Contraception while on Mounjaro

The pill is the one contraceptive that Mounjaro can affect. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.1

The SmPC advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 The MHRA publishes guidance on the same point.7

This applies to the pill only. Implants, coils and injections are unaffected.1

Because you step up the dose over several months, the extra-protection windows come around more than once.

  • The first 4 weeks after your first 2.5mg dose.
  • The 4 weeks after moving to 5mg.
  • The 4 weeks after each further step: 7.5mg, 10mg, 12.5mg and 15mg.

Note that the dose ladder starts at 2.5mg, then 5mg after at least 4 weeks, then steps of 2.5mg at minimum 4-week intervals up to 15mg.1 Tell your prescriber which method you use and they can advise on the safest approach for you.

05 · Planning to conceive

Planning a pregnancy after weight loss injections

If you want to conceive, stop Mounjaro at least one month before you start trying.1 Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

That clearance period lines up with the one-month guidance. Use contraception until you and your prescriber agree the timing is right, rather than stopping the injection and relying on luck.

Stopping treatment has consequences for weight: in SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

06 · Fertility and PCOS

Fertility, PCOS and the "Ozempic baby" idea

The term "Ozempic babies" describes unplanned pregnancies in people using weight loss injections. Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label.

Mounjaro is not licensed specifically for PCOS, but many women with PCOS meet the licensed weight-management criteria. Weight loss itself is a first-line recommendation that can improve insulin resistance, cycles and symptoms, and improving weight can restore ovulation.11

In practical terms, if your cycles were irregular and become more regular, you may ovulate when you previously did not. That is a reason to take contraception seriously from the start, even if you believed pregnancy was unlikely.

If fertility is a concern, raise it in your consultation so your prescriber can look at your history. Mention PCOS and any fertility plans at the outset.

07 · If you get pregnant

What happens if you get pregnant on treatment

If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.1

A sensible sequence looks like this:

  • Do not take another dose.
  • Confirm the pregnancy with a test and contact your prescriber, who can note it on your record.
  • Book an appointment with your GP or midwife so antenatal care starts early.
  • Keep your pen and packaging, as your clinician may ask which doses you took and when.

Mounjaro must not be used during pregnancy, so continuing is not an option.1 A GP or obstetric team can give you personalised advice and reassurance.

08 · Breastfeeding

Breastfeeding and weight loss injections

Current prescribing information indicates minimal transfer of tirzepatide into breast milk.1 That is not the same as saying everyone should use it while breastfeeding.

Discuss breastfeeding with your prescriber so the decision reflects your circumstances, including your baby's age, your own health and your reasons for wanting treatment. If you are planning another pregnancy after breastfeeding, mention that too, because it affects when you would need to stop.

09 · NHS or private route

NHS or private: getting treatment when pregnancy is on your radar

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years.
