01 · The honest picture
NHS or private: how to get a prescription for weight loss injections
You can get weight loss injections on prescription through either the NHS or a regulated private service, and the right route depends on your health, your BMI and how long you are willing to wait. Both routes end in the same place: a prescriber decides whether the medicine is suitable for you.
On the NHS, Mounjaro is available, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.5
Privately, the licence is wider. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. It must be used alongside a reduced-calorie diet and increased physical activity.111

02 · Getting it online
How an online weight loss injection prescription works
An online prescription starts with a free health questionnaire and ends with a prescriber's decision, not an automatic approval. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
You can start your free consultation in a few minutes. A GPhC-registered prescriber then reviews your answers, rather than a scoring script. They look at your BMI, medical history and current medicines before deciding whether treatment is suitable.1 If the order is unsuitable it is refunded automatically in full.

Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99), and a pen lasts four weeks. A new customer's first order at any dose is £40 less, including people switching from another provider at a higher dose. Every later order is £25 off the standard price (code DOSE25).
The standard monthly price is £218.99 at 5mg, £279.99 at 7.5mg, £290.99 at 10mg, £319.99 at 12.5mg and £329.99 at 15mg. Switching at 15mg? Your first order is £289.99.

04 · Who is eligible
Who can get weight loss injections on prescription?
Adults who meet the licensed BMI criteria can be prescribed Mounjaro, subject to the prescriber's assessment. Privately, that means a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1
Meeting a BMI number is the start, not the end. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.11 That is why a genuine service asks health questions and can decline an order.
On the NHS, the picture is different. NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition, and the rollout began with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46

Which weight loss injections can be prescribed?
The main prescription weight loss injections in the UK are Mounjaro (tirzepatide), Wegovy (semaglutide) and liraglutide products such as Saxenda. They differ in how often you inject and in how they work.

Mounjaro is a once-weekly pre-filled multi-dose pen, and each pen contains four weekly doses.1 Tirzepatide activates two gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals.

06 · Will my GP prescribe
Will my GP prescribe weight loss injections?
Your GP may prescribe them if you fall within the current NHS stage, but many people will not yet qualify. Availability is phased, and the earliest groups are those with the highest clinical need.6
Ask your practice three things: whether they are prescribing Mounjaro yet, whether you meet the current criteria, and whether you would be referred to a specialist service instead. A GP who cannot prescribe yet is not refusing you; the rollout is still reaching your group.
If you are not eligible for the NHS now, a private prescription is lawful and does not stop you using NHS services later.
A regulated online service still involves a prescriber: they review your consultation, and the medicine is not supplied until they approve it.
07 · Without a prescription
Can you get weight loss injections without a prescription?
No. Mounjaro and other weight loss injections are prescription-only medicines, and a legitimate supplier must have a prescriber assess you first. Anyone offering to supply without a consultation or prescription is a red flag.
Counterfeit and unregulated pens are a real risk. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.7 Before you buy, check the pharmacy is on the GPhC register. 08 · What to expect
What to expect once you are prescribed
Everyone starts Mounjaro on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself. After at least four weeks the dose increases to 5mg, then can step up by 2.5mg at minimum four-week intervals, up to a maximum of 15mg weekly. Many people settle at 5mg, 10mg or 15mg; higher is not automatically better, and your prescriber guides the pace.1
It is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site each dose.13
How quickly does Mounjaro work? Weight loss is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Think in months, not days, and judge progress by trend, not individual weigh-ins.1 Some people notice appetite change in week one; others feel little until a higher dose, and both are normal. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages and individual results vary.2
Your prescriber reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.1
09 · Safe use basics
What to avoid while taking Mounjaro
Avoid changing your dose yourself, doubling up on a missed dose and letting the pen freeze. Your prescriber and the patient information leaflet are the source for anything beyond this.
- Missed dose: take it as soon as you can within 4 days (96 hours) of the scheduled time. After that, skip it and take the next one on your usual day. Never take two doses within 3 days of each other.13
- Storage: keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can stay out of the fridge not above 30°C for up to 30 days, then discard it.13
- Other medicines: tell your prescriber what you take. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, and doses of insulin or sulfonylureas may need adjusting because of low blood sugar risk.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
Report any suspected side effect to your prescriber and through the Yellow Card scheme.19 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
