In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

Those are trial averages, and individual results vary. They also come from people using the medicine alongside a reduced-calorie diet and increased physical activity, not instead of them. The numbers do not mean you will match the average, and they are not a promise.

The same trial reports how many people reached a meaningful loss at each dose. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 In plain terms, most people on 5mg lost a clinically meaningful amount. Higher doses added to that, and the biggest losses, the ones reaching a fifth of body weight, were reported on the two higher doses.

So the fair reading is this: 5mg delivers a large part of the benefit for many people, while higher doses tend to deliver more on average. Whether the extra is worth it for you depends on your goals, your side effects and how you respond.

A sensible three-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.2

Early weeks are also noisy. Some people notice appetite change and early loss in the first week, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

You do not need to push on simply because the minimum interval has passed; if a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up.

What you should not do is improvise. Altering doses outside the licence means unknown dosing accuracy and no safety data.

Mounjaro is a once-weekly injection, given on the same day each week at any time of day, with or without food. Stretching it to a longer interval is unlicensed and not an approved route. If you want to change your injection day, you can do so as long as there are at least 3 days between two doses.

Serious side effects are uncommon but need urgent attention: for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen with fever or yellowing of skin or eyes, or signs of a severe allergic reaction, seek immediate medical help. Mounjaro works alongside habits, so what you build on 5mg is what you carry forward. Treatment is used alongside a reduced-calorie diet and increased physical activity. The routines you practise while appetite is quieter, such as regular protein, sensible portions and steady movement, are the part you keep if treatment ever changes.

If you stop altogether, the medicine does not vanish at once. Tirzepatide has a long half-life, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Planning ahead with your prescriber is wise instead of stopping abruptly.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
