No. Mounjaro (tirzepatide) is a prescription-only medicine, supplied only after a consultation reviewed by a GPhC-registered prescriber.¹ The same applies to Wegovy, Ozempic and the GLP-1 tablets. There is no over-the-counter version of any of them, and no legitimate pharmacy will sell them to you without a prescription.

What people often really mean by 'without a doctor prescription' is 'without the hassle of a GP appointment'. That is a different question, and the answer is more encouraging. A prescription can come from a registered prescriber working for a regulated online pharmacy, after a proper health review. Why GLP-1 medicines need a prescription

GLP-1 medicines need a prescription because they are powerful medicines with eligibility rules, side effects and interactions that a professional must check. Mounjaro is the brand name for tirzepatide, a once-weekly injection made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.¹

Tirzepatide activates two gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.¹ Most people feel fuller sooner and less hungry between meals. That is a real pharmacological effect, which is exactly why it should not be self-prescribed.

Licensing is also specific. Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹¹¹ A prescriber is the person who decides whether you fit those criteria.

05 · Spotting unsafe sellers

How to spot sellers who skip the prescription

Any seller offering to supply a GLP-1 without a consultation or prescription is operating outside the law and should be avoided. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.⁷¹⁹ Headlines about people buying injections from social media or unregulated websites exist for a reason: a pen with no clinical history behind it could contain the wrong contents or none of the active ingredient.

Warning signs include:

• No health questionnaire and no prescriber review at all.
• Dramatic weight-loss claims or promises of specific results.
• Pens sold as 'research' products, or described as not for human use.
• No GPhC registration number you can look up.
• Sales by direct message, or pressure to pay quickly by bank transfer.

06 · Supplements and 'natural' GLP-1

Supplements, 'natural GLP-1' and over-the-counter pills

Over-the-counter products can be bought without a prescription, but they are not GLP-1 medicines. Supplements such as berberine, sometimes marketed as 'nature's Ozempic', are low-cost supplements with no clinical oversight. They are not licensed for weight management in the way Mounjaro is.

Dosing of these products is not standardised, and there is no prescriber checking whether they suit your health or other medicines. 'Natural' does not mean free of side effects or interactions. A pharmacist can advise on anything you are considering.

Mounjaro's effect comes from tirzepatide acting on GIP and GLP-1 receptors.¹ A supplement that is described as 'like' a GLP-1 has not been shown to do the same thing, so be wary of any product that borrows the name of a prescription medicine to sell itself.

Over-the-counter supplements are not equivalent to prescription treatment, so ask your pharmacist or prescriber for advice. 07 · NHS or private route

NHS or private: which route to a prescription?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁵⁶ Full rollout is expected to take years.

That means an NHS prescription may be available to you, or may not yet be. Your GP is the right person to ask. For Wegovy, NHS access runs through specialist weight-management services, so ask your GP about local availability.

If you do not meet the NHS criteria yet, or you do not want to wait, a regulated private prescription is the alternative. The licensed criteria are broader than the NHS ones: a BMI of 30 or above, or 27 to 30 with a weight-related condition.¹ For NHS prescriptions, the standard English prescription charge is £9.90 per item, but that does not apply to private prescriptions.

You can use both routes in sequence if you wish. Some people start privately and later move to NHS care when they become eligible. Speak to your GP before switching so your records stay complete.

Which GLP-1 medicines can you get with a prescription?

The main prescription options in the UK are Mounjaro, Wegovy, the GLP-1 tablets and daily liraglutide. All of them are prescription-only.

• Mounjaro (tirzepatide): once-weekly KwikPen in six strengths, 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ See the Buy Mounjaro hub.

• Liraglutide (Saxenda and similar): supplied at 6mg per millilitre and given as a daily injection, unlike once-weekly Mounjaro and Wegovy.

Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. Do not compare trial results between different medicines as if they were head to head. Only a trial that tested two drugs directly can do that, so ask your prescriber which option fits your health and goals.

09 · What results to expect

What results to expect once prescribed

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks. The comparison group, using lifestyle changes alone, lost 3.1%.²

These are trial averages, and individual results vary. As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.² Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

This is why shortcuts matter. The SURMOUNT-1 results come from a licensed medicine, taken at known doses, under medical supervision. None of that applies to an unregulated pen or a supplement.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
