Do GLP-1 medicines work immediately?
No. A GLP-1 medicine is not a switch that works from the first injection. Mounjaro, for example, is a once-weekly injectable prescription medicine, and treatment begins on the lowest strength, 2.5mg, which is designed as an adjustment phase.1
So the honest answer has two halves. Some effects, mainly on appetite and fullness, can be felt early. The weight loss that changes your health takes months.
02 · Early vs late effects
What happens early, and what takes months
Early effects are about appetite and digestion. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals.1 That can start soon after treatment begins, but it varies.

04 · The first days
The first days after your injection
In the first days, the commonest experience is side effects rather than weight loss. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13
Some feel a clear drop in appetite. Others feel only a slight queasiness, and some notice almost nothing. Your body is adjusting to a medicine that slows stomach emptying, so digestion can feel different.1
Practical points are modest. Follow the instructions in the patient leaflet, keep your weekly injection routine consistent, and tell your prescriber about anything that worries you. If you notice a symptom that is not on the patient leaflet list, tell your prescriber about it and you can report it through the Yellow Card scheme.19

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
05 · Early scale changes
Why early scale changes can mislead
Early weight loss includes water weight, so the trend over weeks matters more than any single weigh-in.13 A big early drop can feel exciting, but it is not a reliable indicator of how treatment will go. A quiet start can be equally misleading.
Water weight and reduced eating in the early days can show quickly on the scale. That is not the same as fat loss, and the pace usually settles. If you feel unwell or your loss feels out of proportion to how you are eating, speak to your prescriber rather than waiting it out. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.
Judge progress by trend. Weigh at the same time of day, on the same scales, and look at how the pattern moves over several weeks. Waist measurement and how your clothes fit can add useful context.
06 · Realistic expectations
Realistic expectations over months
The headline trial results took far longer than a few weeks to appear. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Those figures were measured over 72 weeks, not over a few weeks. For a worked example of scale, for someone starting at 100 kg, 20.9% is about 21 kg, but that is the trial average at the highest dose after a long treatment period and not a prediction for you.
People often ask how much they will lose in two months or three months. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2 A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure — the dose you respond to may still be ahead of you.2
07 · Dose escalation
Why dose escalation takes time
Treatment steps up gradually, so the dose that works best for you is reached over a period of months. Mounjaro comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, supplied as a pre-filled multi-dose KwikPen.1 Starting low gives your body time to adjust and is the reason there is no instant effect.
Wegovy follows the same principle with a different ladder: 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 If you are comparing options, the Wegovy pen page shows how that medicine is supplied.
A faster climb is not better. Never double a dose or add extra injections to speed things up. Any change in dose should be decided by your prescriber, who can discuss whether a step-up is appropriate.
08 · When progress stalls
Why it might not seem to be working
A slow start is common, and a plateau later is normal too. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1
If you are asking why tirzepatide or another GLP-1 medicine is not working for you, work through the same checks in order:
- Review food intake honestly, because portions drift back up.
- Protect protein and activity.
- Check the basics, such as sleep and alcohol.
- Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.
A plateau is not a reason to double doses or add unlicensed products.1
Formal review matters too. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That is a conversation about the right plan, not a verdict on you.
09 · Beyond the scales
Benefits that do not show on the scales
Some changes are internal and are not visible day to day. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises.

10 · Natural and quick-fix claims
Be wary of products promising instant GLP-1 effects
Anything promising an instant, injection-like result should be treated with caution. Low-cost supplements marketed as natural alternatives carry dramatic weight-loss claims but have no clinical oversight, and they are not equivalent to a licensed prescription medicine. Counterfeit and unregulated products are a real risk. Only buy from a registered pharmacy, and be suspicious of any seller offering to supply without a consultation or prescription. 11 · Stopping and clearance
How long the medicine stays in your system
Just as it is not immediate to start, it is not immediate to stop. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
Appetite effects can therefore continue for a while after your final injection and then fade gradually. Talk to your prescriber before stopping, so you can plan what comes next.
