How to get a diabetes drug for weight loss: your two routes
You can get it through an NHS referral or through a regulated private prescriber, and both need a prescription. Mounjaro is licensed in the UK for weight management in adults and for type 2 diabetes, so it is the medicine most people mean by the diabetes weight loss drug.1 There is no legitimate way to buy it without a prescriber assessing you first.
The NHS route starts with your GP or a specialist weight-management service. Mounjaro is available on the NHS, but access is being phased in over several years. What the diabetes drug for weight loss actually is
The medicine is Mounjaro, the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
It helps to understand why a diabetes medicine affects weight. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. That is the link between blood-sugar control and weight: the same hormone pathways are involved in both.
This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).
The medicine does not work on its own. It must be used alongside a reduced-calorie diet and increased physical activity, and a prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.1 That assessment is the reason you cannot simply buy it from a shelf or a website with no questions asked.
One more practical fact about how long it stays in the body. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 It is worth knowing this before you start, because it means any side effects or effects on appetite do not vanish the day after an injection.

If you already have type 2 diabetes, this matters even more: the same medicine is licensed for your condition, and treatment and monitoring are coordinated differently, so your diabetes team should know about any change.
05 · Who is eligible
Who qualifies: licensed criteria versus NHS criteria
There are two different sets of criteria, and mixing them up is the most common source of confusion. The licensed criteria set out what the medicine may be prescribed for. The NHS criteria are narrower because access is being phased in. Meeting the first does not automatically mean meeting the second.
Under the licence, Mounjaro is for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1 A prescriber then assesses your BMI, medical history and current medicines before deciding whether it is suitable.
On the NHS, NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years.
What does this mean in practice? Someone with a BMI of 33 and no other condition falls outside the stated NHS group but meets the licensed criteria for weight management. Someone with a BMI of 28 and high blood pressure meets the licence through the weight-related condition route. Someone with a BMI of 41 and several qualifying conditions sits in the group the NHS prioritised first. Only a prescriber can confirm suitability for you.
Type 2 diabetes deserves its own note. Mounjaro is also licensed for type 2 diabetes, where it improves blood sugar control (HbA1c) and supports weight loss.1 If you have diabetes, the question is often not how to get it but whether your diabetes team should be the ones prescribing it. Tell your prescriber and your usual diabetes team about any treatment so care stays joined up.
Honest limits apply too. The licence and our assessment still involve a review of your medical history and current medicines, and some people will be told it is not suitable. 06 · NHS route in detail
The NHS route: what to expect
The NHS route works, but it is gradual, and your own wait depends on which group you fall into. Mounjaro is available on the NHS, but access is being phased in over several years.6 Rather than a single launch date for everyone, the rollout reaches people in stages, with the highest-need groups first.
NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition.4 The primary-care rollout began in June 2025, initially for people with a BMI of 40 or above and four or more qualifying conditions. To start, book an appointment with your GP and ask whether you are in a group currently being offered treatment locally. Bring your height and weight, a list of your current medicines, and any weight-related conditions you have been diagnosed with, such as high blood pressure, high cholesterol, sleep apnoea or prediabetes. NHS prescribing for weight management also comes with lifestyle support, and treatment is used alongside a reduced-calorie diet and increased physical activity.1 Wegovy is a different medicine; NHS access to Wegovy runs through specialist weight-management services, so ask your GP.
If you do not meet the current NHS group, you are not out of options, and you are not doing anything wrong. 08 · Dose ladder and results
The dose ladder and realistic results
Everyone starts on 2.5mg once a week, and the dose rises slowly. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.
What do the trials show? In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
Other trial figures at 72 weeks: 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example only, for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. Trial averages do not predict your own result.
Pace matters as much as the numbers. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.1
Experiences in the first weeks differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13
Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

10 · Safety and supply
Safety, counterfeits and where not to buy
The safest way to get this medicine is through a regulated pharmacy that requires a consultation and a prescription. Counterfeit and unlicensed weight-loss injections have been the subject of MHRA alerts, and the MHRA also runs the Yellow Card scheme for reporting side effects.7 Treat any seller offering to supply without a consultation or prescription as a warning sign, and any seller promising specific results the same way.
Check the pharmacy. Every UK pharmacy that sells medicines online should appear on the GPhC public register, which you can search by name or number.12 Ours is registration 9012878. Know how the medicine is used. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.13
Know the missed-dose rule. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other. You can change your injection day as long as at least 3 days (72 hours) have passed since your last dose.13
Storage is simple. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.13 If you travel, carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription. If an unopened pen has been out of the fridge, check with the pharmacy before using it.
Report concerns. 11 · Other drugs people ask about
Metformin, Ozempic and other "diabetes drugs" people ask about
Several different medicines get called diabetes drugs, so it is worth separating them. For questions like how much metformin it takes to lose weight, or how quickly a given amount comes off on Ozempic, your prescriber is the right person to ask.
Metformin is a long-established diabetes medicine. Questions about whether it helps with belly fat, how it feels, or what happens if you eat sugar while taking it are best answered by your prescriber or pharmacist, who know your own medicines and history.
Ozempic and Wegovy both contain semaglutide, a single-action GLP-1 medicine. Mounjaro differs because it activates two gut-hormone receptors, GIP and GLP-1.1 Wegovy's licensed dose ladder runs 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least 4 weeks and reaching the 2.4mg maintenance dose from week 17.18 Ozempic is licensed for type 2 diabetes only, and it is not a UK weight-loss medicine; the licensed weight-management options are Mounjaro and Wegovy. Which suits you depends on your history and your prescriber's advice.
Liraglutide (Saxenda, and products such as Erly) is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy.
If you take insulin or a sulfonylurea, doses may need adjusting because of low blood sugar risk, so tell your prescriber and your diabetes team before starting. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
