Can you take Mounjaro if you have high blood pressure?

Often the answer is that a prescriber can consider it, but it is never a yes or no that a website can give for you. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly.1 In the UK it is licensed for weight management in adults and for type 2 diabetes.1 High blood pressure is a very common condition, and it is not on our list of automatic reasons to decline an order.

What matters is the full picture. A prescriber will want to know how well your blood pressure is controlled, which tablets you take, and whether you have any heart, kidney or circulation history. They will also want to know about any past blood clots. That is why the consultation is a genuine clinical review rather than a tick-box form.

What the prescriber reviews when you have high blood pressure

The prescriber is looking at whether treatment is appropriate and safe for you specifically. For someone with high blood pressure, the review typically covers several areas.

  • How controlled your blood pressure is. Recent readings are more useful than a reading from years ago.
  • Which medicines you take. The names and doses of every tablet, including anything bought over the counter.
  • Other conditions. Heart, kidney, circulation, diabetes and any history of blood clots.
  • Your weight and goals. Mounjaro is licensed for weight management in adults, so the reason for treatment matters.1

04 · Blood pressure tablets

Mounjaro and blood pressure medicines

Do not stop, reduce or change any blood pressure tablet on your own because you are starting a weight-loss injection. Any change to your prescription should come from the clinician who prescribes it.

What you should do is tell us about every medicine you take. The prescriber checks that list against the product information.1 Where something needs attention, they will tell you what to do and, where relevant, suggest you speak to your GP.

Two principles are worth knowing. First, Mounjaro slows how quickly the stomach empties, which is part of how it works.1 That is why contraceptive pills can be absorbed less reliably. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Second, if you also take insulin or a sulfonylurea, those doses may need adjusting because of the risk of low blood sugar. That does not mean they cannot be used together.

05 · Does it change blood pressure?

Does Mounjaro affect blood pressure?

The UK Mounjaro patient leaflet lists dizziness and low blood pressure as common side effects, affecting up to 1 in 10 people.

Keep measuring your blood pressure the way your GP or nurse has advised. If you have a home monitor, keep using it at the same time of day and write the results down. Take that record to your reviews. If your readings change noticeably, or you feel unwell, tell your prescriber or GP rather than waiting for the next scheduled check.

A history of blood clots is exactly the kind of detail that must be disclosed, because it changes how a prescriber weighs up treatment.

The same applies to a previous heart attack, stroke, irregular heartbeat or heart failure. If you have chest pain, a racing heart, fainting or difficulty breathing, get medical help straight away. These are not covered by a blanket yes or no. The prescriber reads your answers, may ask follow-up questions, and decides whether it is appropriate to supply. The product information is the reference they work from.1

Practical tips for the consultation:

  • Name each condition and roughly when it happened.
  • List the medicines you take for it, including blood thinners.
  • Mention any specialist who looks after you.
  • Say if you have been told to avoid certain medicines in the past.

07 · Who should not take Mounjaro

Who should not take Mounjaro, and what to avoid

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.113 Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Some other things are worth stating plainly:

  • Mounjaro is a prescription-only medicine. Anyone offering to supply it without a consultation or prescription is not operating safely.
  • It is used alongside a reduced-calorie diet and increased physical activity. It is not a substitute for them.
  • Do not use leftover pens from someone else, or pens from unverified sellers.
  • Do not adjust your own dose, or use extra doses, to speed things up.

What the weight-loss evidence shows

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Those averages do not predict your own result.

It also helps to understand how the medicine works. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

09 · NHS or private route

NHS or private: which route if you have high blood pressure?

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Your GP is the right person to ask where you currently sit. If you choose the private route, the medicine is the same genuine Mounjaro, supplied through official LillyDirect supply from Eli Lilly's UK distribution.

11 · Lifestyle alongside treatment

Losing weight with high blood pressure: the whole picture

Mounjaro is used alongside a reduced-calorie diet and increased physical activity.1 For someone with high blood pressure, that framing matters, because the diet and activity side of treatment is something you control every day.

Sensible, general habits to discuss with your GP or practice nurse include:

  • Keeping a regular record of your blood pressure at home, if advised.
  • Eating a varied diet with plenty of vegetables, and keeping an eye on salt, as general NHS advice suggests.
  • Building physical activity up gradually, at a level your clinician is happy with.
  • Being cautious with drinks and products marketed as quick blood-pressure fixes. There is no shortcut that replaces your prescribed treatment.

None of that replaces medical advice. If a supplement, tea or drink is promoted for lowering blood pressure or for dramatic weight loss, treat the claim with scepticism. Low-cost supplements with no clinical oversight are not equivalent to a licensed medicine, and some can interact with the tablets you already take.

If you stop Mounjaro, tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system, and nothing needs doing while it clears.1 In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. Speak to your prescriber before stopping, so that your plan and your blood pressure monitoring continue sensibly.

13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
