Two pens cost the sum of the two strengths you choose. At standard prices, a 2.5mg pen is £169.99, 5mg is £218.99, 7.5mg is £279.99 and 10mg is £290.99. A 12.5mg pen is £319.99 and a 15mg pen is £329.99. All of these are per pen, which is one 4-week supply.

04 · Dose ladder limits

Why the dose ladder limits what you can order

The dose ladder is the main reason a two-pen order is not automatic. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg.¹ It can then step up by 2.5mg, at a minimum of 4-week intervals, through 7.5mg, 10mg and 12.5mg, to a maximum of 15mg weekly. How far you go depends on how you respond and tolerate it.¹¹

That means a pen of the next strength is only useful once a prescriber agrees you are ready to move up. A pen contains four weekly doses, so the earliest you could reach the next step is after one full pen at your current strength. Ordering a 2.5mg and a 5mg pen together, for example, only works if you have already agreed the plan. The 5mg pen would then sit unopened in the fridge for four weeks.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.¹ Because of this, buying a large stock of high-strength pens up front can be a mistake. If side effects mean you stay on a lower dose, the extra pens are not useful to you, and dispensed prescription medicines cannot be returned unless they are faulty, damaged or the wrong item.

What the review looks at

Reviews consider how you tolerated the last pen, your weight trend and whether the dose is still right. Response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹ A multi-pen order can sit awkwardly with that check-in, which is one reason prescribers weigh it carefully.

05 · How one pen works

One pen, four weekly doses

Each Mounjaro KwikPen is designed and licensed to deliver exactly four weekly doses.¹ That fixed design is what makes two pens simple arithmetic: two pens, eight doses, eight weeks at one injection a week. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.

The licensed injection sites are the stomach area, at least 5cm from the belly button, the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Needles, a sharps bin and injection guidance are included with every order.

Setting the dose

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

The day matters more than the hour. There is no best time of day, because Mounjaro works the same morning or evening. Many people choose a weekend morning so early side effects fall on days off. 06 · Storing a second pen

Storing a second pen safely

A second pen should go straight into the fridge, unopened. Keep Mounjaro pens at 2–8°C and never freeze them.¹ Unopened pens keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.¹³

The in-use pen

Once you start a pen, it can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips. After that it should be discarded. Keep the cap on to protect it from light. Never use a pen that has been frozen. Four weekly doses fit comfortably inside 30 days (not above 30°C), so an in-use pen should be finished before the limit arrives.

When you hold two pens, label which one is in use. The unopened one stays in the fridge, and the in-use one can travel in your bag if needed. Mixing them up is a real risk, particularly if they have different strengths. Check the strength on the label and confirm the dose in the pen window before every injection.

Travelling with spare pens

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing. Ideally use a cool bag with the prescription label or a copy of your prescription. For hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹

Holiday cover is a practical reason to ask your prescriber about a second pen. Because an in-use pen tolerates up to 30 days not above 30°C, most holidays need no special arrangements. Raise longer trips with your prescriber well ahead of time.

On delivery

Your pens arrive in temperature-controlled packaging by free next-day tracked delivery. Open the parcel the day it arrives and move the pens to the fridge. 07 · No doubling or sharing

Two pens never means two doses a week

Holding two pens does not change the dosing rule: one injection a week. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ A spare pen gives you no licence to catch up by injecting twice in a week.

Can two people use the same pen?

No. Treatment is prescribed to one person after their own consultation. A prescriber assesses BMI, medical history and current medicines before deciding if Mounjaro is suitable.¹ Someone else's suitability has not been assessed, and a pen is dispensed against one person's prescription. Ordering two pens for two people under one account is not a route we can recommend. Each person needs their own consultation.

08 · Cost planning

Planning the cost of several pens

Planning cost across pens is easier than it looks. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A new customer's first order is £40 less, and every later order is £25 less.

Your ongoing monthly cost depends on the dose you settle at. A first year (52 weeks, 13 pens) costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg, again at standard prices before discounts. For other maintenance doses, use that dose's monthly price: £279.99 at 7.5mg, £290.99 at 10mg or £319.99 at 12.5mg.

Why bulk buying rarely saves money

Because the discount structure is per order, with a first-order discount followed by a reorder discount on every future order, ordering two pens together does not inherently unlock extra savings. What matters more is avoiding wasted pens if your dose changes.

Buying a second pen safely

The risk with multi-pen buying is fakes, so verify the seller first. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.¹² Genuine treatment always involves a prescription and a clinical consultation. Anyone offering to supply without a consultation or prescription is a red flag, and a very large bundle at an unusually low price should make you more cautious, not less.

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date. It arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days (not above 30°C) in use.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹ Pregnancy: Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

11 · NHS and private routes

NHS and private supply: how pens are obtained

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Who qualifies privately

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.¹

How NHS supply compares for multi-pen requests

If you are treated through the NHS, supply follows the arrangements of your clinical team and local pharmacy, so ask them how many pens can be issued at a time. For private supply, the prescriber's review of each order applies, with ongoing monthly reviews built in. The monthly rhythm also protects you: a pen supply that outruns your review schedule can leave you on the wrong dose.

Moving between routes

If you move from private to NHS care, or the reverse, tell both teams which strength you take and when your last dose was. Do not assume a new provider will begin where the old one stopped. Cover that handover in your consultation.

What the evidence says about staying on course

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages and individual results vary. For someone starting at 100 kg, 20.9% is about 21 kg. Results build month by month, so steady, correctly dosed treatment is the point, not a big stockpile of pens.
