Why Mounjaro is a weekly injection
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly.1 The weekly interval is part of how the medicine was developed and licensed, not a marketing choice. The patient information leaflet and the summary of product characteristics both describe weekly use, and your prescriber works from those documents.13
Tirzepatide has a half-life of about five days.1 A half-life is the time it takes for the amount of medicine in your body to fall by half. With a half-life of that length, a weekly injection tops up the medicine before it has largely cleared. That is the logic behind the licensed schedule.
The way the medicine works is also relevant. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That steady effect between meals depends on the medicine being present consistently, which is what regular weekly dosing is designed to provide.
It also helps to understand what a pen is. The KwikPen is a pre-filled multi-dose pen that you use at one strength over the course of your weekly injections. You do not choose how many days apart to inject, and you do not combine pens. Your prescriber chooses the strength, and you inject on your usual day each week.

05 · Spacing doses out
What happens if you space doses out
Because the medicine clears over time, leaving longer gaps than licensed means the medicine you inject is no longer working as the product information describes.
Many people hope that a longer gap will reduce side effects while keeping the benefit. Taking it every two weeks is unlicensed, makes levels rise and fall unpredictably, and has no trial evidence for keeping weight off. Appetite control is one of the main reasons people take the medicine, and the effect on appetite is linked to how the medicine works on gut-hormone receptors.1
There is also a practical risk. Once you start improvising with the interval, it becomes hard for your prescriber to judge whether a particular dose is working for you. If your weight stalls, or side effects return, nobody can tell whether the strength is wrong or the schedule is. Honest information to your prescriber is what allows safe adjustments. Tell them exactly how you have been injecting, even if you think it was not ideal. They will not judge you, and they need the real picture.
The same caution applies to the closely related questions people ask. Alternating weeks, injecting every ten days, or leaving a pen to last much longer than usual are all changes to the licensed pattern. If you have already done one of them, do not try to make up for it by injecting extra. Contact your prescriber and ask what to do next.
If you notice a new or lasting symptom after changing how you inject, tell your prescriber. 06 · Pen life and storage
Pen life, storage and why you cannot stretch a pen
An in-use Mounjaro KwikPen is licensed for up to 30 days (not above 30°C) out of the fridge, and that limit matters if you are thinking about spacing doses.1 A pen is built around a weekly routine over its in-use life. If you inject less often so that the same pen lasts much longer, you run past the period the pen is licensed for. At that point sterility and potency are no longer assured.
Storage works like this. Keep unused pens in the fridge until first use, at 2–8°C, and they keep until the printed expiry date.13 Once you start using a pen, it can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. If an unopened pen has been left out of the fridge by accident, check the patient information leaflet and contact the pharmacy that supplied it.
People sometimes ask whether they can use a pen for a long time on the grounds that there is still medicine inside. Visible liquid does not tell you the pen is still reliable. The in-use window exists because the product has only been tested within it. If you are past the window, do not use the pen. Speak to your prescriber about a replacement instead.
A few practical habits keep pens within their life:
- Write the date you first used a pen on it or in your phone, so you always know where you are against the in-use limit.
- Keep pens upright and protected from damage, and follow the pen's instructions for each injection.

- Do not share a pen. It is prescribed for you.

07 · Skipping and breaks
Skipping a week, taking a break or restarting
If you skip a dose, tell your prescriber and follow the patient information leaflet rather than doubling up. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other. What we strongly advise against is injecting extra to catch up, or treating an occasional skip as a new routine.
Occasional missed doses and a deliberate fortnightly routine are different things. A one-off skip is an accident or an interruption, and you return to the licensed schedule. A fortnightly routine would be a planned change to the interval, which is not what the medicine is licensed for.
A longer break is another situation. After a final dose, tirzepatide takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That helps explain why people notice changes in appetite gradually after stopping rather than overnight, though how you feel is individual.
If you have had a break and want to restart, do not simply jump back to your old strength because it is the one you remember. Speak to your prescriber first. They will consider how long you have been off and how you feel, and they decide the strength you restart on. 08 · Other medicines
What to avoid and which medicines need care
Always tell your prescriber and pharmacist about every medicine you take, because some need extra thought alongside Mounjaro. This is the case whatever schedule you follow, and it matters even more if you change how often you inject, as that makes the picture harder to interpret.
- Oral contraceptives. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro can reduce absorption of oral contraceptives, because stomach emptying slows.7 Ask your prescriber what protection to use.
- Insulin and sulfonylureas. Doses of these may need adjusting because of the risk of low blood sugar. They are not simply banned alongside Mounjaro, but the dose conversation is important.13

When it comes to what to avoid in daily life, the most useful principle is not to add unregulated products. Supplements marketed as natural alternatives are low-cost and available without assessment, with no clinical oversight, and dosing is not standardised. They are not a substitute for a licensed medicine, and combining unlicensed products with a prescription medicine without telling your prescriber is a poor idea.
Also avoid buying Mounjaro from anyone offering to supply without a consultation or prescription. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, so what you eat remains a central part of treatment.2 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Because the medicine also slows how quickly the stomach empties, many people find that smaller, simpler meals suit them better than large heavy ones.
People often ask what snacks are good on Mounjaro. Snacks that are filling and not heavy tend to sit better when your stomach empties slowly. Think of plain yoghurt, fruit, a handful of nuts, vegetables with a dip, or a boiled egg. Very rich, greasy or sugary snacks are the ones many people find harder to tolerate, though responses differ.
Some habits help regardless of your schedule:
- Eat slowly and stop when you feel comfortably full.
- Keep water to hand through the day.
- Plan meals around protein and vegetables, and do not skip meals entirely.
- Keep moving. Increased physical activity is part of the treatment, not an optional extra.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. They apply to treatment taken as licensed, alongside diet and activity. They say nothing about what happens on an altered schedule.
A balanced approach protects both your results and your wellbeing. If your relationship with food feels difficult, or you find yourself under-eating, say so to your prescriber or GP. Weight management should feel sustainable, and support exists if it does not.
10 · Holidays and travel
Holidays and travel without changing your schedule
If you can, plan to keep your usual weekly injection while you are away, and talk to your prescriber before you go if your trip complicates it. People often ask whether they can stop taking Mounjaro on holiday. Pausing is a decision for you and your prescriber, and it should not be an impulse. Remember that after a final dose, tirzepatide takes roughly four to five weeks to clear the system.1
Storage is the main practical issue. Unused pens belong in the fridge at 2–8°C until first use, and an in-use pen is licensed for up to 30 days (not above 30°C) out of the fridge.13 Think about where you will keep pens at your destination, and how you will carry them. A small insulated travel bag is an accessory purchase worth considering, and the exact choice is up to you.
Other points to think about before travelling:
- Carry pens and needles in your hand luggage, not your hold luggage, so they are not lost and are not exposed to extreme temperatures.
- Keep your prescription information or a note from your prescriber available in case you are asked about the medicine.
- Check rules for medicines at your destination.
- Pack a little extra sharps disposal capacity or a plan for disposing of needles safely.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Planning the cost sensibly is a better answer than stretching doses. A first year (13 pens) costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg, at standard prices. Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
