Mounjaro is a prescription medicine that works alongside a reduced-calorie diet and increased physical activity, and its licensed action is described in terms of appetite, stomach emptying and insulin response.1

The question usually arises because people notice that food, alcohol or snacking feel less compelling after starting treatment. That experience is common enough to be widely discussed. But a change in how you feel about food does not by itself show what is happening in any particular brain chemical pathway.

03 · What dopamine does

What dopamine does in the brain

Dopamine is a chemical messenger involved in motivation, anticipation and learning what is worth seeking out. It is often called the pleasure chemical, but that is a simplification: it is more closely tied to wanting and drive than to enjoyment itself.

Food, social contact, alcohol, nicotine, gambling and many other things can engage the brain's reward pathways. That is why the same circuitry gets discussed whenever someone describes cravings or compulsive habits. It is also why dopamine appears in conversations about appetite, and why we have a separate page on whether dopamine suppresses appetite.

Dopamine is not simply good or bad, and more is not always better. Too little and too much can both be linked to difficulties, which is one reason doctors are cautious about claims that a medicine boosts or lowers it.

04 · Licensed mechanism

What Mounjaro is licensed to do

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Notice what is absent from that description: dopamine. The product information and patient leaflet explain the medicine in terms of gut hormones, appetite and blood sugar.13 Anything beyond that, including effects on reward pathways, belongs to research rather than to the label.

05 · Research and food noise

What researchers are exploring

GLP-1 receptors are found in the gut, but scientists have also been studying whether GLP-1 signalling reaches brain regions involved in appetite and reward.

Many people describe a quieter pull towards food, sometimes called food noise. What can be said is that the medicine helps regulate appetite and that people feel fuller sooner, as the product information describes.1 Whether reward signalling contributes is not established.

If a page tells you flatly that GLP-1 medicines lower, raise or deplete dopamine, treat it with caution. The honest position is that the picture is still developing, and that different pages reach opposite conclusions because the evidence does not yet support a simple answer.

06 · Mood and wellbeing

Do GLP-1 drugs affect mood?

Some people feel better in themselves on treatment, and others feel no different or feel worse.

The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and low mood or suicidal thoughts, and the MHRA keeps this under continuous review.17

In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

If you notice low mood or distressing thoughts, tell your prescriber and seek help promptly. You can also report suspected side effects through the MHRA Yellow Card scheme.19

07 · ADHD, addiction, dementia

ADHD, addictive behaviours and dementia

Questions about dopamine often lead to other conditions. Here is where Mounjaro stands on each.

ADHD. ADHD is a condition with its own diagnosis and treatment pathway. Mounjaro is licensed for weight management and type 2 diabetes, not ADHD.1 If you take medication for ADHD, tell the prescriber during your consultation so they can consider your full medication list. Do not change or stop any existing treatment on the strength of something you have read online.

Alcohol and other addictive behaviours. Some people report drinking less after starting a GLP-1 medicine. Mounjaro is not licensed to treat alcohol problems or any addiction. If you are struggling with alcohol or another dependency, your GP or a specialist service is the right route.

Dementia. People ask whether GLP-1 medicines raise or lower the risk of dementia. Research is ongoing and you should treat headlines with care.

08 · Stopping and clearance

If you stop treatment, does anything change?

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.

Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

What to tell your prescriber

If dopamine is on your mind because of your own history, say so in your consultation. Useful details include any diagnosed conditions affecting attention, mood or impulse control, any current medicines, and any past difficulties with alcohol, eating or other compulsive behaviours.

10 · Evidence and limits

What the trial evidence does and does not show

The best-known weight-management evidence for tirzepatide comes from SURMOUNT-1. In that trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

That trial measured weight and related outcomes. It does not tell us what happens to dopamine in the brain.
