GLP-1 medicines copy a natural gut hormone, and the clinical effect is that most people feel fuller sooner and less hungry between meals. For Mounjaro, the mechanism is dual. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1

Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is the key point: the medicine makes eating less easier, but it does not do the dieting for you.

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

04 · Realistic pace

How quickly does it work?

Slowly and by design, so think in months, not days. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.2

What if it does not seem to be working?

A slow start or a stall is not automatically failure, and your prescriber reviews your response on a set timeline. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2

Plateaus are part of the picture too. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.2 Within the licence, the responses are:

  • review food intake honestly, because portions drift back up;
  • protect protein and activity;
  • check the basics, such as sleep and alcohol;
  • discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

A plateau is not a reason to double doses or add unlicensed products.2 If you stop treatment altogether, the medicine does not leave the body at once. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

What are the downsides of GLP-1 medicines?

The main downsides are early digestive side effects, a long-term commitment and the cost. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.2

Other honest downsides are practical:

  • It is a prescription-only medicine, so a prescriber must decide it suits you.
  • It works alongside a reduced-calorie diet and increased physical activity, so it does not remove the effort.
  • Results vary, and some people respond less than the trial averages.
  • You need a plan for after treatment, which is best made with your prescriber.

Does it do more than reduce weight?

In trials it did, though these are secondary outcomes rather than promises. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2

08 · Boosters and supplements

Do "GLP-1 boosters" actually work?

"GLP-1 boosters" are supplements marketed as raising your own GLP-1 levels. They are available cheaply without assessment, with no clinical oversight, and their dosing is not standardised.

The same applies to the nickname "poor man's Ozempic", which is a marketing label usually attached to berberine. Be wary of sellers making dramatic weight-loss claims. Likewise, if anyone offers prescription injections without a consultation or prescription, walk away: that is a safety risk and may involve counterfeit pens.

09 · Mounjaro and semaglutide

What does semaglutide do, and is Mounjaro better?

Semaglutide is the single-action GLP-1 medicine in Wegovy, a licensed weight-management treatment, and in Ozempic, which is licensed for type 2 diabetes only and is not a UK weight-loss medicine. Wegovy comes as pens of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks to the 2.4mg maintenance dose from week 17.18

Mounjaro differs because it activates both GIP and GLP-1 receptors, while semaglutide acts on GLP-1 alone.1 Which suits you depends on your health profile, tolerance and prescriber advice, and your prescriber can advise. You can compare the Wegovy pen with Mounjaro on our site.

No single medicine is best for everyone. Both options are licensed, prescription-only and work alongside diet and activity.

10 · NHS or private

Can you get it on the NHS?

Yes, but access is phased. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46

Do you gain weight back after stopping?

Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 The medicine itself clears the body over about four to five weeks after the last dose.1

Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. That conversation is far better held before the last pen than after it.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
