For most people, progress slows rather than the medicine stopping. 11 Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.
That matters because a plateau and a real loss of effect need different responses. A plateau is managed with a review of habits and dose. A genuine lack of response is judged over a longer period, on the highest dose you can tolerate, by your prescriber. What to do when your GLP-1 seems to have stopped working
Start with the licensed, low-risk checks before you consider anything else. Work through them in order and write down what you find, because your prescriber can act on specifics far better than on "it stopped working".
- Confirm which dose you are on and how long you have been on it.
- Check you have not missed or delayed doses, and that pens are stored properly.
- Review food intake honestly, including drinks and snacks.
- Protect protein and keep active, and look at sleep and alcohol.
- Book a review and ask whether a dose step-up is appropriate if you are not yet at the maximum.

03 · Why plateaus happen
Why weight loss plateaus on a GLP-1
Plateaus happen because the body adapts as weight falls, so the same routine produces less change than it did at the start. 11
Mounjaro manages weight, but it does not cure the biology behind it. That is why the effect on appetite is something you keep using, not a one-off reset. It is also why a stall can feel like the medicine fading when it is really the body defending a new, lower weight.
The headline trial results show how gradual this process is. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. 2 These are trial averages and individual results vary. Averages built up over many months, not in a straight line, so a flat patch is part of the picture for many people.
Judge progress by the trend across weeks, not by a single weigh-in. Early loss includes water weight, and day-to-day readings move for reasons unrelated to fat. 1
04 · Dose may be too low
When the dose is the issue
If appetite control feels weaker, the dose you are on may simply not be the one that works for you yet. Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself. 1
After the starting phase the dose rises to 5mg, and can then step up in 2.5mg stages (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Steps are taken after at least 4 weeks on each dose. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace. 1

If you are already on the top dose and still stalled, the next conversation is a review of response rather than a bigger dose, because the licensed ceiling is 15mg.
05 · Habits and routine
Habits that quietly undo progress
Food habits drift over the months, and that is one of the commonest reasons a GLP-1 appears to weaken. Within the licence, the responses to a plateau are to review food intake honestly because portions drift back up, protect protein and activity, and check the basics such as sleep and alcohol. 11
Treatment is designed to work alongside a reduced-calorie diet and increased physical activity, not instead of them. When appetite is quieter, it is easy to eat smaller meals but add back liquid calories, snacks or alcohol without noticing. Protein and movement matter because they support the body as weight falls. Keep activity that you can sustain rather than a short burst you will drop. Sleep and alcohol are worth checking, as both can push appetite and choices in the wrong direction.
A plateau is not a reason to double doses or add unlicensed products. 11 If the habits check finds nothing, that is useful information to bring to your prescriber.
06 · Pen and injection checks
Check your pen, storage and injection routine
Practical mistakes can make treatment seem weaker, so check the pen routine before assuming the medicine has failed. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C. 13
Keep the pen cap on to protect it from light, and never use a pen that has been frozen. Unopened pens should stay in the fridge and keep until the printed expiry date. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels. 13
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Rotate the site each dose. 1 There is no best time of day, so what matters is consistency.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day, and never take two doses within 3 days of each other. 1 Repeated gaps can leave appetite control feeling patchy.
07 · Stopping and breaks
What happens if you take a break or stop
Stopping a GLP-1 usually leads to weight regain on average, so any break should be planned. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. 14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears. 1 That means appetite changes can take a while to return, and the full effect of a break only shows over the following weeks.

08 · What not to do
Shortcuts to avoid when progress stalls
Mounjaro is a once-weekly medicine, with blood levels designed around a 7-day cycle. 1
Altering doses outside the licence means unknown dosing accuracy and no safety data. 1
Talk to your clinical team rather than self-adjusting.

09 · Formal response review
When your prescriber formally reviews response
A formal review point exists so that a lack of response is judged properly, not on a bad fortnight. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. 4
The review is based on the highest dose you can tolerate. Someone held at a lower dose because of side effects is in a different position from someone at the top of the ladder. Your prescriber weighs both.
Beyond the scales, trial participants saw average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises. 2 So a scale that has stopped moving does not tell the whole story, and your clinician can look at the wider picture.
If a different licensed medicine might suit you better, that is a conversation for your prescriber. NHS access to Wegovy runs through specialist weight-management services, so ask your GP.

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
