Which GLP-1 is the cheapest?

The honest answer is that it depends on the medicine, the dose and the provider, and a low starting price can be followed by much higher maintenance prices. The three injectable GLP-1-type options most UK readers compare are Mounjaro (tirzepatide), Wegovy (semaglutide) and liraglutide (Saxenda and similar products). Each has a different dosing pattern, and that shapes what you pay across a year.

Mounjaro is a once-weekly KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Wegovy is also once weekly, with a ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg. It steps up after at least four weeks and reaches the 2.4mg maintenance dose from week 17.18 Liraglutide is supplied at 6mg per millilitre and is a daily injection, unlike the weekly pens.18

The monthly price of one pen is only part of the picture. Wegovy reaches its maintenance dose from week 17, whereas Mounjaro maintenance depends on the strength you and your prescriber settle on, anywhere from 5mg up to 15mg.

Trial results from different medicines should not be compared as if they were head to head, so do not choose on price using them. A medicine that costs less per month but suits you poorly, or that you stop early, is not cheaper in any meaningful sense. Ask your prescriber which option fits your health history first, then compare prices for that option.

When you compare providers, check the same dose, the same month, and what is included: consultation, delivery and follow-up. A headline starting price that excludes any of these is not the same product as one that includes them. 05 · NHS versus private

NHS or private: the honest picture

NHS treatment is the lowest-cost route for people who qualify, but access is being phased in over several years. Mounjaro is available on the NHS, and NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.6

In England, the NHS prescription charge is £9.90 per item for people who pay it, and some people are exempt. That charge only applies once you are actually prescribed the medicine on the NHS, which depends on meeting local criteria and where the rollout has reached. Ask your GP surgery what applies to you. For Wegovy, NHS access runs through specialist weight-management services; ask your GP.

Many people who meet the licensed criteria but not the early NHS groups choose regulated private treatment rather than wait. That is a fair choice if you can afford it, but it is not a requirement, and nobody should feel pushed into private care. If you are in a priority NHS group, ask your GP first.

When private makes sense

Private does not mean unregulated

Whichever route you take, a genuine prescription medicine involves a clinical consultation and a prescriber. Anyone offering to supply without a consultation or prescription is not offering genuine treatment, however low the price looks.

What a GLP-1 costs over a year

A first year of Mounjaro costs £2,711.88 at standard prices if you settle at 5mg, and up to £3,599.88 if you settle at 15mg. These totals cover 52 weeks, which is 13 pens, before any discounts. A new customer's first order is £40 less, and every later order is £25 less than the standard price.

The dose ladder is the main cost driver. Mounjaro starts at 2.5mg and steps up after at least 4 weeks on each dose; if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. How to keep the cost down safely

In 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

As a worked example only, for someone starting at 100 kg, 20.9% is about 21 kg. That is straight arithmetic of the trial average, not a prediction for you, and trial averages do not predict your own result. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

At 72 weeks in SURMOUNT-1, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Your prescriber decides with you which dose suits you; higher is not automatically better.

Tirzepatide versus semaglutide

In the first large head-to-head trial (SURMOUNT-5), adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy); both are effective, licensed treatments, and which suits you depends on your health profile, tolerance and prescriber advice.15 The practical point for price-led readers is that the cheaper pen is not automatically the better value if it suits you less.

How Mounjaro works

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That dual action distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. Nothing needs doing while it clears, and nothing speeds it up.1

Ask the questions that decide value: are you eligible, do you tolerate it, and will you keep up the diet and activity changes alongside it? A pen you cannot use properly costs the same as one that works.

08 · Who can use it

Are you eligible for a prescription GLP-1?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you. The decision is clinical, not a price decision, and it is why a low price is never a reason to skip the consultation. If you are not eligible or the medicine is unsuitable, the order is refunded automatically in full.

Why eligibility affects the cost

Mounjaro can reduce absorption of oral contraceptives, because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take an oral contraceptive, speak to your prescriber before starting. If you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar, which your prescriber will plan with you. 09 · Cheap done safely

Cheap without taking risks

The cheapest-looking GLP-1 can become the most expensive if it is fake or unsafe. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified.7 Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.

Are there cheaper alternatives to Mounjaro?

There are lower-priced options, but cheaper is not the same as equivalent. The alternatives are a different prescription medicine such as Wegovy or liraglutide, or non-drug support such as diet and activity programmes. Which is right depends on eligibility and your prescriber's advice.
