What the licence says about Mounjaro and diabetes

Mounjaro is the brand name for tirzepatide, a once-weekly injection licensed in the UK for weight management in adults and for type 2 diabetes.1 In type 2 diabetes it improves blood sugar control (HbA1c) and supports weight loss.8 A medicine licensed to treat a condition is not designed to cause it.

02 · Timing

Why a diagnosis can coincide with starting treatment

Many people who are prescribed Mounjaro for weight management already have weight-related conditions. The licence lists prediabetes and type 2 diabetes among the conditions that can support eligibility at a BMI between 27 and 30.111 So a person starting treatment is more likely than average to have blood sugar problems already, whether or not they have been diagnosed yet.

Starting treatment also tends to mean more contact with clinicians and more testing. That can bring an existing condition to light.

03 · Next steps

What to do if you are worried about diabetes

Tell your prescriber about any new or worrying symptoms. If you have severe, persistent stomach pain that may spread to your back, with or without vomiting, pain in the upper right abdomen, fever or yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing, get medical help straight away. Do not stop or change your dose on your own. If you have a diagnosis of type 2 diabetes, tell your prescriber and your usual diabetes team so care stays joined up.1

You can also report a suspected side effect through the MHRA Yellow Card scheme.19 Reports help regulators monitor safety, but a report is not proof of cause.

05 · How Mounjaro works

How Mounjaro works on blood sugar

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

That insulin response is why the same medicine is useful in type 2 diabetes. For weight management, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide.

06 · After stopping Mounjaro

Becoming diabetic after stopping Mounjaro

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

07 · Who it is licensed for

Who Mounjaro is licensed for

For weight management, Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.111 A prescriber assesses your BMI, medical history and current medicines first.

On the NHS, access is being phased in over several years. What the weight-loss trial showed

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 For someone starting at 100 kg, 20.9% is about 21 kg.

What should I avoid while taking Mounjaro?
Treatment is used alongside a reduced-calorie diet and increased physical activity. Tell your prescriber about all your medicines, as insulin or sulfonylureas may need dose adjusting, and oral contraceptive pills can be less well absorbed because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Your prescriber will advise on anything else specific to you.
What are the long-term effects of Mounjaro?
In SURMOUNT-1, average weight loss at 72 weeks was 15% on 5mg, 19.5% on 10mg and 20.9% on 15mg, with individual results varying. The most common side effects are digestive (nausea, diarrhoea, vomiting, constipation and abdominal discomfort), and for most people they ease within days to a few weeks. What is the age limit for using Mounjaro?
For weight management, the licence covers adults with a BMI of 30 or above, or 27–30 with a weight-related condition. A prescriber assesses your BMI, history and current medicines before deciding whether it suits you. No dose adjustment is needed for older adults, and trial participants included people over 65; experience in people aged 75 and over is more limited, so prescribers take extra care.
