Yes, 7.5mg Mounjaro works as part of a licensed titration, as a step on the dose ladder rather than a destination. Everyone starts on 2.5mg, which is about letting your body adjust rather than weight loss itself. The dose then rises to 5mg, and can step up by 2.5mg at a minimum of every four weeks to 7.5mg, 10mg and 12.5mg, up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
That means 7.5mg is the point where many people are moving beyond the early adjustment phase. Most measurable progress builds from 5mg onward, month by month.11

04 · How it works
How Mounjaro works at 7.5mg
Mounjaro works the same way at every strength: it is the active ingredient tirzepatide, delivered weekly, and the dose changes how strongly that action is felt. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That last part matters. Mounjaro is licensed for use alongside a reduced-calorie diet and increased physical activity, not instead of them.1 The medicine lowers the effort needed to eat less; it does not eat less on your behalf.
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss. What the mechanism does explain is why a dose step can matter. Each step up gives a stronger signal to the same pathways, which is why prescribers titrate gradually: your body adjusts to the effect and the side effects are easier to manage.
Why 7.5mg exists on the ladder
The ladder is deliberately fine-grained. After 2.5mg and 5mg, the licence allows steps of 2.5mg at a minimum of every four weeks. The 7.5mg strength gives your prescriber a measured step between 5mg and 10mg rather than a larger jump. That is helpful if you responded well to 5mg but found appetite returning, or if you want to avoid a bigger step that might bring heavier side effects.

What the mechanism does not do
It does not promise a particular rate of loss, and it does not remove the need for food and activity changes. It also does not work instantly after a dose increase. Because tirzepatide has a half-life of about five days, levels shift gradually, and the full feel of a new strength can take a little while to settle. After a final dose it takes roughly four to five weeks to clear the system; there is no way to speed this up, and nothing needs doing while it clears.1
That slow rise and fall is why early judgement after a single injection is unreliable. A better approach is to watch how you feel across several weekly doses at the same strength before drawing conclusions.

How long does 7.5mg take to work?
7.5mg does not have its own separate timeline; it sits inside the overall pace of treatment, which is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.11
When you reach 7.5mg you are usually well past the starting phase. Your body has had time to adapt to the medicine, and you may notice that the change in appetite from a step up is more subtle than the very first effect you felt. That is normal. Some people notice a clear shift in the days after the first 7.5mg injection; others feel little for a while and then see the scales move over the following weeks.
What to expect in the early months
Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full averages measured at 72 weeks, not at the early stage. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2
Slower early progress does not predict failure. The dose you respond to may still be ahead of you. That is a useful thought if you are impatient on 7.5mg: the medicine has not stopped working just because you have not yet reached the dose that suits you.
The review point your prescriber uses
Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.11 This is a structured checkpoint, not a pass-or-fail test at every step. It is judged on the highest dose you tolerate, so a period at 7.5mg while you step up does not on its own count as failing.

Judging by trend
Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Weigh at the same time of day, on the same day each week if you weigh at all, and look at a rolling picture. Many people also find that clothing fit, appetite and portion sizes tell them more than the scales do in any single week.

What the trial evidence says about higher doses
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result. Some people lose more than the average and some less, and that spread is a real feature of the data, not an error.
Are higher doses more effective?
On those averages, yes: the figures rise from 5mg to 10mg to 15mg. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
It would be tempting to read that as a reason to climb to the top of the ladder immediately. We would not. Those figures come from people who titrated at a measured pace and tolerated each step. Higher is not automatically better for every person, and many people settle at a lower maintenance dose where benefit and tolerability balance well.
What about 7.5mg specifically?
Trial figures are given for 5mg, 10mg and 15mg, not for 7.5mg alone. If you are on 7.5mg, you are in the range where trial averages were building, and further steps remain available if your prescriber judges them appropriate.

Beyond the scales
In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.2
07 · Not working?
Why am I not losing weight on 7.5mg?
The most common reason is simply that weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.11 A stall at 7.5mg does not mean the medicine has stopped working. It often means your body is catching up with the changes it has already made.
Within the licence, there are clear steps to try. Work through them in order before you assume the dose is the problem.
- Review food intake honestly. Portions drift back up over time, and the medicine makes it easier to eat less but does not set the portion for you.
- Protect protein and activity. Keeping protein up and staying active helps you hold on to the changes you have made.
- Check the basics. Poor sleep and regular alcohol both make appetite harder to manage.
- Discuss a dose step-up. If you are not yet at maximum, ask your prescriber whether a step is appropriate.
A plateau is not a reason to double doses or add unlicensed products.11
What stops Mounjaro from working?
Nothing about the medicine itself switches off at a given dose, but several things can blunt the result: not following the weekly routine, missed doses, eating patterns that undo the appetite effect, or simply being early in the process. If you have missed a dose, the rule is to take it as soon as you can within four days (96 hours) of the scheduled time, then carry on with your usual day. If more than four days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within three days of each other.1
Why has my appetite suppression faded?
Appetite effects can feel strongest in the first days after an injection and may feel quieter at the end of the week. A step up in strength is one licensed response if that pattern troubles you, and your prescriber can advise on it. Appetite that returns also sometimes reflects changes in stress, sleep or routine rather than the medicine itself.
Talk to your prescriber
If you are unsure, raise it at your monthly review rather than changing anything yourself. 08 · Side effects
Side effects at 7.5mg and what to do about them
The side effects at 7.5mg are the same kind as at other strengths, and they tend to be most noticeable around dose changes. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If you have just stepped up to 7.5mg, it is sensible to expect a short period of adjustment again.
Many people choose a weekend morning for their injection so early side effects fall on days off. There is no best time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1
If side effects are a problem
If side effects are the concern, your prescriber can hold a dose longer or step down within the licence. That is a normal part of treatment, not a failure.

Reporting something new
If you notice a new or lasting symptom, it is still worth raising. Discuss any new symptom with your prescriber and report it through the Yellow Card scheme.19 Reporting helps regulators spot patterns that no single person can see.
Contraception
If you use oral contraception, the medicine can reduce its absorption because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
Other medicines
If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. Tell your prescriber about everything you take so that any adjustment is planned rather than guessed.
09 · Second time around
Does Mounjaro work a second time, and does age matter?
Whether you stopped for side effects, cost, a holiday or because you felt you had reached your goal, the sensible route is to speak to a prescriber about where to restart rather than assuming you can go back to your old dose.
The medicine takes roughly four to five weeks to clear the system after a final dose, and nothing needs doing while it clears.1 If you have been off for a while, your prescriber will tell you whether to start again from the beginning of the ladder or pick up closer to where you left off. That is a clinical judgement based on how long you have been away and how you tolerated the medicine before, so do not guess it.
Why a restart is not the same as the first time
Your body, routine and circumstances will have changed. Appetite may behave differently, and side effects may show up again at the dose you restart on. Starting low is not a setback: the licensed ladder begins at 2.5mg precisely so that tolerability comes first.
What about age?
People often ask about the hardest age to lose weight. Response is individual, and it is shaped by many things including medicines, sleep, activity and underlying conditions. If you are concerned about how your age or health affects treatment, the right person to ask is your prescriber, who can look at your whole picture.
What helps most on any dose

10 · NHS and private
NHS or private: how do you get 7.5mg?
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Genuine product matters
Counterfeit and unlicensed injectables are a real risk. Be wary of anyone offering to supply without a consultation or prescription, or of anything described as a Mounjaro pill, because Mounjaro is an injection. The MHRA publishes counterfeit alerts and runs the Yellow Card scheme.7
11 · Using the pen
Using your 7.5mg pen correctly
Getting the technique and routine right helps 7.5mg work as intended. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1
Changing your injection day
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1
Storage and travel
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements; for hot climates use an insulated pouch, and check destination rules for prescription injectables before you fly. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.13

Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
