How GLP-1 medicines work

GLP-1 medicines copy a natural gut hormone that helps regulate appetite, slows how quickly the stomach empties and improves how the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

That mechanism explains most of the common challenges. A slower-emptying stomach means large portions sit heavily, rich food can feel unpleasant, and nausea is a recognised early issue. 04 · Common challenges for users

Common challenges GLP-1 users face

The most common challenges are practical and centre on eating, drinking and routine. Because the stomach empties more slowly, large portions sit heavily, so smaller, slower meals work better.

Many people feel worse after very fatty, fried or very sugary food. No food is banned, but prioritising protein at each meal, vegetables, fibre and regular fluids makes treatment easier and more effective. Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.

Other everyday challenges include:

  • Low appetite making protein hard to reach. Some of any rapid weight loss comes from lean tissue. Include a protein source at every meal and do resistance exercise two or more times a week, which matters more as the dose rises.
  • Hydration. Reduced appetite means less fluid from food. Vomiting or diarrhoea can dehydrate you quickly, so keep water within reach and get medical help straight away if you cannot keep fluids down, as dehydration needs medical help straight away.
  • Coffee and alcohol. There is no interaction with caffeine, but strong coffee on an empty stomach can aggravate nausea or reflux for some people. There is no direct interaction with alcohol, but drinking on a reduced appetite can hit harder than usual.
  • Nutritional gaps. No supplement is required, but a standard daily multivitamin is a reasonable safety net while losing weight.

05 · Headaches, gastroparesis, pancreatitis

How common are headaches, gastroparesis and pancreatitis?

Headaches are not listed as a side effect in the Mounjaro patient leaflet. If you develop a new or worrying symptom, tell your prescriber and report it through the Yellow Card scheme.19

Gastroparesis. GLP-1 medicines slow how quickly the stomach empties, which is part of how they work.1 Slower stomach emptying is listed as an uncommon side effect in the patient leaflet.13 Prescribers take extra care, or may recommend against treatment, if you have severe gut or digestive disease. Pancreatitis. Serious side effects are uncommon but need urgent attention. Severe, persistent stomach pain that may spread to your back, with or without vomiting, could be pancreatitis: stop using Mounjaro and get medical help straight away. 06 · Warning signs needing urgent help

Warning signs that need urgent help

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, as this could be pancreatitis.13

  • Pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could signal gallbladder problems: get medical help straight away.
  • Swelling of the face or throat and difficulty breathing, which are signs of a severe allergic reaction: get medical help straight away.
  • Shakiness, sweating or confusion if you also take insulin or a sulfonylurea. Your prescriber may need to adjust those medicines.
  • Being unable to keep fluids down: contact your clinical team, as dehydration needs medical help straight away and can indirectly strain the kidneys.

Mounjaro is not known to damage the kidneys or liver directly. The practical risk is indirect, through dehydration after severe vomiting or diarrhoea.1

07 · Who should not take them

Who should not take a GLP-1 medicine?

You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.1 Mounjaro should be stopped at least one month before trying to conceive, and if you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may advise against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems.

On the thyroid, rodent studies found C-cell tumours with tirzepatide. It is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2.1

On mood, the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link between GLP-1 medicines and suicidal thoughts. The MHRA keeps this under review. Even so, tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

08 · Supplements and 'natural' options

'Poor man's Ozempic' and natural alternatives

No supplement is a GLP-1 equivalent. Products marketed as 'natural GLP-1 boosters', or nicknamed 'nature's Ozempic' or the 'poor man's Ozempic', are low-cost supplements with no clinical oversight, and dosing is not standardised. They are not substitutes for prescribed treatment.1 Dramatic weight-loss claims attached to them should be treated with scepticism.

Oral supplements may also be absorbed more slowly when you start a GLP-1 medicine. If you want to take anything beyond a standard multivitamin, discuss it with a pharmacist first.

Is there a tablet equivalent to Mounjaro? There is no tablet form of Mounjaro, because tirzepatide is injection-only in the UK, but licensed GLP-1 tablets exist, including Wegovy tablets and Foundayo. They are different medicines from tirzepatide, with their own dosing, eligibility checks and trial results. Our product pages show current availability. What results look like in trials

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

In trials, weight loss on tirzepatide also came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures. These are trial-average secondary outcomes, not promises.2

10 · NHS or private access

NHS or private access

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

How can I mimic Ozempic naturally?
You cannot. Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply it for weight loss. Supplements sold as natural GLP-1 boosters are not equivalents and are no substitute for prescribed treatment. What does help is the lifestyle foundation that treatment is licensed alongside: protein at every meal, vegetables and fibre, regular fluids, regular moderate activity and resistance exercise.
Is there a tablet equivalent to Mounjaro?
There is no tablet version of tirzepatide. Licensed GLP-1 tablets such as Wegovy tablets and Foundayo are different medicines, with their own dosing and trial results.
How long does a GLP-1 medicine stay in the body?
For Mounjaro, tirzepatide has a half-life of about five days. After a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears. Other GLP-1 medicines have different profiles, so check their product information.
