It depends on your treatment stage and on your prescriber, not on a fixed shop rule. A Mounjaro pen is a pre-filled KwikPen containing four weekly doses, so one pen is one four-week cycle.13 Two pens at the same strength would be eight weeks of injections. Two pens at different strengths would cover a dose step-up in the middle.

Everyone starts on 2.5mg once a week. After at least 4 weeks the dose rises to 5mg, and it can then step up by 2.5mg at a minimum of 4-week intervals, to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Because the right next dose is only known after you have lived with the current one, a prescriber may prefer to review you between pens. That review is the reason a second pen is not always sent with the first.

04 · What one pen contains

What one Mounjaro pen contains

One Mounjaro KwikPen contains four weekly doses, which is the starting point for any question about ordering more than one. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

The strength printed on the pen is the weekly dose, not the total in the pen. A 5mg pen therefore gives you four injections of 5mg, one a week.

Each pen is a self-contained four-week unit, so you always know where you are in the cycle. Two pens means eight weekly injections, and the third pen starts week nine. Keeping a simple log helps. Many people use the free GLPTrackr companion app (iOS and Android) to record injection days, doses and which pen they are on.

The injection itself is the same every week. Mounjaro goes under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area at least 5cm from the belly button, the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

There is no best time of day. What matters is picking a day and time you will remember and sticking to it. Many people choose a weekend morning so that early side effects fall on days off.13 If you change your injection day, make sure at least 3 days (72 hours) have passed since your last dose.

05 · Why dose steps matter

Why dose steps decide your second pen

Your second pen is usually decided by your dose step, because the dose can only rise after at least four weeks at the current one. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

In practice this means two sequential pens are often two different strengths. A 2.5mg pen followed by a 5mg pen is the standard opening. If you order both together, you would hold the 5mg pen unused for four weeks. That is not harmful in itself, but it fixes a plan before you know how the first month went.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.1 So the pens you need in the first four months are not a simple repeat. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A new customer's first order is £40 less, and every later order £25 less.

Response is also reviewed along the way. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

06 · Your first order

What to expect on your first order

On a first order you will almost always be supplied the 2.5mg starting pen, because every patient starts there. Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). The starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose. Both are normal.1

Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 This is the main reason a prescriber wants to see how you do on the first pen before committing you to the next one.

Eligibility comes first. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.1

What about people switching from another provider at a higher dose? The new-customer discount applies at any dose, so switching at 15mg means your first order is £289.99. Your prescriber will need to know your current dose and the date of your last injection, and may decide whether to continue at that dose or restart lower. That decision is clinical and it affects how many pens are appropriate.

As for results, think in months, not days. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2

07 · Repeat orders and maintenance

Repeat orders and maintenance planning

Repeat orders are where ordering more than one pen is most practical, because by then your dose is settled. Repeat orders re-check suitability, and nothing is auto-shipped, so each supply is a fresh clinical decision rather than a subscription. That protects you if your circumstances change, for example a new medicine, a new condition or side effects.

Once you reach a maintenance dose, each pen is simply a repeat of the same strength. At 5mg, the standard price is £218.99 per pen. At 10mg it is £290.99, and at 15mg it is £329.99. Every later order is £25 off the standard price. A first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg, at standard prices. These year totals count 13 pens over 52 weeks, and the discounts reduce them.

On the clinical side, trials measured outcomes at 72 weeks. At that point, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.2 Again, trial averages do not predict your own result, and treatment works alongside diet and activity.

08 · Storing more than one pen

Storing more than one pen

If you hold two pens, only the one in use may be out of the fridge, and only for up to 30 days at a temperature not above 30°C. Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.1

Because one pen delivers four weekly doses, a pen started on week one is used up by the end of week four, well inside the 30-day window. The spare pen stays sealed in the fridge. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13

A practical fridge tip: store the spare pen in its box, away from the back wall and the freezer compartment, where it could freeze. A frozen pen must not be used. Check the expiry date when each pen arrives, as holding pens for longer means expiry matters more.

Travelling is straightforward. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly.13

09 · Never stretch a pen

Why you should never stretch a pen or change how it is used

Buying more pens should never be a way to change how the medicine is used, and the safe rule is to use each pen as designed. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.

Talk to your clinical team rather than self-adjusting.

Use the pen as designed, then dispose of it.1

What if you miss a dose? Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.13 Holding a second pen does not change these rules.

10 · Counterfeits and bundle claims

Counterfeit risk when buying in bulk

Bulk or bundle offers are where fake pens do most harm, so verify the seller before you buy. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7

Only buy from a pharmacy you can verify on the GPhC register, and never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation. A seller offering to supply several pens without a consultation or prescription is a red flag, however attractive the price looks.7 Our registration number is 9012878.

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. 11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS and private supply

NHS and private: how pen supply differs

Mounjaro is available on the NHS, but access is being phased in over several years, so many people use regulated private treatment. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions. Full rollout is expected to take years.46

On the NHS, the clinical team sets the supply and review schedule, so the question of ordering two pens yourself does not arise in the same way.
