell and others saying it rose. People who start a weight-loss medicine are also changing food, activity, sleep and often their whole routine. Any single story mixes the medicine with everything else going on.

How to report a new symptom

If you notice something that is not in the leaflet, tell your prescriber and consider reporting it through the MHRA Yellow Card scheme.19 Anyone can report, including patients. Reports are pooled and reviewed by the regulator, which is how genuine new signals get identified. A report from you does not mean you are blaming the medicine. It simply adds a data point.

05 · How the medicine works

How tirzepatide works, and why that is not the same as a libido effect

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

That description covers appetite, digestion and blood-sugar handling. It does not describe an action on sexual desire. Some websites claim otherwise, but this is not established.

Why people wonder about it anyway

The question is natural. If a medicine quietens the urge to eat, it is reasonable to ask whether it quietens other urges. Appetite and desire both involve the brain and hormones, so the idea feels plausible. Plausible is not the same as established, though, and the label does not list the effect.

It also helps to separate two things. Feeling less interested in food is the intended result of treatment. Feeling less interested in sex is not a result the product information describes. If both happen together, they might be connected, or they might simply be happening at the same time.

What the medicine is licensed for

Mounjaro is licensed in the UK for weight management in adults and for type 2 diabetes.1 It is used alongside a reduced-calorie diet and increased physical activity. Those lifestyle changes are part of the treatment, and they are also a source of the big routine shifts that can influence how you feel day to day.

Single-action and dual-action medicines

The practical takeaway: understanding the mechanism does not hand you a libido answer. Treat any claim of a precise brain-reward explanation as unconfirmed, and focus on what you can observe and discuss with a clinician.

06 · Overlapping factors

Everyday factors that can lower desire during weight loss

Several ordinary factors can reduce libido at any time, and they often coincide with starting a weight-loss treatment. Sorting through them is usually more productive than asking whether one medicine is to blame.

Energy and routine

Eating less, changing meal patterns and becoming more active can all leave you feeling different from usual. Tiredness is one of the most common reasons anyone feels less interested in sex. If you are running on low energy because you are adjusting to a new routine, desire can understandably take a back seat. Talk to your prescriber if tiredness is a persistent problem, because that is worth looking at in its own right.

Digestive comfort

When your stomach feels unsettled, intimacy is rarely the first thing on your mind. This applies to any cause of digestive discomfort. If you are having digestive symptoms that interfere with daily life, tell the prescribing team, who provide side-effect support and can advise on what to do.

Mood, stress and sleep

Mood and stress are among the best-recognised influences on libido. Many people begin treatment at a point of high stress about health or body image, or after a difficult period. Poor sleep adds to that. If your mood has dipped, please speak to your GP. Mood changes deserve attention regardless of cause, and you should never feel you have to cope alone.

Relationships and body image

Desire is not purely physical. Changes to how you see your body, how your partner reacts, or how your routines fit together can all play a part. Some people feel more confident as their body changes. Others feel self-conscious while the change is under way. Both are normal, and neither tells you anything about the medicine.

Other medicines and health conditions

Many people who use weight-management treatment also take other medicines or live with conditions such as type 2 diabetes, which can themselves affect sexual function. None of this rules the medicine in or out. It simply shows how many candidates there are, which is why careful observation and a conversation with a clinician beat guessing.

07 · Hormones and contraception

Does GLP-1 treatment mess with your hormones?

GLP-1 and GIP medicines act on gut-hormone receptors that regulate appetite, stomach emptying and insulin response.1 That is a different system from the sex hormones that govern reproduction and, in part, desire. The product information does not list an effect on sex hormones or libido, so it would be wrong to say the medicine disrupts them.

It is true that weight change itself can interact with the body's hormonal balance in some people. If you have symptoms that suggest a hormonal problem, such as changes to your cycle, a GP can arrange sensible tests.

The contraception point that does matter

One hormone-related issue is well established. Mounjaro can reduce the absorption of oral contraceptives because it slows how quickly the stomach empties.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 That is relevant to anyone for whom a pregnancy would be unwelcome, because a contraceptive that is absorbed less well may protect less well.

If you use an oral contraceptive, speak to your prescriber or pharmacist about the best way to stay protected while you are on treatment. Do not assume your usual pill is enough. This is practical advice that has nothing to do with libido, but it often comes up in the same conversation, so it is worth knowing.

Who this affects

The MHRA has published guidance on contraception with GLP-1 and GIP medicines.7 If you could become pregnant, check the current guidance with your prescriber before you start and whenever your dose changes. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Talking about intimacy and sexual health

Clinicians are used to questions about desire, arousal and sexual function. Raising them is not embarrassing for them, and it helps them give better advice. Whatever the cause, being specific about what you are experiencing makes the conversation quicker and more useful.

In short, the medicine acts on appetite and metabolism hormones, the label does not list a libido or sex-hormone effect, and the one hormone-related thing to act on is contraception.

08 · After stopping

Does libido return after stopping a GLP-1 medicine?

If your libido has changed and the medicine is part of the cause, it may improve after treatment ends.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Any change that is directly tied to the medicine would not be expected to resolve instantly, so give things time before drawing conclusions.

Do not stop on a hunch

Stopping a weight-management treatment because of a change that may not even be caused by it can mean giving up benefits for nothing. If libido is a concern, discuss it first. If it does not return

If desire stays low weeks after the medicine has cleared, that points strongly to another cause. Mood, stress, sleep, relationships, other medicines, hormonal changes and underlying health conditions are all worth checking. A GP appointment is the right next step, and it is better to book one sooner than to wait months hoping it resolves.

Restarting or changing doses

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. How to improve libido while on treatment

There is no clinical treatment for libido to recommend here, and supplements or quick fixes are best avoided. What we can offer are sensible, low-risk steps that put you in a better position to work out what is going on.

Start with observation

Keep a simple note for a few weeks. Record your injection day, your mood, your sleep, how your stomach feels and how interested you are in intimacy. Patterns often become visible quickly. Look after the basics

Sleep, stress and mood influence desire more than most people expect. Treat them as part of your plan, not as extras. Follow the diet and activity advice your prescriber has agreed with you, because treatment is meant to work alongside it, and keep regular meals and routines where you can.

Talk to your partner

Desire often responds to feeling close and relaxed, and pressure tends to make things harder. An honest conversation about what is changing, and why you are not sure, can reduce pressure on both sides. It also helps if your partner understands you are looking into it rather than withdrawing.

Ask for a medicines review

Is low libido normal while losing weight?

A change in desire during weight loss is common enough to be worth talking about, and it does not always involve a medicine. Large changes to eating, activity, body shape and daily rhythm touch many parts of life, so it is reasonable for sexual interest to shift too, in either direction.

The trial evidence for Mounjaro is about weight, not libido. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Quick change and how it feels

People who lose weight steadily often say they feel more confident and energetic. Others find the process unsettling, because their body and habits feel unfamiliar. Neither reaction is a verdict on the treatment. What matters is how you are doing overall, and whether you can talk about it.

Setting expectations

Starting treatment is not a promise of any particular change in your wellbeing, positive or negative. A fair expectation is that you will lose weight at a pace that varies from person to person, that you may notice shifts in appetite and digestion, and that other areas of life may change as you adapt. If one of them worries you, you are encouraged to raise it.

NHS or private?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
