What BMI do you need for weight loss injections?

You need a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. Treatment must be used alongside a reduced-calorie diet and increased physical activity.1

02 · NHS and private

NHS versus private access

Mounjaro is available on the NHS, but access is phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus a weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.4 Private treatment follows the licensed criteria. Either way, being under the threshold does not qualify you.

03 · Why prescribers say no

Why prescribers decline at a healthy weight

The licence and the trial evidence both relate to adults with overweight or obesity. In SURMOUNT-1, 2,539 adults on tirzepatide lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, versus 3.1% with lifestyle changes alone. Individual results vary.2

04 · Risks of taking it anyway

What happens if you take it without need

Mounjaro slows stomach emptying, and the most common early effects are mild nausea, altered appetite and tiredness in the first days after each injection.1 It can also affect how quickly some oral medicines are absorbed, so your prescriber needs to know everything you take.1

Sellers offering to supply without a consultation or prescription are a red flag. Buy only from a registered pharmacy, which you can check on the GPhC register. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. Report suspected side effects through the Yellow Card scheme.19

Start with your GP, who can look at diet, activity and any health concern behind your wish to lose weight. Low-cost supplements sold as "poor man's Ozempic" carry no clinical oversight, and dosing is not standardised, so they are no substitute for a licensed medicine. Can you get weight loss injections if you are not overweight?
Not on the licence. Mounjaro is licensed for a BMI of 30 or above, or 27 to 30 with a weight-related condition. A prescriber reviews BMI, history and medicines, and would decline a request at a healthy weight. Our free consultation can confirm whether you meet the criteria.
Who is not suitable for weight loss injections?
Anyone outside the licensed criteria, meaning a BMI under 27, or 27 to 30 without a weight-related condition. Suitability also depends on medical history and current medicines, which a prescriber reviews, including warfarin, insulin and sulfonylureas, where doses may need adjusting. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.
How heavy do you have to be to get weight loss injections?
It is about BMI rather than a set weight. You need a BMI of 30 or above, or 27 to 30 with a condition such as high blood pressure or prediabetes. NHS access is stricter: NICE recommends a BMI of at least 35 plus a weight-related condition.
Can a normal weight person take semaglutide?
Not as a licensed weight treatment. Wegovy is prescribed for weight management in adults with overweight or obesity, and a prescriber would not issue it at a normal BMI. If you are worried about your weight, speak to your GP.
Can I get Ozempic if I'm only slightly overweight?
Ozempic is licensed for type 2 diabetes, not weight loss. For weight management, slightly overweight means a BMI of 27 to 30, which only qualifies for Mounjaro with a weight-related condition. A prescriber decides after reviewing your history.
What is a cheaper alternative to Ozempic?
