A 15mg Mounjaro pen costs £299.99 at our standard price, which works out at £75.00 per week because each pen holds four weekly doses. 04 · What 15mg means

What 15mg Mounjaro is, and why nobody starts there

15mg is the highest licensed weekly dose of Mounjaro, and it is a step on a ladder rather than a starting point. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹ Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.¹

If you are a new patient, you cannot buy a 15mg pen as a first-ever dose. The price that applies to you in the first weeks is the 2.5mg starting pen, followed by 5mg and so on. Each pen still holds four weekly doses, so a 15mg pen covers four weeks at 15mg.

Who is Mounjaro for? It is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. 05 · What trials show at 15mg

What the SURMOUNT-1 trial showed at 15mg

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

The trial also reported how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

To make that concrete with a worked example: for someone starting at 100 kg, 20.9% is about 21 kg, while 15% is about 15 kg and 19.5% is about 19.5 kg. Those are straight conversions of the trial averages, not a prediction for any one person, and trial averages do not predict your own result.

In the trial, the average loss was 19.5% on 10mg and 20.9% on 15mg. That is not a reason to avoid 15mg, because some people need it and a prescriber can recommend it. It is a reason not to assume the highest dose is the right dose for you. Many people settle at 5mg, 10mg or 15mg, and your prescriber will guide you based on response and tolerance.¹

Your prescriber reviews your response at your monthly check-ins and can discuss whether your current dose is right.

One more point on honesty: these trial results were measured at 72 weeks. Think in months, not days. Weight loss on Mounjaro is gradual by design, and the weeks at lower doses are part of the process, not a delay to be skipped for a faster result.

06 · Cost of reaching 15mg

What it costs to work up to 15mg, and what a year looks like

Reaching 15mg means paying for every pen on the way up, so the true cost of the highest dose includes the titration. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. A new customer's first order is £40 less, and every later order is £25 less.

After that, the next steps are 12.5mg at £289.99 per pen and 15mg at £299.99 per pen. A first year (52 weeks, 13 pens) costs £2,399.88 if you settle at 5mg, up to £3,279.88 at 15mg. Both figures are at standard prices before discounts. For 7.5mg, 10mg or 12.5mg maintenance, the monthly pen prices are £244.99, £274.99 and £289.99 respectively.

These are before discounts. Your first order is £40 less with code FIRST40, and every later order is £25 less with code DOSE25, so your real spend will be lower than the table.

What should you take from this? First, the headline price of a 15mg pen is only part of what a year at that dose costs. Second, the gap between settling at 5mg and at 15mg in the first year is the difference between £2,399.88 and £3,279.88.

Third, price is only comparable if you compare like with like. A competitor's first-order 15mg price against our standard 15mg price tells you nothing about what either costs on pen three. 07 · Fake pens and low prices

Why a very low 15mg price is a warning sign

A price far below the rest of the market is more likely to signal a fake pen than a bargain. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

The safest rule is to buy only from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.¹² Anyone offering to supply 15mg pens without a consultation or prescription is not offering genuine treatment, however attractive the price.

How does genuine supply work? Mounjaro is manufactured by Eli Lilly, and genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies. Every pen carries a batch number and expiry date that can be checked.⁷ Our own stock comes through official LillyDirect supply, directly from Eli Lilly's UK distribution, and our GPhC registration number is 9012878, which you can look up on the GPhC register.

When your pen arrives, check it. A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹³

Practical red flags include a seller with no visible registration number, no questions about your health, pens sold loose without packaging, and prices that undercut every regulated provider. If something goes wrong with any medicine, you can report it through the MHRA Yellow Card scheme.

The point of comparing prices is to spend less on the same genuine product. It is not to reach a number so low that nobody could source a real pen for it. Across the UK pharmacies we track, the spread between providers for the same dose is £20 to £36.03. That is a real gap, but it is a gap between regulated pharmacies, not between regulated and unregulated sources. A figure far outside the tracked range is the one to be suspicious of.

08 · No stretching or extra doses

Saving money by stretching pens is not safe

Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.

Talk to your clinical team rather than self-adjusting.¹

Each Mounjaro KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it.¹

Altering doses outside the licence means unknown dosing accuracy and no safety data.¹

So where can you save legitimately? Compare the price of the dose you actually need across regulated providers and use the first-order and reorder discounts. Each of those lowers cost without leaving the licence. None of them involves guessing at how much medicine you are injecting.

If cost becomes the barrier to continuing at all, tell your prescriber. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

09 · NHS or private

NHS or private: the honest picture for 15mg

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you qualify under the current NHS stage, the NHS route is worth asking your GP about, because the cost to you is the standard prescription charge in England, £9.90 per item. NHS dose decisions follow the same licensed ladder. The NHS guide to tirzepatide gives the patient-facing details.⁵

Private treatment is open to a wider group. Which should you choose? There are three realistic situations.

• You meet the current NHS criteria: ask your GP. It may cost you far less than any private price, though availability depends on local rollout.
• You meet the licence but not the current NHS stage: private treatment is a regulated option now, and you can move to NHS care later if you become eligible.
• You are already on 15mg privately: compare standard prices across regulated pharmacies and use the first-order discount on your first order with us. A prescriber confirms suitability.

For the lowest possible cost, the NHS is the answer when it is available to you. For everyone else, the practical goal is a genuine, prescribed pen at a transparent price with proper follow-up. We would rather you understood that than assumed private prices are your only option.

10 · Using your pen

Using a 15mg pen: injecting, missed doses and storage

A 15mg pen is used the same way as every other strength: one injection a week, on the same day. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.¹ Needles, a sharps bin and injection guidance are included with every order.

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people pick a weekend morning so early side effects fall on days off.¹

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹ You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹

Storage affects whether the pen you paid for stays usable. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹³

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check the patient information leaflet and contact the pharmacy before using it, rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.¹³

Travelling with a 15mg pen is straightforward. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹

These handling points are worth knowing before you compare prices, because a pen damaged by freezing or heat is wasted money however little you paid. Do not store it in a freezer compartment or against the back wall of the fridge where it can freeze.

11 · Pace and side effects

Realistic pace, early weeks and reviews at higher doses

Weight loss on Mounjaro is gradual by design, so a 15mg pen is the end of a long process, not a quick fix. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.²

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹ That review is also the moment to ask whether you really need 15mg, or whether a lower dose is enough.

The early weeks look different for different people. Some notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹

How does it work? Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide.¹

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

Side effects can be more noticeable after a dose increase, which is one reason the ladder moves in steps of at least 4 weeks. 12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
